    Abstract: Variations in health care use within a country are complicated. In some cases they may reflect differences in health needs, in patient preferences or in the diffusion of a therapeutic innovation; in others they may not. There is evidence that some of the observed variations are unwarranted, signalling under- or over-provision of health services, or both. This study documents geographic variations for high-cost and high-volume procedures in select OECD countries. It finds that there are wide variations not only across countries, but within them as well. A mix of patient preferences and physician practice styles likely play an important part in this, but what part of the observed variations reflects over-provision, or whether there are unmet needs, remain largely unexplained. This report helps policy makers better understand the issues and challenges around geographic variations in health care provision and considers the policy options.

    Abstract: " In Settler Common Sense, Mark Rifkin explores how canonical American writers take part in the legacy of displacing Native Americans. Although the books he focuses on are not about Indians, they serve as examples of what Rifkin calls "settler common sense," taking for granted the legal and political structure through which Native peoples continue to be dispossessed.In analyzing Nathaniel Hawthorne's House of the Seven Gables, Rifkin shows how the novel draws on Lockean theory in support of small-scale landholding and alternative practices of homemaking. The book invokes white settlers in southern Maine as the basis for its ethics of improvement, eliding the persistent presence of Wabanaki peoples in their homeland. Rifkin suggests that Henry David Thoreau's Walden critiques property ownership as a form of perpetual debt. Thoreau's vision of autoerotic withdrawal into the wilderness, though, depends on recasting spaces from which Native peoples have been dispossessed as places of non-Native regeneration. As against the turn to "nature," Herman Melville's Pierre presents the city as a perversely pleasurable place to escape from inequities of land ownership in the country. Rifkin demonstrates how this account of urban possibility overlooks the fact that the explosive growth of Manhattan in the nineteenth century was possible only because of the extensive and progressive displacement of Iroquois peoples upstate.Rifkin reveals how these texts' queer imaginings rely on treating settler notions of place and personhood as self-evident, erasing the advancing expropriation and occupation of Native lands. Further, he investigates the ways that contemporary queer ethics and politics take such ongoing colonial dynamics as an unexamined framework in developing ideas of freedom and justice. "..

Countering the inclination to associate indigenous peoples with "wilderness" or to conflate everything "Indian" with a vague sense of the ecological, Joy Porter shows how Indian communities were forced to migrate to make way for the nation's "wilderness" parks in the nineteenth century. Among the first American communities to reckon with environmental despoliation, they have fought significant environmental battles and made key adaptations. By linking Native American history to mainstream histories and current debates, Porter advances the important process of shifting debate about climate change away from scientists and literary environmental writers, a project central to tackling environmental crises in the twenty-first century. Countering the inclination to associate indigenous peoples with "wilderness" or to conflate everything "Indian" with a vague sense of the ecological, Joy Porter shows how Indian communities were forced to migrate to make way for the nation's "wilderness" parks in the nineteenth century. Among the first American communities to reckon with environmental despoliation, they have fought significant environmental battles and made key adaptations. By linking Native American history to mainstream histories and current debates, Porter advances the important process of shifting debate about climate change away from scientists and literary environmental writers, a project central to tackling environmental crises in the twenty-first century.     Abstract: Psychiatric disorders in adolescents are an important social problem relevant to almost all healthcare professionals. The prevalence of mood, anxiety, substance use and other psychiatric disorders among adolescents is high. However, medical professionals are not sufficiently trained about adolescent psychiatric disorders. Primary care providers correctly identify less than a fourth of youth with a depressive or anxiety disorder and many clinicians underestimate the importance of the problem of adolescent psychiatric illnesses and suicidal behavior. Multiple clinical, neurobiological, cultural and legal aspects of psychiatric disorders in adolescents are discussed in this book.

    Abstract: In Zombie Seed and the Butterfly Blues: A Case of Social Justice students from a liberal arts class help Professor Delta Quinn and reporter, Caleb Barthes, uncover the political and corporate story behind the scientific development and implementation of the zombie seed . As the secrets of the seed are revealed, so are the secrets of Delta’s tragic past which explain her desire to study the sequestered stories of domestic violence, which may lead the reader to ask whether there is a connection between cultural violence and interpersonal violence, and more importantly, whether such knowledge will awaken the zombie in all of us. Socrates’ oft quoted maxim—the unexamined life is not worth living —speaks to the current image of the zombie who walks through life without critically thinking, without addressing political issues, without participating in civil discourse or democratic entitlements. Zombie Seed and the Butterfly Blues: A Case of Social Justice is meant to engage the college student, to have students address and discuss issues of relevance to society at large.     Abstract: An occupation is, most simply put, any activity we participate in that engages (occupies) our attention, interests, and/or expectations, at any point throughout the life course. This book offers an emerging and innovative perspective on occupation, based in the work of American philosopher John Dewey and other pragmatists, that challenges accepted ideas. Each chapter presents a lively and multifaceted dialogue on transactional perspectives on occupation.     Abstract: This text is intended to help social work practitioners move beyond both these often-accepted constructions of sexuality and the range of methods that are available to social workers in their clinical practice. Various themes are apparent throughout each of the chapters in this volume: the range of sexual experience and expression that exists across individuals; a recognition of our societys responses to expressions of sexuality, including the social, attitudinal, and cultural barriers that inhibit the expression of healthy sexuality and that constrain our approaches to assisting individuals with their recovery from trauma; the need to consistently and painstakingly examine our own assumptions relating to sexuality in order to be more effective with our clients; and the delicate balance that is often required when working with clients around issues of sexuality in the context of institutions, community, and societal structures.

The first volume in this series is a legacy to C. Henry Kempe. This is a timely publication because 2012 marks 50 years after the appearance of the foundational article by C. Henry Kempe and his colleagues, 'The Battered-Child Syndrome.' This volume capitalizes on this 50 year anniversary to stand back and assess the field from the perspective that Dr. Kempes early contributions and ideas are still being played out in practice and policy today. The volume will be released at the next ISPCAN meeting, also in 2012.

    Abstract: The metaphor of the monster or predator-usually a sexual predator, drug dealer in areas frequented by children, or psychopathic murderer-is a powerful framing device in public discourse about how the criminal justice system should respond to serious violent crimes. The cultural history of the monster reveals significant features of the metaphor that raise questions about the extent to which justice can be achieved in both the punishment of what are regarded as "monstrous crimes" and the treatment of those who commit such crimes.This volume is the first to address the connections between the history of the monster metaphor, the 19th century idea of the criminal as monster, and the 20th century conception of the psychopath: the new monster. The book addresses, in particular, the ways in which the metaphor is used to scapegoat certain categories of crimes and criminals for anxieties about our own potential for deviant, and, indeed, dangerous interests. These interests have long been found to be associated with the fascination people have for monsters in most cultures, including the West.The book concludes with an analysis of the role of forensic psychiatrists and psychologists in representing criminal defendants as psychopaths, or persons with certain personality disorders. As psychiatry and psychology have transformed bad behavior into mad behavior, these institutions have taken on the legal role of helping to sort out the most dangerous among us for preventive "treatment" rather than carceral "punishment."

    Abstract: "Medical or health-oriented screening programs are amongst the most debated aspects of health care and public health practices in health care and public health ethics, as well as health policy discussions. In spite of this, most treatments of screening in the research literature restrict themselves to isolated scientific aspects, sometimes complemented by economic analyses or loose speculations regarding policy aspects. At the same time, recent advances in medical genetics and technology, as well as a rapidly growing societal focus on public health concerns, inspires an increase in suggested or recently started screening programs. This book involves an in-depth analysis of the ethical, political and philosophical issues related to health-oriented screening programs. It explores the considerations that arise when heath care interacts with other societal institutions on a large scale, as is the case with screening: What values may be promoted or compromised by screening programs? What conflicts of values do typically arise - both internally and in relation to the goals of health care, on the one hand, and the goals of public health and the general society, on the other? What aspects of screening are relevant for determining whether it should be undertaken or not and how it should be organised in order to remain defensible? What implications does the ethics of screening have for health care ethics as a whole? These questions are addressed by applying philosophical methods of conceptual analysis, as well as models and theories from moral and political philosophy, medical ethics, and public health ethics, to a large number of ongoing and proposed screening programs which makes this book the first comprehensive work on the ethics of screening.     Abstract: Childhood schizophrenia is a rare but clinically difficult form of schizophrenia. Early childhood schizophrenia should be diagnosed as early as possible, also because of the poor prognosis. The new book by schizophrenia expert Christian Eggers comprehensively conveys the symptoms, the course characteristics and the prognosis of schizophrenia in childhood and adolescence. Other focal points of the book are the presentation of thinking and language disorders in childhood schizophrenia, detailed differential diagnostic discussions (with case studies) and the premorbid development, as well as early warning and precursor symptoms, mortality and the importance of post-psychotic personality and personality changes. The stigmatization caused by the environment or the delinquency and forensic significance of schizophrenia in childhood and adolescence is also discussed. Etiological findings and hypotheses and the resulting therapeutic, socio-educational, rehabilitative and preventive conclusions are presented in detail. One focus is on the close interlinking between modern (molecular) genetic findings and environmentally dependent and life history constellations. The presentation is based on the author's outstanding scientific work with his worldwide unique sample of the course of the disease of children with schizophrenia, which were personally examined by the author after an average of 15 and 42 years. With the book, the author also documents his extensive practical experience from many decades of clinical activity, which focuses on the schizophrenia of childhood and adolescence

    Abstract: Yuri Leving's Keys to "The Gift": A Guide to Vladimir Nabokov's Novel is a new systematization of the main available data on Nabokov's most complex Russian novel, The Gift (1934-1939). From notes in Nabokov's private correspondence to scholarly articles accumulated during the seventy years since the novel's first appearance in print, the work draws from a broad spectrum of existing material in a succinct and coherent way, as well as providing innovative analyses. The first part of the monograph, "The Novel," outlines the basic properties of The Gift (plot, characters, style, and motifs) and reconstructs its internal chronology. The second part, "The Text," describes the creation of the novel and the history of its publication, public and critical reaction, challenges of the English translation, and post-Soviet reception.     Abstract: "In 1761 and again in 1768, European scientists raced around the world to observe the transit of Venus, a rare astronomical event in which the planet Venus passes in front of the sun. In The Transit of Empire, Jodi A. Byrd explores how indigeneity functions as transit, a trajectory of movement that serves as precedent within U.S. imperial history. Byrd argues that contemporary U.S. empire expands itself through a transferable "Indianness" that facilitates acquisitions of lands, territories, and resources. Examining an array of literary texts, historical moments, and pending legislations--from the Cherokee Nation of Oklahoma's vote in 2007 to expel Cherokee Freedmen to the Native Hawaiian Government Reorganization bill--Byrd demonstrates that inclusion into the multicultural cosmopole does not end colonialism as it is purported to do.     Abstract: Die heterologe bzw. donogene Insemination meint die künstliche Befruchtung einer Eizelle zu Fortpflanzungszwecken mit dem Samen eines Fremdspenders, um den betroffenen Paaren eine ungewollte Kinderlosigkeit zu ersparen. Diese von der modernen Reproduktionsmedizin schon seit längerem angebotene und auch tatsächlich in Anspruch genommene Methode wirft jedoch eine Reihe von rechtlichen, humangenetischen und psychologischen Fragen auf, die auch im Kreise der Experten bisher als nicht hinreichend geklärt betrachtet werden. Der vorliegende Band unternimmt es, diese Fragen aufzugreifen und im interdisziplinären Gespräch denkbare Antworten und Lösungen auszuloten. Die hier versammelten Beiträge gehen auf einen Workshop zurück, den in Kooperation das Zentrum für Medizinrecht und das Göttinger Institut für Humangenetik gemeinsam veranstaltet haben. Mit den hieraus hervorgegangenen Empfehlungen hoffen die Herausgeber, wichtige Impulse und Hilfestellungen für die in diesem Bereich Verantwortlichen, aber auch für Gesetzgebung und Rechtsprechung geben zu können.
    Abstract: The heterologous or donogenous insemination is the artificial fertilization of an ovum for reproductive purposes with the sperm of an outside donor to save the affected couples a infertility. This method is long been offered and used by the modern reproductive medicine but, however raises a number of legal, human genetic and psychological issues that are not at sufficiently resolved in the circle of experts. The present volume addresses these questions and possible answers in an interdisciplinary discussion and explores solutions. The articles go back to a workshop, which was jointly organized in cooperation of the Centre for Medical Law and the Göttingen Institute of Human Genetics. The editors hope that the recommendations resulting from this will be able to provide important impetus and support to the leaders in this field, but also to legislation and case law
