    Abstract: In many countries, public expenditure, including transfers, plays a major role in reducing income inequality. The report reviews the various ways that budgeting can be used to this end. A first includes taking a broad approach to results-based budgeting, taking social and distributional goals into consideration. A second relies on integrating distributional impact analysis directly into the budget process. The report discusses the concrete experience of eight OECD countries in this area, analysing how they are integrating distributional impact assessment in spending and budgeting decisions. Finally, it discusses the tools, frameworks and data that are needed to take distributional considerations into account as part of evidence-informed policy making.

This shifting policy context and emerging challenges called for a revision in how OECD countries assess health system performance, to help ensure that health systems meet people’s health needs and preferences while providing quality healthcare for all. This document presents the OECD’s renewed health system performance assessment framework. It incorporates new performance dimensions, notably people-centredness, resilience, and environmental sustainability, and places increased emphasis on addressing inequalities, including those related to gender. This framework expands on existing OECD efforts in these domains and integrates the most recent advancements in health system performance assessment. By offering common definitions and fostering a shared understanding among policy makers, stakeholders and organisations, the updated framework will enhance international collaboration. Furthermore, it lays the foundation for developing future indicators, facilitating data collection, policy analysis, and the integration of knowledge.

    Abstract: Early childhood education and care (ECEC) staff are central to promoting young children’s development, learning and well-being. Novice staff, defined as staff with up to three years of experience, account for 27% of staff in TALIS Starting Strong 2018. Novice staff are generally less educated than experienced staff in terms of the level of educational attainment and education or training with a focus specifically on working with children. More novice staff than experienced staff report a strong need for additional professional development in several areas, including “classroom/playgroup/group management” and “working with parents or families”. Policy approaches to support novice staff in ECEC can include ensuring that initial education prepares novice staff for their jobs by providing relevant knowledge specifically on working with children, supporting novice staff to engage in ongoing professional development, and clarifying how novice staff can progress professionally.

    Abstract: This publication presents guidance for the compilation of distributional results on household income, consumption and saving consistent with national accounts totals. These results are a key input for evidence-based policies targeting inequality and fostering inclusive growth, providing insights into key dimensions of material well-being across household groups. The results complement existing inequality measures by including elements that are often lacking from micro statistics and by providing inequality measures consistent with macroeconomic aggregates, broadening the scope for analyses, while also capturing households and transactions that are typically underrepresented in micro data. Moreover, while the estimates do require a number of statistical choices and assumptions, they have a high degree of international comparability because of the common methodology and their alignment to national accounts results. The handbook provides an overview of the conceptual framework underlying the distributional results and discusses various aspects in relation to the compilation and presentation of the distributional results. It aims to assist compilers in deriving high-quality distributional results and to provide users with more insights into the main benefits of these results as well as into the way that the results have been derived.

    Abstract: Finding sufficient funds to pay for more resilient health systems is challenging in the current economic context. COVID-19 has shown the need for additional targeted spending on public health interventions, the digital transformation of health systems, and bolstering the health workforce. Rising incomes, technological innovation and changing demographics put further upward pressure on health spending. This could result in health spending reaching 11.8% of GDP across OECD counties by 2040. This publication explores the policy options to finance more resilient health systems whilst maintaining fiscal sustainability. It finds that the scale of the additional health financing needs requires ambitious and transformative policy changes. Robust actions to encourage healthier populations and policies to reduce ineffective spending can put future health expenditure on a far gentler upward trajectory. These would enable spending to reach a more sustainable 10.6% of GDP in 2040. Better budgetary governance is critical. It improves how public funds for health are determined, executed and evaluated. Therefore, a focus of this report is on how good budgeting practices can increase the efficiency of current public spending, and also enable more ambitious policy changes in the medium to longer-term. Findings of this report are targeted at health and finance policy makers, with improved dialogue between health and finance ministries especially important when governments are operating in a constrained fiscal setting.

    Abstract: Return migration has emerged as a critical policy concern for both destination and origin countries. While policy attention in destination countries has been focused on assisted voluntary return and reintegration (AVRR) programs, particularly for migrants with expulsion orders, these efforts represent only a fraction of broader return movements. This report, based on a project carried out by the OECD with support from the German Corporation for International Co-operation (Deutsche Gesellschaft für Internationale Zusammenarbeit GmbH – GIZ), commissioned by the Federal Ministry for Economic Co-operation and Development, analyses the scope and characteristics of different categories of return migration. The report draws on three workshops, held in Tunis, Rabat and Brussels, that discussed return migration in different contexts. It examines the multiple factors that influence migrants' decisions to return to their countries of origin and their reintegration at home, including the role of family and community. The report emphasises the pre-existing structures and networks of returning migrants in origin countries and their role in supporting different types of return migrants. The report also looks at potential re-migration.

    Abstract: Parental emotional support, alongside material and temporal support, is an important determinant of children's subjective well-being and academic success. However, not all children benefit from the same level of parental support, and there are major differences depending on families' socio-economic status and child gender. Using the PISA 2018 surveys, this paper examines differences in parental support reported by 15-year-olds both within countries according to social status and between girls and boys, and between countries. We show that differences in parental emotional support by parents' education level and child gender are substantial. Some of these differences are (largely) explained by other characteristics such as family wealth, country of origin, and school urbanicity and private/public status. Greater parental emotional support is also found to be associated with higher PISA test scores and greater subjective wellbeing, with little variation by parental education. On the whole, our findings suggest that a significant enhancement in parental support and related child outcomes, especially in countries with lower average levels of parental emotional support, can be attained through a combined effort on several fronts: by addressing monetary and material poverty within families, by facilitating parents in balancing work and taking care of their children, by promoting greater parental involvement in their children's school life, and by offering appropriate services to assist families with special needs and facing greater challenges.

    Abstract: In Europe, the introduction of the Council Recommendation establishing a European Child Guarantee has helped put concerns relating to childhood disadvantages higher up on the policy agenda. Structured around ensuring effective access to a series of key child services, the ultimate goal of the European Child Guarantee is to promote opportunities for children from disadvantaged backgrounds and help stop the transmission of poverty and social exclusion across generations. But as European countries move forward with their plans for implementation, it is important to understand what child disadvantage looks like, as well as the potential lifelong effects it can have on children’s lives. This paper explores childhood socio-economic disadvantage in Austria from a comparative perspective. It sketches the breadth and depth of child poverty and material deprivation in Austria today, and discusses the potential longer-run economic costs of childhood disadvantage once disadvantaged children reach adulthood, based on findings from Clarke et al. (2022). It also discusses key policy challenges Austria will need to consider to tackle childhood socio-economic disadvantage.

    Abstract: Women’s employment rates and wages are still lagging those of men across OECD countries, with average employment and wage gaps now around 15% and 12% respectively. Gaps narrowed at a relatively modest pace over the past decade, calling for further policy action. A lack of affordable high-quality childcare is often an obstacle to women’s participation in the labour market and notably to working full time. A very unequal sharing of parental leave between parents and challenges upon return to work further hampers women’s careers. Biases in the tax system may discourage women from working in some countries. Women face disadvantage in accessing management positions and entrepreneurship. A range of policies can help reduce gender gaps, including better childcare provision, incentivising parents to better share parental leave, re-skilling and upskilling on return from parental leave, encouraging gender equality within firms, integration programmes for foreign-born women, promoting women entrepreneurship and financial inclusion, and levelling taxation for second earners. Moreover, the multiple dimensions and root causes of gender inequality call for mainstreaming gender across policy domains.

    Abstract: In response to the increase of cyberattacks in health care settings, the Health Committee of the OECD asked for a paper on Digital Security as part of the OECD ongoing work on health data governance. This working paper emphasizes that as the healthcare industry undergoes digital transformation it brings significant benefits while simultaneously escalating the vulnerability to cyber threats. This working paper summarises survey results based on the OECD framework for digital security risk management. The paper reveals varying levels of digital security alignment among countries, with Ireland and Korea exhibiting full alignment. Countries with specific strategies for digital security in health showed higher alignment to leading practices. The paper identifies key areas for improvement, including fostering a digital security culture through training, strengthening strategy and governance, and embedding risk assessment and treatment. The paper also emphasises the need for collaboration on innovative tools to detect and manage digital security threats, such as multi-factor authentication and encryption. These collaborative efforts are essential to safeguard the digital foundations of modern healthcare systems and ensure the security of health data and services.

    Abstract: One in seven working age adults identifies as having a disability in OECD countries. Many of them are excluded from meaningful work and have low levels of income and social engagement. Governments can help create an environment that supports social and labour market integration for people with disability. This report reviews the effectiveness of the social protection system for people with disability in Italy and summarises the results of a pilot carried out in four regions testing an alternative disability assessment. The current assessment process can sometimes deny services to people in need of support while being generous to people with significant health problems but who are not experiencing severe disability. A disability assessment that takes medical and functional aspects into account helps to direct services and resources to those most in need and to harmonise access to disability support across the country. Implementing disability reform has proved to be difficult in Italy in the past decade. This report provides evidence that reform can improve outcomes for people with disability.

This report builds on the OECD work on “government evaluations of COVID-19 responses”. It evaluates Belgium’s responses to the pandemic in terms of risk preparedness, crisis management, as well as public health, education, economic and fiscal, and social and labour market policies. Preserving the country’s resilience in the future will require promoting trust in public institutions and whole-of-government approaches to crisis management, reducing inequalities, and preserving the fiscal balance. The findings and recommendations of this report will provide guidance to public authorities in these efforts.

    Abstract: Citizenship and residency by investment (CBI/RBI) programmes are government-administered programmes that grant citizenship or residency to foreign investors by expediting or bypassing normal migration processes. These programmes can help spur economic growth through foreign direct investment, but they are also attractive to criminals and corrupt officials seeking to evade justice and launder the proceeds of crime reaching into the billions of dollars. This report highlights how CBI programmes can allow criminals more global mobility and help them hide their identity and criminal activities behind shell companies in other jurisdictions. It highlights the vulnerabilities of these complex and international investment migration programmes, including the frequent use of intermediaries, involvement of multiple government agencies, abuse by professional enablers and lack of proper governance of the CBI/RBI programmes. The report proposes measures and examples of good practice, that can help policy makers and those responsible for managing the investment migration programmes address these risks. These include an in-depth analysis and understanding of how criminals can exploit CBI or RBI programmes and incorporating risk mitigation measures, such as multi-layer due diligence, in the design of the investment migration programme.

    Abstract: Over the past few decades, economies and technologies have changed in ways that have made people’s economic prospects more insecure. While non-standard work and digital transformation have created opportunities for many, they have also exposed individuals to fluctuations in their incomes, known as "income instability", as have major recent shocks. Recognising that individuals’ jobs and circumstances can change multiple times in a year, this report uses novel techniques to identify who is most exposed to income instability in European OECD countries and examines the effects it has on their lives, social mobility, and inequality. Income instability can be difficult to manage for individuals who lack the financial resources to smooth their incomes. In this report, people facing the twin problems of exposure and vulnerability to income instability are considered to be economically insecure. Economic insecurity falls predominantly on people with weak attachments to the labour force and on those who are not well-placed to leverage the benefits of digitalisation. People at risk of economic insecurity are more likely to worry about losing their jobs in the future than economically secure individuals and, as shown in other research, experience poor health, food insecurity, and poor childhood development outcomes, which can impede social mobility. Finally, the report reviews a range of policies to improve the timeliness of social protection to better support people with highly unstable incomes and explores options to help those most at risk of economic insecurity build financial buffers.

    Abstract: Education systems around the world are increasingly recognising that social and emotional skills (SES) are essential for students and societies. This paper contributes to the Innovative approaches to measuring social and emotional skills project, which aims to complement self-report measures of SES with more direct assessments. It addresses criticisms of the OECD framework for SES used by the OECD Survey on Social and Emotional Skills and discusses the use of the Big Five model as an underlying structure for organising SES. The paper then examines the teachability of SES through a comprehensive literature review. In addition, it reviews the evidence for the relationship between different SES and key life outcomes. The paper also seeks to reconcile divergent conceptual understandings in the literature by providing a clear definition of SES and highlighting discrepancies in skill definitions. The conclusions of the paper identify gaps in current research, highlight teachable skills with notable predictive value SES relevant for educators and policy makers, and support the development of direct SES assessments.

To develop ‘trustworthy’ and ‘responsible’ AI systems, there is a need to identify and manage AI risks. As calls for the development of accountability mechanisms and risk management frameworks continue to grow, interoperability would enhance efficiency and reduce enforcement and compliance costs. This report provides an analysis of the commonalities of AI risk management frameworks. It demonstrates that, while some elements may sometimes differ, all the risk management frameworks analysed follow a similar and sometimes functionally equivalent risk management process.

    Abstract: Two out of five OECD countries contract out some of the job brokerage and counselling functions of publicly financed employment services using outcome-based payment models. This paper examines several important aspects related to the design and implementation of such outsourcing. First, innovative payment models can improve incentives for external providers to offer training and more effective services for hard-to-place clients. Second, providing forward guidance to providers and accounting for contingencies can mitigate their risks, e.g. of being underpaid relative to expenses incurred, thus lowering service costs. Third, letting individuals choose a provider can result in services that are better tailored and foster ongoing competition between providers. Finally, automating data exchange can, somewhat paradoxically, improve data privacy and data protection while enabling new payment models. These and related findings are discussed with country examples based on desk research and interviews with stakeholders in several OECD countries. The paper builds on work conducted in the project “Reforming the Swedish Public Employment Service”, which was carried out with funding from the European Union via the Technical Support Instrument and was implemented by the OECD in cooperation with the European Commission's Directorate-General for Structural Reform Support.

    Abstract: The EU’s ambitious Green Deal aims at achieving net zero emissions by 2050. The EU is starting from a relatively good position. It has successfully reduced greenhouse gas emissions over the past decade. But further efforts are needed to reach the net zero target. These include an extension of emission trading to agriculture and the phase-out of generous subsidies for fossil fuels. Such efforts should be complemented by additional measures to shift to clean energy, notably more integrated electricity markets and deeper capital markets that provide the necessary investment in new technologies. Accelerating the green transition will also involve costs for displaced workers. Bolstering workers’ mobility and training will help improve labour reallocation and reduce transition costs.

Beliefs in social mobility may help strengthen the social contract; however, strongly held views on the role of individual effort in determining success in life can also contribute to diminishing support for policies designed to promote greater opportunity for vulnerable populations and reduce inequality. Drawing on data from the Opportunities module of the 2022 wave of the OECD Risks that Matter survey, this policy insights investigates people’s views of the main determinants of social mobility in 27 OECD countries. On average, 60% of respondents believe that hard work is essential or very important in determining one’s social position. However, of this majority, only one-fifth believe that working hard is the only key factor to success.

Smart, targeted investments in health system resilience are needed to improve health and ensure the next shock is less disruptive and costly. This report reviews the lessons of the COVID-19 pandemic and applies them to build policy recommendations to ensure the global community is ready for the next crisis. The reviews and recommendations cover health system issues – including workforce, digitalisation, continuity of care and mental health – and other topics, including long-term care, supply chains and international co-operation.

    Abstract: How artificial intelligence (AI) will impact workplaces is a central question for the future of work, with potentially significant implications for jobs, productivity, and worker well-being. Yet, knowledge gaps remain in terms of how firms, workers, and worker representatives are adapting. This study addresses these gaps through a qualitative approach. It is based on nearly 100 case studies of the impacts of AI technologies on workplaces in the manufacturing and finance sectors of eight OECD countries. The study shows that, to date, job reorganisation appears more prevalent than job displacement, with automation prompting the reorientation of jobs towards tasks in which humans have a comparative advantage. Job quality improvements associated with AI – reductions in tedium, greater worker engagement, and improved physical safety – may be its strongest endorsement from a worker perspective. The study also highlights challenges – skill requirements and reports of increased work intensity – underscoring the need for policies to ensure that AI technologies benefit everyone.

    Abstract: The OECD's Patient-Reported Indicator Surveys (PaRIS) initiative aims to measure outcomes and experiences of healthcare as part of an effort to improve the value of health system investments. The PaRIS survey, a survey of people living with chronic conditions, is currently being implemented in twenty countries. The PaRIS survey has been developed together with government officials, patients, providers, and researchers. However, the extent of stakeholder involvement varies between countries. This paper reports on the stakeholder engagement in design, development and implementation of the PaRIS survey Field Trial in seventeen countries. Engagement strategies were analysed by target group (patients, providers, or other stakeholders), and engagement level (co-designing, involving, consulting, and informing). The results provide valuable lessons for the implementation of the full PaRIS survey in 2023 and illustrate how stakeholders could be more actively engaged in health services research and policymaking.

It explores legal and policy progress towards LGBTI+ equality in Germany at both the national and subnational levels, and identifies good practices. The report first investigates the life situation of LGBTI+ Germans by presenting the most up-to-date data on the share of Germans who self-identify as LGBTI+, evaluating the extent to which LGBTI+ Germans face discrimination and violence, and assessing how this population fares in terms of well-being, mental and physical health. The report then examines whether laws critical to achieving LGBTI+ equality have been passed and how LGBTI+ equality in Germany could be further improved through legislation. Finally, beyond laws, the report focuses on policy achievements towards LGBTI+ equality, by distinguishing between remedial policies, aimed at enforcing antidiscrimination and anti-violence laws, and preventive policies, aimed at fostering a culture of equal treatment of LGBTI+ individuals at school, in the workplace, and in healthcare.

    Abstract: Despite the fundamental importance of childhood, countries tend to invest much less in this area than in old age. Effective child well-being policies can bring high long-term returns on early life investments and public interventions. However, these benefits are not fully understood and thus do not always receive the necessary political attention in public policy making. Mobilising evidence to support effective policies for child well-being is crucial, especially as responsibilities for such policies tend to be fragmented across government departments and levels of government. Therefore, organisations such as knowledge brokers, which help ensure that evidence is shared with those responsible for designing and implementing public policies, have a critical role to play in improving the effectiveness of child well-being policies and practices. They can help make evidence accessible, trustworthy, and understandable, so that it has the greatest impact on policy. This working paper sheds light on the best practices identified for mobilising evidence to enhance the effectiveness of child well-being policies, based on a stocktaking of 81 knowledge broker organisations across 24 countries, complemented by a qualitative survey carried out among senior experts operating at national and international level.

    Abstract: Flanders experienced large inflows of immigrants over the past decade, coming from an increasingly diverse range of countries, with growth rates outpacing the Netherlands, France and Germany, as well as Belgium as a whole. While integration outcomes have improved in recent years, some of the core indicators remain unfavourable in international comparison, especially for non-EU immigrant women, refugees, and youth with migrant parents. Against this backdrop, Flanders has developed a comprehensive integration policy. This review, the fourth in the series Working Together for Integration, provides an in-depth analysis of the Flemish integration system, highlighting its strengths, weaknesses, and potential areas for improvement. Earlier reviews in this series looked at integration in Sweden (2016), Finland (2018) and Norway (2022).

    Abstract: Many Lithuanian households struggle to afford good-quality housing. The housing stock is dominated by owner-occupied, multi-apartment buildings that are energy inefficient and face persistent quality gaps. While average household spending on housing is relatively low, house prices have been rising, and many households cannot afford to move to higher quality homes that better suit their needs. Lithuanian policy makers have stepped up support for housing in recent years, but more actions are needed. This report presents the main features of the Lithuanian housing market, highlights housing challenges that have been amplified by the current economic and geopolitical crises, and assesses current policies to address housing affordability and quality gaps. It proposes a series of recommendations to strengthen the supply of and access to affordable housing, and to support Lithuania’s commitment to make housing policy a priority.

    Abstract: Russia’s war against Ukraine is causing a humanitarian, social and economic crisis for the Ukrainian people. The consequences of this full-scale military invasion are disrupting the global supply of commodities, sharply increasing food and energy prices, and threating the recovery from the COVID-19 pandemic. Countries with established commercial and financial ties with the economies of Russia and Ukraine appear to be particularly vulnerable. Assessing the Impact of Russia’s War against Ukraine on Eastern Partner Countries investigates the exposure of Eastern Partner countries (Armenia, Azerbaijan, Georgia, Republic of Moldova and Ukraine) to the economic shocks caused by the war, and in particular through the impact that the conflict is having on inflation, migration, remittances, investment and trade. This report is published as part of the multi-country project “EU4Business: From Policies to Action – phase 2”, implemented in the Eastern Partnership with the financial support of the European Union within the EU4Business initiative.

    Abstract: This working paper proposes a methodology to monetise five aspects of employee well-being (wage inequality, being employed, excess working hours, relationships with management and job security) using theoretical and empirical frameworks drawn from welfare economics. Preliminary results highlight a large loss of welfare arising from within-firm wage inequality as well as a strong impact of working conditions on workers’ well-being. On the aggregate, suppressing the negative externalities of the firm linked to excess working hours, tensions with management and job insecurity would yield an increase in social welfare equivalent to a 25% increase in household income, representing many years of economic growth. Greater transparency on company wage distributions and working conditions is necessary to apply these methodologies to real firms.

    Abstract: There is ample evidence that exposure to various chemicals can increase the probability of children to be born with low or very low birth weight. Infants born with very low birth weight have a higher risk of suffering from neurosensory problems, issues related to behavioural and social competencies, and learning disabilities than infants born with normal birth weight. Authorities face challenges in regulating chemical substances through actions such as bans and prohibitions, because of the difficulty in explicitly considering the economic benefits and costs of such regulations. Moreover, existing Values of a Statistical Case (VSC) of very low birth weight are rare and cannot be directly applied to the cost benefit analysis of chemical management options for a wide range of countries. This paper is part of the series of large scale willingness to pay (WTP) studies resulting from the Surveys to elicit Willingness to pay to Avoid Chemicals related negative Health Effects (SWACHE) project that intends to improve the basis for doing cost benefit analyses of chemicals management options and environmental policies in general. The present paper details a stated preference survey estimating WTP to reduce the risk of very low birth weight, filling an important gap in the valuation literature and addressing a need for applied benefits analysis for chemicals regulation. The SWACHE very low birth weight survey was fielded in 9 countries: Canada, the Czech Republic, Italy, Mexico, the Netherlands, Switzerland, Türkiye, the United Kingdom, and the United States.

This report is the first of a number of OECD country reviews of income support policies. Each report analyses key policy challenges, discusses recent reform initiatives, and identifies good practices from other OECD countries. This report on Korea focuses on avenues for strengthening benefit coverage, income security and re-employment in the context of a “dual” labour market with large parts of the workforce in short-duration, non-standard or informal employment. It welcomes Korea’s recent reforms towards more accessible jobseeker support, and points to priorities for additional policy action to make income and employment support more effective and inclusive.

    Abstract: Many people with health problems or disabilities leave the labour market permanently even if they still can and want to work. This can lead to low income and reduced social engagement. Governments and employers can help create an environment that supports job retention and a return to work in such situations. This report looks at one critical policy lever: the role of paid sick leave and sickness benefits in protecting workers’ health, jobs and incomes. Korea is among the very few OECD countries without statutory social protection for sick workers and is currently considering closing this gap in its welfare system. This report provides an overview of key features of sickness insurance systems in OECD countries and draws policy lessons for Korea to introduce equitable and adequate social protection for sick workers with a robust return-to-work component and financially sustainable payments that encourage employer involvement.

Are societies adapting quickly enough to cope with ageing at an unprecedented scale? What are some of the key structural barriers, the embedded attitudes and behaviours, that stand in the way of helping midcareer and older workers stay employed longer? What steps can we take to support them as we transition to a more digital and sustainable economy, and make sure that age is no longer a barrier to work? In search of answers, the OECD and Generation, a global employment nonprofit network, teamed up to develop new research based on a survey of thousands of employed and unemployed people, and hiring managers in eight countries: the Czech Republic, France, Germany, Romania, Spain, Sweden, the United Kingdom, and the United States. The evidence presented in this report aims to foster a more inclusive and resilient labour market for workers at all stages of their careers.
