    Abstract: Pharmacogenetics helps us understand the relationship between an individual’s genetic make-up and the way medicines work for each person. This book reviews the use of pharmacogenetics across all stages of the health innovation cycle from research through to uptake by doctors and patients. It focuses on how to optimise the use of pharmacogenetics to deliver effective innovations for public health, and design policies that enhance their economic and social benefits. The book argues for large-scale studies to validate the biomarkers that underpin pharmacogenetics and policies to share the cost and risk of using pharmacogenetics to improve the use of existing medicines. Governments and others need to align regulatory, reimbursement and other incentives and work with industry to measure better the impacts of pharmacogenetics. Health systems need to take positive steps to adapt to the use of pharmacogenetics and ensure that health professionals receive adequate training.

    Abstract: The report analyses the retirement income systems of 18 Asian countries, including Australia, China, India, Indonesia, Pakistan, the Philippines and Vietnam. It says that reform is needed because: coverage of formal pension systems is relatively low; withdrawal of savings before retirement is very common; pension savings are often taken as lump sums and often do not provide people with adequate income over their lifetime; pensions payments are not automatically adjusted to reflect changes in the cost of living. In OECD countries, an average of 70% of the working-age population are eligible for a pension. However in South Asia, just 7.5% of the working-age population are eligible and in East Asia 18%. Furthermore, few countries in Asia/Pacific have social pensions to provide safety-net retirement incomes for people who are not members of formal schemes. Only in India are social pensions significant, with around 10 to 15% of older people covered.

    Abstract: This report analyses in detail the implications of recent developments in Chile's labour market and social policy and considers the available policy options from the perspective of OECD countries’ experience. The report finds that Chile has enjoyed rising living standards over two decades of strong economic growth. The incidence of poverty is now much lower and there is better access to adequate housing, education and healthcare. Nevertheless, Chile’s income distribution remains disturbingly unequal by OECD standards. This is partly due to Chile’s a relatively low employment rate, especially for women, but it also reflects a segmented labour market, where much of the recent job creation has occurred in relatively low-productive sectors. Moreover, despite the existence of an internationally renowned pension programme, Chile’s social protection system as a whole has still a relatively long way to go before reaching the standards of developed countries in terms of effective coverage and capacity to assist needy households. Chilean policy makers have begun to develop and implement a series of ambitious reforms, intended to promote the twin goals of work and equity.

    Abstract: This publication demonstrates that natural resources can contribute to growth, employment, exports and fiscal revenues in low-income countries, where natural capital constitutes a quarter of total wealth. Part I provides an overview of the economics and politics of natural resources.     Abstract: This report analyses the main challenges for labour market and social policies in Slovenia and considers the available policy options from the perspective of OECD countries' experience. It finds that Slovenia has one of the most equal income distributions in the world and a tradition of social dialogue. However, its unemployment insurance and employment service system are not sufficient to deal effectively with the present economic crisis and the country is also faced with rapid population ageing and a low employment rate among 55-to-64 year olds. This report makes recommendations to deal with these issues.

    Abstract: This paper examines aspects of the policy environment and market characteristics of Slovakia's pharmaceutical sector, and assesses the degree to which Slovakia has achieved certain policy goals. Pharmaceutical expenditure in Slovakia accounts for a higher share of total health expenditure than it does in any other OECD country, and the share of national income going to pharmaceuticals is exceeded only in Hungary. Although its relatively low national income is a partial explanation for Slovakia's status in this respect, this review finds that Slovakia has scope to reduce its expenditures and the rapid rate of growth in its pharmaceutical spending. Financing of pharmaceutical expenditure in Slovakia rests more heavily on the public sector than is typical in the OECD, with out-of-pocket spending accounting for just a quarter of total expenditure. The effectiveness of international price referencing in limiting Slovak prices for on-patent pharmaceutical products is questionable. For products that have gone off-patent and for those with similar chemical structure, a reference-pricing scheme and competition among generic alternatives results in effective price control, although incentives for generic substitution are weak (for patients) and misaligned (for pharmacists). When deciding whether a drug will be reimbursed through the social insurance scheme, the cost-effectiveness of new pharmaceuticals is not assessed. On the other hand, certain policy goals have been achieved. The accessibility and availability of medicines--including the most innovative products--is good; affordability is supported by relatively low average co-payment levels. While more expensive drugs usually have higher cost-sharing, drugs are not excluded from coverage on affordability grounds.

    Abstract: This publication discusses the dimensions, significance, and policy implications of international flows of human resources in science and technology. The international mobility of highly skilled workers is increasing in scale and complexity as more economies participate in R&D and innovation activity. Mobile talent diffuses knowledge both directly and indirectly across borders. This can boost global innovation performance, with benefits accruing to both sending and receiving countries. It is clear that mobility is leading to an increasing level of labour-market internationalisation and integration, and competition for talent is now influencing innovation policy initiatives across the globe. Most countries offer a range of policies focused on assisting and encouraging mobility, although few have a specific and coherent mobility strategy. Many nations aim to attract the same pool of highly skilled talent; thus, relying on international flows to fill existing or future gaps in supply may entail risks. Addressing shortcomings in national policies that may limit domestic supply of skilled workers, and ensuring that the wider environment for innovation and scientific endeavour is sound, are key policy challenges for countries.

Labour regulations must be flexible, but they should also be enforced more consistently. For all this to happen, it is essential that an effective social dialogue can take place and that it encompasses expanding and declining segments of the labour market.

    Abstract: When immigrants arrive in a new country, they are confronted with new labour market requirements such as language proficiency, familiarity with job search procedures and work practices which they are not always able to satisfy. These obstacles affect not only new immigrants, but, surprisingly, their children too, even if the children are born and educated in the receiving country. This publication presents reviews of the labour market integration of immigrants and their children in four OECD countries (Belgium, France, the Netherlands and Portugal), and provides country-specific recommendations. Governments have a role to play in promoting language and vocational training, and encouraging diversity in the workplace. Immigrants themselves must accept the requirements of the host country employers. The viability of future migration policies, in particular greater recourse to immigration, will depend to a large extent on how successful OECD countries and immigrants are in achieving these objectives.

    Abstract: Pharmaceutical policy making raises particular challenges in reconciling key objectives for health policy, such as ensuring affordable access to the latest effective drugs, with other important policy considerations, such as providing support to a valuable national industry. Unusually among health policy issues, it also raises international considerations that further complicate decision making, particularly as the nature and extent of such considerations are not well understood. This report assesses how pharmaceutical pricing and reimbursement policies have contributed to the achievement of certain health policy objectives. It examines the national and transnational effects of these policies, in particular, their implications for the availability of medicines in other countries, the prices of these medicines, and innovation in the pharmaceutical sector. This publication presents an analysis of comparative price levels, making use of a unique dataset to construct the most comprehensive pan-OECD pharmaceutical price index to date. It also draws upon original case studies of pharmaceutical pricing and reimbursement policies in six OECD countries to provide specific examples of the impacts of policies on health system performance.

    Abstract: This report examines the role played by immigrant health workers in the Canadian health workforce but also the interactions between migration policies and education and health workforce management policies. Migrant health worker makes a significant contribution to the Canadian health workforce. Around 2005-06, more than 22% of the doctors were foreign-trained and 37% were foreign-born. The corresponding figures for nurses are close to 7.7% and 20%, respectively. Foreign-trained doctors play an important role in rural areas as they contribute to filling the gaps. In most rural areas, on average, 30% of the physicians were foreign-trained in 2004. Over past decades the evolution of the health workforce in Canada has been characterised notably by a sharp decline in the density of nurses and a stable density of doctors, which is in contrast with the trends observed in other OECD countries. This evolution is largely the result of measures were adopted at the end of the 1980s and early 1990s in order to address a perceived health workforce surplus.

    Abstract: Growing Unequal? brings together a range of analyses on the distribution of economic resources in OECD countries. The evidence on income distribution and poverty covers, for the first time, all 30 OECD countries in the mid-2000s, while information on trends extending back to the mid-1980s is provided for around two-thirds of the countries. The report also describes inequalities in a range of domains (such as household wealth, consumption patterns, in-kind public services) that are typically excluded from conventional discussion about the distribution of economic resources among individuals and households. The report provides evidence of a fairly generalised increase in income inequality over the past two decades across the OECD, but the timing, intensity and causes of the increase differ from what is typically suggested in the media. Precisely how much inequality there is in a society is not determined randomly, nor is it beyond the power of governments to change, so long as they take note of the sort of up-to-date evidence included in this report.     Abstract: As the number and share of the population aged 65 and over will continue to grow steadily in OECD countries over the next decades, improvements in the functional status of elderly people could help mitigate the rise in the demand for, and hence expenditure on, long-term care. This paper assesses the most recent evidence on trends in disability among the population aged 65 and over in 12 OECD countries: Australia, Belgium, Canada, Denmark, Finland, France, Italy, Japan, the Netherlands, Sweden, the United Kingdom and the United States. The focus is on reviewing trends in severe disability (or dependency), defined where possible as one or more limitations in basic activities of daily living (ADLs, such as eating, washing/bathing, dressing, and getting in and out of bed), given that such severe limitations tend to be closely related to demands for long-term care. One of the principal findings from this review is that there is clear evidence of a decline in disability among elderly people in only five of the twelve countries studied (Denmark, Finland, Italy, the Netherlands and the United States). Three countries (Belgium, Japan and Sweden) report an increasing rate of severe disability among people aged 65 and over during the past five to ten years, and two countries (Australia, Canada) report a stable rate. In France and the United Kingdom, data from different surveys show different trends in ADL disability rates among elderly people, making it impossible to reach any definitive conclusion on the direction of the trend. One of the main policy implications that can be drawn from the findings of this study is that it would not be prudent for policymakers to count on future reductions in the prevalence of severe disability among elderly people to offset completely the rising demand for long-term care that will result from population ageing. Even though disability prevalence rates have declined to some extent in some countries, the ageing of the population and the greater longevity of individuals can be expected to lead to increasing numbers of people at older ages with a severe disability and in need of long-term care. The results of the projection exercise to 2030 for all countries, regardless of different trends in disability prevalence, confirm this important finding.

    Abstract: The impact of four labour market policies – employment protection legislation, minimum wages, parental leave and unemployment benefits – on productivity is examined here, using annual cross-country aggregate data on these policies and industry-level data on productivity from 1979 to 2003. We use a "difference-in-differences" framework, which exploits likely differences in the productivity effect of policies in different industries. Our identifying assumption is that a specific policy influences worker or firm behaviour, and thereby productivity, more in industries where the policy in question is likely to be more binding than in other industries. The advantage of this approach is twofold. First, as in standard cross-country analysis, we can exploit the cross-country variation of policies. Second, in contrast with standard cross-country analysis, we can control for unobserved factors that, on average, are likely to have the same effect on productivity in both policy-binding and non-binding industries.

    Abstract: This report discusses methods of measuring unauthorized migration to the United States. The “residual method” involves comparing an analytic estimate of the legal foreign-born population with a survey-based measure of the total foreign-born population. The difference between the two population figures is a measure of the unauthorized migrant population in the survey; it can then be corrected for omissions to provide a measure of the total unauthorized population. The report includes a detailed description of the residual methods and the underlying data and assumptions as it has been applied to recent data from the Current Population Survey (CPS) and decennial censuses. The paper presents new results of estimates derived from the march 2006 CPS which show that the unauthorized population in the U.S. has reached 11.5 million; of these, 6.5 million or 57% are from Mexico. The report also presents derived data on a range of social and economic characteristics of the unauthorized population developed with an extension of the residual estimates. Finally, historical data on trends in unauthorized migration and several alternative estimation methods are presented and discussed.

    Abstract: This report is an update to the OECD Health Working Paper No. 22, Health Care Quality Indicators Project: Initial Indicators Report that was based on data collected between 2003 and 2005 and released in 2006. That report presented the OECD’s initial work on developing a set of health care quality indicators that could be used to raise questions about differences in quality of care across countries. The 2006 report covered 21 “initial indicators” with data provided by 24 countries. It identified 17 of these indicators as being fit for international comparisons of which 4 were identified as needing further work. Following the release of that report in March 2006, the OECD undertook a second round of data collection on the initial indicator set and also gathered data for the first time on new indicators in a questionnaire sent to participating HCQI countries. This paper reports on the results of that second round of data collection. Data is presented here on an augmented indicator set considered fit for the purpose of making international comparisons on quality of health care. The data is comprised of 19 indicators (17 initial indicators plus 2 new ones).     Abstract: When immigrants arrive in a new country, they are confronted with new labour market requirements such as language proficiency, familiarity with job search procedures and work practices which they are not always able to satisfy. These obstacles affect not only new immigrants, but, surprisingly, their children too, even if the children are born and educated in the receiving country. This publication presents reviews of the labour market integration of immigrants and their children in four OECD countries (Australia, Denmark, Germany and Sweden), and provides country-specific recommendations. Governments have a role to play in promoting language and vocational training, and encouraging diversity in the workplace. Immigrants themselves must accept the requirements of the host country employers. The viability of future migration policies, in particular greater recourse to immigration, will depend to a large extent on how successful OECD countries and immigrants are in achieving these objectives.

    Abstract: Institutions whose goal is to help those at the margins have been a feature of social life for centuries. Today, social assistance institutions have integration and encouragement as their goal. They seek to avoid exclusion and stigmatisation.Poverty can tear at the fabric of society. However, preventing hardship among those with no resources while reducing exclusion and marginalisation is no easy task. How can social assistance best balance these goals, minimising disincentives to paid employment? What can be done to promote independence and individual responsibility?This book compares the social assistance policies of Belgium, the Czech Republic, the Netherlands and Norway. Although at first sight these countries appear very different, in fact the same policy dilemmas exist in the four countries. That local action is necessary to identify and help the excluded is accepted, but the balance between ensuring that sufficient resources are available at the local level while limiting budgets so as to ensure efficient use of resources is a continuing preoccupation. Governments stress their commitment to combat poverty, but benefits cannot be raised too high because this would harm work incentives. Different institutions sometimes disagree about whether benefit recipients are "job ready" or whether more social help is needed. No country has yet found a policy package which prevents exclusion, but innovative policies in each of these four countries are proving successful in limiting its extent.

    Abstract: This paper examines aspects of the policy environment and market characteristics of the Swedish pharmaceutical sector, assesses the degree to which Sweden has achieved certain policy goals, and puts forth some key findings and conclusions. Thanks to low mark-ups in the distribution chain and no VAT for prescribed medicines, Sweden's public prices for pharmaceuticals are relatively low, in contrast to average prices received by manufacturers, which are among the highest in Europe. Recent reforms have helped to restrain pharmaceutical expenditure growth, following a period of double digit growth in the 1990s. Pharmaceutical expenditure per capita in Sweden is lower than the OECD average. Only five OECD countries devote less of their national income to pharmaceuticals. What limited evidence exists tends to suggest that relatively low pharmaceutical expenditures in Sweden are due to its low public prices, rather than to low levels of consumption. Sweden introduced a new pricing and reimbursement scheme in 2002. Its main features are the use of cost-effectiveness analysis for determining the reimbursement status of new pharmaceuticals and mandatory substitution of the lowest-cost generic alternative. The use of cost-effectiveness analysis in reimbursement decisions helps to relate the reimbursement price paid to the social value of the product, but does not necessarily result in the lowest possible price.The generic substitution policy has enabled Sweden to achieve fairly high penetration of generic drugs into the market in terms of volume, with a considerably low share of the total value of the market.     Abstract: Korea faces an extraordinary ageing challenge. Korea will age much faster than other OECD countries: in 2000, about 7% of Korea’s population were over 65; in 2050, senior citizens will constitute about 37% of Korea’s population. Population ageing will unfold at high speed, firstly because of the dramatic increase in life expectancy from just over 52 years in 1960 to over 77 years in 2004. a major cause of this was the spectacular decrease in infant mortality rates from 45 infants per 1000 live births in 1970 to 5.3 in 2002. The second cause of population ageing is the sharp decline in birth rates from close to 3 children per woman in 1975 to less than 1.2 in 2004. Reduced child mortality rates and increased life expectancy are indicators of the success of the Korean economy and society. However, the decline in fertility rates in Korea is evidence of strains in society which will damage prosperity in the future. Population ageing on this scale will inevitably lead to a huge increase in spending on old-age income support and health care, and will also require the development of a public family policy which supports the reconciliation of work and care commitments of workers. To successfully meet the ageing challenge, the OECD believes that three policy objectives must be targeted. First, the decline in the working-age population needs to be slowed. Second, working opportunities need to be extended. Third, affordable and sustainable pension and health care policies must be implemented. This volume looks at existing Korean family, health and pension policies from an international perspective, considers them in view of the emerging policy challenges, and outlines some of the policy options that are available to policy makers in Korea.

    Abstract: The current situation of the labour market integration of migrants in Germany has to be viewed in the light of its immigration history. During the post-war economic boom, until 1973, Germany focused on the recruitment of low-skilled foreign labour. Many of these “guestworker” immigrants settled and were joined by their foreign spouses, which has given rise to a second generation of persons with an immigrant background. In the late 1980s and early 1990s, Germany received massive immigration flows of ethnic Germans from Eastern Europe. Shortly after the peak immigration of ethnic Germans, Germany received large numbers of humanitarian migrants. German statistics only distinguish along nationality lines. This hampers assessment of the situation as this does not take account of ethnic Germans – who have German nationality and are now the most important immigrant group, although they face difficulties similar to those of other migrant groups.     Abstract: This report presents the results of a survey of over 800 genetic testing laboratory directors in 18 OECD countries. It provides the first detailed overview of the availability and extent of molecular genetic testing across OECD member countries. The survey allowed the OECD to compare practices in individual countries in order to inform international action in setting standards and developing guidelines for practice. Based on the survey results, the report puts forward recommendations for action for better quality assurance and proficiency of molecular genetic testing. It shows, for example, that requirements for licensing and accreditation/certification of diagnostic molecular genetic testing laboratories have not penetrated OECD countries to a high degree or with any consistency. Considerable variations exist in mechanisms of licensing, certification and accreditation, including the standards by which tests are performed, results are reported, and the qualifications for laboratory personnel.

    Abstract: This paper examines aspects of the policy environment and market characteristics of Mexico's pharmaceutical sector, and assesses the degree to which Mexico has achieved certain policy goals. This paper questions the effectiveness of the maximum price regulation. It notes that retail prices for pharmaceuticals are relatively high, although proximity to the United States may have some influence. Although not wholly successful in containing overall drug expenditures, the federal government can claim some measure of success for the public sector market. A high reliance on out-of-pocket spending brings into question the sustainability of financing pharmaceuticals in Mexico. It also contributes to greater inequality, although a new health insurance scheme, the Seguro Popular, is addressing the latter with some success as it endeavours to provide coverage for the half of Mexico's population without health insurance. Finally, the paper acknowledges the government.s efforts in improving efficiency of expenditures and quality of care through new bioequivalency requirements for generics. However, an unintended side-effect of the loss of low cost, non-bioequivalent drugs may be higher average prices for pharmaceuticals.

    Abstract: This paper examines aspects of the policy environment and market characteristics of the Swiss pharmaceutical sector, and assesses the degree to which Switzerland has achieved certain policy goals. In Switzerland, pharmaceutical spending has not been growing faster than health expenditure as a whole, as has been the case in many other OECD countries. Swiss pharmaceutical spending per capita and as a share of GDP is modest by OECD standards. This in part reflects relatively low levels of pharmaceutical consumption, given that public prices are among the highest in Europe and the Swiss tend to be early adopters of new pharmaceutical products. Switzerland’s regulation of prices for reimbursed drugs, based on referencing across countries and within the therapeutic class for products with comparators, appears to result in prices lower than what would be obtained absent regulation. Although ex-manufacturer prices are somewhat high relative to other European countries, recent reforms have reduced the differential. While costs are under control, Switzerland has scope to improve the cost-effectiveness of its expenditures in the pharmaceutical area. Generic penetration of the market is increasing but falls short of what has been achieved elsewhere and the prices of generic products are higher than what is found in other countries. Relatively high mark-ups over ex-factory prices suggest that the distribution chain is a source of further potential efficiencies, although high costs could also reflect characteristics of the Swiss economy...

    Abstract: Globalisation is having important effects on labour markets in OECD countries. The global supply of labour has increased enormously with the emergence of China and India. At the same time technological advances have contributed to heightened income inequality and changed the nature of globalisation itself, most vividly demonstrated by the rapid growth of offshoring of business services that were previously nontradable. It is argued in this paper that these developments are best characterized as an intensification and broadening of the process of globalisation rather than a fundamental change in the nature of globalisation. They will, nevertheless, have long-lasting effects on OECD labour markets, increasing the urgency of implementing the labour market policies set out in the Restated OECD Job Strategy. The paper concludes that the most important implication of the emergence of China and India in the context of widespread perceptions of increasing economic inequality may be to reduce support for globalisation in OECD countries.

    Abstract: Recent social security reform has significantly improved the long-run sustainability of the pension system. However, the pension system continues to serve as an important barrier to a more rapid expansion of the formal-sector economy in two ways. First, early-retirement incentives (including severance payments) continue to push many incumbent formal sector workers into the informal sector, often at ages as young as 40-45. While new labour force entrants face a much higher retirement age, policies for incumbents are fiscally expensive, inequitable, and serve to swell the ranks of the informal sector. Second, even when the transition to the new pension rules is complete, net replacement rates will remain very high by OECD standards, requiring high social security contribution rates that make it too expensive for firms to employ low-skilled labour in the formal sector. Thus, further pension reform is one of the keys to overcoming Turkey’s economic duality. Finally, since the pension system does not cover the informal sector, it does little to alleviate poverty among the wider population of older people. This paper discusses a number of reforms that would increase the retirement age, reduce inter-generational inequities, and permit a significant cut in the tax wedge on labour, while better addressing old-age poverty concerns at all levels of income.

    Abstract: This review of early childhood education and care (ECEC) in twenty OECD countries describes the social, economic, conceptual and research factors that influence early childhood policy. These include increasing women’s labour market participation; reconciling work and family responsibilities on a more equitable basis for women; confronting the demographic challenges faced by OECD countries; and in particular, addressing issues of access, quality, diversity, child poverty and educational disadvantage. Starting Strong II outlines the progress made by the participating countries in responding to the key aspects of successful ECEC policy outlined in the previous volume, Starting Strong (OECD, 2001). It offers many examples of new policy initiatives adopted in the ECEC field. In their conclusion, the authors identify ten policy areas for further critical attention from governments. The book also presents country profiles, which give an overview of ECEC systems in all 20 participating countries.

    Abstract: Speeding is the number one road safety problem in a large number of OECD/ECMT countries. It is responsible for around one third of the current, unacceptably high levels of road fatalities. Speeding has an impact not only on accidents but also on the environment, energy consumption and quality of life, particularly for residents in urban areas. Reducing average speeds on the roads by only 5% will save around 20% of current fatalities. There is good experience available on how to quickly reduce the extent of speeding and thereby reduce current fatalities and injuries. Reduced speeding will also reduce the adverse environmental and social impacts associated with excessive speed, particularly in urban areas. Comprehensive measures are required which are best developed as part of a coordinated speed management policy package. What contributions can be made by infrastructure improvements, speed limits, signing and education, as well as by enforcement? Which are the most cost-effective elements of a speed management policy? What are the prospects for the use of new technologies which encourage drivers to choose appropriate speeds and assist in achieving greater compliance with speed limits? This Report, prepared by a Working Group of the Joint OECD/ECMT Transport Research Centre, addresses the key issues, highlights the improvements in policy and operations needed to reduce the extent of speeding. It outlines a framework for achieving the best possible safety outcomes while protecting the environment and supporting sustainable mobility. The report is compelling reading for all those interested in addressing the major speed-related problems that are widespread in OECD/ECMT member countries and will become increasingly important in developing countries as levels of motorisation increase.

    Abstract: All discussions about the desirability of policy reforms rest on judgements about their effects on individuals and societal well-being. Yet, suitable measures for assessing how well-being is changing over time or compares across countries are lacking. This problem is, of course, not new and standard economic theory has provided, over the years, a range of insights about the criteria and domains that are most critical for the measurement of well-being, and on the relation between well-being and measures of economic resources. This paper does not revisit this theoretical discussion, nor does it provide a comprehensive review of different approaches to the measurement of well-being. It rather assesses whether GDP per capita is an adequate proxy as a measure of well-being or whether other indicators — used either as substitutes or as complements to GDP per capita — are more suitable for that purpose. Attention is limited to only some of the factors that influence well-being, and excludes some critical elements such as the environment, home production and other non-market factors.

    Abstract: According to Becker [1964], when labour markets are perfectly competitive, general training is paid by the worker, who reaps all the benefits from the investment. Therefore, ceteris paribus, the greater the training wage premium, the greater the investment in general training. Using data from the European Community Household Panel, we compute a proxy of the training wage premium in clusters of homogeneous workers and find that smaller premia induce greater incidence of off-site training, which is likely to impart general skills. Our findings suggest that the Becker model provides insufficient guidance to understand empirical training patterns. Conversely, they are not inconsistent with theories of training in imperfectly competitive labour markets, in which firms may be willing to finance general training if the wage structure is compressed, that is, if the increase in productivity after training is greater than the increase in pay.

    Abstract: The OECD Health Care Quality Indicator (HCQI) Project was started in 2001. The long-term objective of the HCQI Project is to develop a set of indicators that can be used to raise questions for further investigation concerning quality of health care across countries. It was envisioned that the indicators that were finally recommended for inclusion in the HCQI measure set would be scientifically sound, important at a clinical and policy level and feasible to collect in that data would be available and could be made comparable across countries. It was also envisioned that the indicators would not enable any judgement to be made on the overall performance of whole health systems.     Abstract: This paper explores the impact of policies and institutions on employment and unemployment of OECD countries in the past decades. Reduced-form unemployment equations, consistent with standard wage setting/pricesetting models, are estimated using cross-country/time-series data from 21 OECD countries over the period 1982- 2003. In the “average” OECD country, high and long-lasting unemployment benefits, high tax wedges and stringent anti-competitive product market regulation are found to increase aggregate unemployment. By contrast, highly centralised and/or coordinated wage bargaining systems are estimated to reduce unemployment. These findings are robust across specifications, datasets and econometric methods. As policies and institutions affect employment not only via their impact on aggregate unemployment but also through their effects on labour market participation - particularly for those groups “at the margin” of the labour market, group-specific employment rate equations are also estimated. In the “average” OECD country, high unemployment benefits and high tax wedges are found to be associated with lower employment prospects for all groups studied, namely prime-age males, females, older workers and youths. There is also evidence that group-specific policy determinants matter, such as targeted fiscal incentives.

    Abstract: This paper discusses how social disadvantage affects the learning experiences of households with fewer economic resources, at each stage of the individuals' life-course, and on some of the "social" effects of such learning. It argues that while education can be an escalator out of social disadvantage — leading to better job prospects for youths facing greater risks of poverty and reducing the prevalence of income poverty in adult age — educational failure can reinforce it: a significant minority of students in several OECD countries do not even complete compulsory education; students' test scores in lower secondary education are strongly shaped by family characteristics; and the expansion of university education has most often benefited households with better educated parents. Far from "equalising" opportunities, education can be a powerful driver of social selection. When returns to education increase over time, this may lead to greater inter-generational persistence of poverty and less equality of opportunities.

    Abstract: The relationship between environment and children’s health has been the subject of increasing interest these last ten years. For example, many OECD member countries are reporting asthma epidemics exacerbated by air pollution: in the United States nearly 1 in 13 school-age children (approximately 4.8 million) has asthma, and the rate is increasing more rapidly in school-age children than in any other group. The importance of this issue has resulted in a growing number of epidemiological studies aiming at better understanding and better characterising the relationship between environmental pollution and the health of children. However, in many respects, the valuation of children’s health strongly differs from the valuation of adults’ health and constitutes a real challenge for analysts as well as for decision-makers. Consequently, this book proposes an in depth analysis of the main methodological difficulties associated with estimating the social value of a reduction in risk to children. Questions such as how to elicit children’s preferences, what valuation methodology and benefit measure to choose, how to discount benefits to children’s health, and how to account for economic uncertainties in this specific context of economic valuation will be systematically examined in order to define key policy implications and to pave the way for further research.

    Abstract: Confidence in a country’s health care system requires indemnification and deterrence systems that adequately cover liability, provide fair compensation for injury, and deter medical malpractices. Over the last years, in many OECD countries, these systems have experienced difficulties resulting in high-risk specialty physicians and surgeons leaving the practice and the development of expensive and useless—if not risky—defensive medicine. This publication surveys and assesses various types of mechanisms and reforms that best limit and indemnify medical accidents. Reasons for difficulties faced by some compensation and prevention regimes, given the specificities of national circumstances and in particular of health care systems, are examined. The study offers a series of unique and focused policy options for establishing more efficient indemnification and deterrence systems to cope with medical accidents.
