    Abstract: Finding sufficient funds to pay for more resilient health systems is challenging in the current economic context. COVID-19 has shown the need for additional targeted spending on public health interventions, the digital transformation of health systems, and bolstering the health workforce. Rising incomes, technological innovation and changing demographics put further upward pressure on health spending. This could result in health spending reaching 11.8% of GDP across OECD counties by 2040. This publication explores the policy options to finance more resilient health systems whilst maintaining fiscal sustainability. It finds that the scale of the additional health financing needs requires ambitious and transformative policy changes. Robust actions to encourage healthier populations and policies to reduce ineffective spending can put future health expenditure on a far gentler upward trajectory. These would enable spending to reach a more sustainable 10.6% of GDP in 2040. Better budgetary governance is critical. It improves how public funds for health are determined, executed and evaluated. Therefore, a focus of this report is on how good budgeting practices can increase the efficiency of current public spending, and also enable more ambitious policy changes in the medium to longer-term. Findings of this report are targeted at health and finance policy makers, with improved dialogue between health and finance ministries especially important when governments are operating in a constrained fiscal setting.

Si les formes d’emploi atypiques et la transformation numérique ont ouvert le champ des possibles, elles ont aussi, comme les grands chocs économiques récents, exposé de nombreuses personnes à une fluctuation de leurs revenus, un phénomène désigné sous le nom « d’instabilité des revenus ». Dans la mesure où la situation professionnelle et personnelle des individus peut changer à plusieurs reprises au cours d’une même année, ce rapport s’appuie sur de nouvelles techniques pour déterminer les personnes les plus exposées à l’instabilité des revenus dans les pays européens de l’OCDE, et étudie les retombées de cette instabilité sur la vie quotidienne, la mobilité sociale et les inégalités. L’instabilité des revenus peut être difficile à gérer pour les personnes qui ne disposent pas des ressources financières nécessaires pour lisser leurs revenus. Dans ce rapport, les personnes qui sont à la fois exposées à l’instabilité des revenus et vulnérables dans ce contexte sont considérées en situation d’insécurité économique. L’insécurité économique touche principalement les personnes qui ont des liens ténus avec le marché du travail et celles qui ne sont pas en situation de profiter des avantages tirés de la transformation numérique. Les personnes exposées au risque d’insécurité économique sont plus susceptibles que les autres d’être inquiètes quant à la possibilité de perdre leur emploi et, comme en attestent d’autres recherches, de connaître des problèmes de santé, de souffrir d’insécurité alimentaire et d’obtenir de mauvais résultats en termes de développement dans l’enfance, autant de freins potentiels à la mobilité sociale. Enfin, le rapport passe en revue une panoplie de mesures conçues pour améliorer la réactivité des systèmes de protection sociale, de sorte à mieux soutenir les personnes aux revenus très instables ; il étudie en outre différentes pistes d’action pour aider les plus exposés au risque d’insécurité économique à se constituer des réserves financières.

    Abstract: Return migration has emerged as a critical policy concern for both destination and origin countries. While policy attention in destination countries has been focused on assisted voluntary return and reintegration (AVRR) programs, particularly for migrants with expulsion orders, these efforts represent only a fraction of broader return movements. This report, based on a project carried out by the OECD with support from the German Corporation for International Co-operation (Deutsche Gesellschaft für Internationale Zusammenarbeit GmbH – GIZ), commissioned by the Federal Ministry for Economic Co-operation and Development, analyses the scope and characteristics of different categories of return migration. The report draws on three workshops, held in Tunis, Rabat and Brussels, that discussed return migration in different contexts. It examines the multiple factors that influence migrants' decisions to return to their countries of origin and their reintegration at home, including the role of family and community. The report emphasises the pre-existing structures and networks of returning migrants in origin countries and their role in supporting different types of return migrants. The report also looks at potential re-migration.

This document presents the OECD’s renewed health system performance assessment framework. It incorporates new performance dimensions, notably people-centredness, resilience, and environmental sustainability, and places increased emphasis on addressing inequalities, including those related to gender. This framework expands on existing OECD efforts in these domains and integrates the most recent advancements in health system performance assessment. By offering common definitions and fostering a shared understanding among policy makers, stakeholders and organisations, the updated framework will enhance international collaboration. Furthermore, it lays the foundation for developing future indicators, facilitating data collection, policy analysis, and the integration of knowledge.

Ses auteurs ont fait appel à des techniques innovantes pour identifier les catégories de population en situation d’insécurité économique dans les pays européens de l’OCDE – autrement dit celles qui sont les plus exposées et les plus vulnérables à l’instabilité des revenus – et pour déterminer comment l’évolution des revenus et l’insécurité économique influencent les conditions de vie des individus, la mobilité sociale et les inégalités. Il passe également en revue une série de mesures propres à rendre la protection sociale plus réactive afin d’améliorer le soutien apporté à ceux dont les revenus sont instables et étudient des solutions qui permettraient d’aider ceux que l’insécurité économique guette plus particulièrement à constituer des réserves financières.

These results are a key input for evidence-based policies targeting inequality and fostering inclusive growth, providing insights into key dimensions of material well-being across household groups. The results complement existing inequality measures by including elements that are often lacking from micro statistics and by providing inequality measures consistent with macroeconomic aggregates, broadening the scope for analyses, while also capturing households and transactions that are typically underrepresented in micro data. Moreover, while the estimates do require a number of statistical choices and assumptions, they have a high degree of international comparability because of the common methodology and their alignment to national accounts results. The handbook provides an overview of the conceptual framework underlying the distributional results and discusses various aspects in relation to the compilation and presentation of the distributional results. It aims to assist compilers in deriving high-quality distributional results and to provide users with more insights into the main benefits of these results as well as into the way that the results have been derived.

Novice staff, defined as staff with up to three years of experience, account for 27% of staff in TALIS Starting Strong 2018. Novice staff are generally less educated than experienced staff in terms of the level of educational attainment and education or training with a focus specifically on working with children. More novice staff than experienced staff report a strong need for additional professional development in several areas, including “classroom/playgroup/group management” and “working with parents or families”. Policy approaches to support novice staff in ECEC can include ensuring that initial education prepares novice staff for their jobs by providing relevant knowledge specifically on working with children, supporting novice staff to engage in ongoing professional development, and clarifying how novice staff can progress professionally.

    Abstract: Parental emotional support, alongside material and temporal support, is an important determinant of children's subjective well-being and academic success. However, not all children benefit from the same level of parental support, and there are major differences depending on families' socio-economic status and child gender. Using the PISA 2018 surveys, this paper examines differences in parental support reported by 15-year-olds both within countries according to social status and between girls and boys, and between countries. We show that differences in parental emotional support by parents' education level and child gender are substantial. Some of these differences are (largely) explained by other characteristics such as family wealth, country of origin, and school urbanicity and private/public status. Greater parental emotional support is also found to be associated with higher PISA test scores and greater subjective wellbeing, with little variation by parental education. On the whole, our findings suggest that a significant enhancement in parental support and related child outcomes, especially in countries with lower average levels of parental emotional support, can be attained through a combined effort on several fronts: by addressing monetary and material poverty within families, by facilitating parents in balancing work and taking care of their children, by promoting greater parental involvement in their children's school life, and by offering appropriate services to assist families with special needs and facing greater challenges.

    Abstract: Ensuring affordable access to novel medicines has been identified as a policy priority among OECD and EU countries, yet systematic monitoring of the various dimensions of access is lacking. Previous efforts to measure access have focused primarily on one or at most two of these dimensions, such as availability and affordability, but a more holistic picture is needed. The OECD undertook a pilot study in EU Member States that aimed to determine the utility and feasibility of routine, cross-national monitoring of access to medicines across multiple dimensions. The work included a desk review to define the dimensions of access and associated indicators, followed by an OECD survey to explore the feasibility of collecting and analysing the relevant data for a convenience sample of 15 recently authorised product/indication pairs. This working paper presents key learnings from the desk review and country survey to which 21 EU Member States responded, with a focus on exploring the utility and feasibility of the processes of monitoring and measurement.

Those living in deprived areas, migrant population, and ethnic minorities are at higher risk of catching and dying from the virus than other groups, and they also face significant indirect health impacts of the COVID-19 pandemic - both mental health impacts and disruption of routine care. The working paper gathers evidence on the direct and indirect health impacts of the COVID-19 on the poor population and the ethnic minorities. It reviews factors underlying these inequalities, and maps policy interventions adopted by OECD countries to help address the disproportionate impacts of the COVID-19 pandemic on vulnerable population groups.

    Abstract: Innovative providers’ payment models represent an important policy lever that could be used to promote value-based health systems. By bundling services across the continuum of care or to target acute events or chronic conditions, innovative payment models set financial incentives for providers to increase efficiency in service delivery, improve health outcomes and enhance patient experience with care. This paper offers insights on value-based payment models, a type of innovative payment model implemented in several OECD countries and reviews the publicly available evidence on the impact of those payment models on value. Innovative payment models tend to be exceptional and small-scale compared to activity-based payment models and have been extensively piloted in the United States while implementation and evaluation in other countries is limited. The publicly available empirical evidence points to modest efficiency and quality gains from value-based payment models. Impact on healthcare spending, outcomes and patient experience varies across programmes. Given the significant variation in the key features of value-based payment models and the context-specific issues they address, those models do not offer a one-size-fits-all solution. This paper outlines several intervention points that policy makers need to consider when designing and implementing value-based payment models to maximise their positive outcome.

    Abstract: New OECD surveys of employers and workers in the manufacturing and finance sectors of seven countries shed new light on the impact that Artificial Intelligence has on the workplace —an under-researched area to date due to lack of data. The findings suggest that both workers and their employers are generally very positive about the impact of AI on performance and working conditions. However, there are also concerns, including about job loss—an issue that should be closely monitored. The surveys also indicate that, while many workers trust their employers when it comes to the implementation of AI in the workplace, more can be done to improve trust. In particular, the surveys show that both training and worker consultation are associated with better outcomes for workers.

    Abstract: How artificial intelligence (AI) will impact workplaces is a central question for the future of work, with potentially significant implications for jobs, productivity, and worker well-being. Yet, knowledge gaps remain in terms of how firms, workers, and worker representatives are adapting. This study addresses these gaps through a qualitative approach. It is based on nearly 100 case studies of the impacts of AI technologies on workplaces in the manufacturing and finance sectors of eight OECD countries. The study shows that, to date, job reorganisation appears more prevalent than job displacement, with automation prompting the reorientation of jobs towards tasks in which humans have a comparative advantage. Job quality improvements associated with AI – reductions in tedium, greater worker engagement, and improved physical safety – may be its strongest endorsement from a worker perspective. The study also highlights challenges – skill requirements and reports of increased work intensity – underscoring the need for policies to ensure that AI technologies benefit everyone.

The PaRIS survey has been developed together with government officials, patients, providers, and researchers. However, the extent of stakeholder involvement varies between countries. This paper reports on the stakeholder engagement in design, development and implementation of the PaRIS survey Field Trial in seventeen countries. Engagement strategies were analysed by target group (patients, providers, or other stakeholders), and engagement level (co-designing, involving, consulting, and informing). The results provide valuable lessons for the implementation of the full PaRIS survey in 2023 and illustrate how stakeholders could be more actively engaged in health services research and policymaking.

It relies on a rich set of available data and policy models and combines them in a way that is informative for mapping the gains and losses at the household level in the short term as countries transition to a low-carbon economy. After accounting for direct burdens from higher fuel prices, indirect effects from higher prices of goods other than fuel, and households’ behavioural responses, overall burdens are only mildly regressive. Recycling carbon-tax revenues back to households allows considerable scope for avoiding or cushioning losses for large parts of the population, and existing policy models can be used to design compensation measures that facilitate majority support for carbon tax packages.

Based on extensive feedback from 35 out of 38 OECD countries and a consultation with non-governmental service providers, this report identifies best practices and investigates the barriers to resolving one of the most pressing human rights issues in OECD countries today.

Elk nationaal kankerprofiel geeft een korte samenvatting van: de nationale kankerlast; risicofactoren voor kanker (met de nadruk op gedrags- en omgevingsrisicofactoren); programma’s voor vroegtijdige opsporing; prestaties van de kankerzorg (met de nadruk op toegankelijkheid, kwaliteit van de zorg, kosten en het effect van COVID-19 op de kankerzorg).

    Abstract: The use of telemedicine was quite limited in most OECD countries before the COVID-19 pandemic, held back by regulatory barriers and hesitancy from patients and providers. In early 2020, as COVID-19 massively disrupted in-person care, governments moved quickly to promote the use of telemedicine. The number of teleconsultations skyrocketed, playing a vital role in maintaining access to care, but only partly offsetting reductions in in-person care. This report provides an overview of the use of telemedicine in OECD countries, describing how governments scaled up remote care during the pandemic and exploring the impact that this massive shift to remote care has had on health care system performance. Telemedicine may be here to stay, but questions remain concerning how to regulate its use, how to pay for it, how to integrate it with in-person care, and how to make sure that it constitutes good value for money for all. This report puts forth priorities for policy makers to inform the discussion and to promote the best use of remote care services in the future.

    Abstract: The COVID-19 pandemic has highlighted that access to timely health spending data is crucial for informed policy-making. This Health Working Paper summarises and compares the methodologies applied in around half of OECD countries to estimate public and private health spending for the most recent year (i.e., t-1) as well as the approaches taken by the OECD Secretariat to fill existing data gaps for the remaining OECD countries. For the first time, the paper also explores the feasibility of nowcasting health spending for the current year (i.e., t) and examines data sources that could be potentially useful in such an exercise. While this review should help OECD countries that do not yet have experience in estimating health spending for year t-1 to improve the timeliness in their data reporting, the paper also analyses the applicability of the methods in low- and middle-income countries.

    Abstract: OECD countries continue to face persistent gender inequalities in social and economic life. Young women often reach higher levels of education than young men, but remain under-represented in fields with the most lucrative careers. Women spend more time on unpaid work, face a strong motherhood penalty, encounter barriers to entrepreneurship and fare worse in labour markets overall. They are also under-represented in politics and leadership positions in public employment. These elements permeate many policy areas and economic sectors - from international trade and development assistance to energy and the environment - in which policy often lacks a strong gender focus. Violence against women, the most abhorrent manifestation of gender inequality, remains a global crisis. It extends the perspective on gender equality to include foreign direct investment, nuclear energy and transport. This report on the United States is the second of a number of OECD country reviews of income support policies. The report examines the reach and generosity of unemployment insurance and other income support for working age households, with a special focus on disadvantaged labour market groups. What are key gaps in benefit receipt between wage- and salaried employees and non-standard workers (part-time workers, those on temporary contracts, and self-employed workers including own-account workers)? What factors, including race/ethnicity and gender, drive non-entitlement to unemployment compensation? The report examines these questions, considers the impact of recent extensions to the unemployment insurance programme in response to the COVID pandemic, and outlines policy directions for strengthening out-of-work support.

    Abstract: This paper investigates the demand for language skills using data on online job vacancies in 27 European Union member countries and the United Kingdom in 2021. Evidence indicates that although Europe remains a linguistically diverse labour market, knowing English confers unique advantages in certain occupations. Across countries included in the analyses, a knowledge of English was explicitly required in 22% of all vacancies and English was the sixth most required skill overall. A knowledge of German, Spanish, French and Mandarin Chinese was explicitly demanded in between 1% and 2% of all vacancies. One in two positions advertised on line for managers or professionals required some knowledge of English, on average across European Union member countries and across OECD countries in the sample. This compares with only one in ten positions for skilled agricultural, forestry and fishery workers and among elementary occupations.

This paper is part of the series of large scale willingness to pay (WTP) studies resulting from the Surveys to elicit Willingness to pay to Avoid Chemicals related negative Health Effects (SWACHE) project that intends to improve the basis for doing cost benefit analyses of chemicals management options and environmental policies in general. The present paper details a stated preference survey estimating WTP to avoid IQ loss, filling an important gap in the valuation literature and addressing a need for applied benefits analysis for chemicals regulation. The SWACHE IQ loss survey was fielded in 11 countries: Australia, Canada, Denmark, Korea, Netherlands, Poland, Portugal, South Africa, Sweden, the United Kingdom and the United States.

    Abstract: From the onset of Russia’s war of aggression against Ukraine, most refugee arrivals have been women and children, creating unique challenges for integration. The share of women among adult refugees is around 70% in most host countries and many have arrived with children, with the share of minors around 30%. Most OECD countries are supporting the socio-economic integration of Ukrainian refugee women through their pre-existing mainstream integration systems. Refugee integration policies have been, however, often created with different gender and educational profiles in mind, which may weaken outcomes in the longer term. There are some promising targeted measures available in OECD countries to support the socio-economic integration of Ukrainian refugee women. These include counselling, networking opportunities, training, and work placements, but also general community-building activities aimed at women.

The OECD Skills Profiling Tool assesses three types of skills: occupation-specific skills; foundational skills (literacy, numeracy and digital skills); and a set of noncognitive skills using academically validated self-reported tests. On retrouve ces caractéristiques dans de nombreux domaines d’action et secteurs économiques (du commerce international à l’aide au développement en passant par l’énergie et l’environnement) où les questions de genre sont peu prises en compte dans l’action publique. La violence à l’encontre des femmes, expression la plus abominable des inégalités entre les genres, reste un enjeu de portée mondiale. Cette publication analyse les évolutions et les mesures en faveur de l’égalité des genres, comme la prise en compte de la dimension du genre dans les politiques et les budgets, les réformes visant à accroître la participation des pères aux congés parentaux et à la garde des enfants, les initiatives en faveur de la transparence salariale pour lutter contre les inégalités de rémunération entre les genres, ou les systèmes de lutte contre les violences basées sur le genre. Elle vise à élargir la vision de l’égalité entre les genres, afin d’inclure l’investissement direct étranger, l’énergie nucléaire et les transports. Faire progresser l’égalité entre les genres n’est pas seulement un impératif moral : en cette période marquée par un vieillissement démographique rapide, des taux de fécondité en berne et des crises qui se multiplient, les actions dans ce domaine contribueront à favoriser une croissance économique et une cohésion sociale plus respectueuses de l’égalité des genres à l’avenir. Il s’agit d’une version abrégée de la publication d’origine, composée du résumé et du chapitre 1 intitulé Mettre l’égalité des genres au cœur de toutes les politiques publiques, qui est le chapitre de synthèse.

    Abstract: Many people with health problems or disabilities leave the labour market permanently even if they still can and want to work. This can lead to low income and reduced social engagement. Governments and employers can help create an environment that supports job retention and a return to work in such situations. This report looks at one critical policy lever: the role of paid sick leave and sickness benefits in protecting workers’ health, jobs and incomes. Korea is among the very few OECD countries without statutory social protection for sick workers and is currently considering closing this gap in its welfare system. This report provides an overview of key features of sickness insurance systems in OECD countries and draws policy lessons for Korea to introduce equitable and adequate social protection for sick workers with a robust return-to-work component and financially sustainable payments that encourage employer involvement.
