Cette évolution du contexte politique et ces défis nouveaux appellent à revoir la manière dont les pays de l'OCDE évaluent la performance des systèmes de santé afin de leur permettre de répondre aux besoins et aux préférences des citoyens en matière de santé, tout en fournissant des soins de qualité pour tous. Ce document présente le nouveau cadre d'évaluation de la performance des systèmes de santé de l'OCDE. Il intègre de nouvelles dimensions de la performance, notamment la question des soins centrés sur la personne, la résilience et le développement durable, et met davantage l'accent sur la lutte contre les inégalités, y compris celles liées au genre. Ce cadre s'appuie sur les efforts déjà déployés par l'OCDE dans ces domaines et intègre les avancées les plus récentes en matière d'évaluation de la performance des systèmes de santé. En proposant des définitions communes et en favorisant une compréhension partagée entre décideurs, parties prenantes et organisations, le cadre renouvelé facilitera la collaboration internationale. En outre, il permettra l'élaboration de futurs indicateurs, facilitera la collecte de données, l'analyse des politiques et l'intégration des savoirs.

    Kurzfassung: Les mutations économiques et technologiques des dernières décennies ont rendu les perspectives économiques individuelles plus incertaines. Si les formes d’emploi atypiques et la transformation numérique ont ouvert le champ des possibles, elles ont aussi, comme les grands chocs économiques récents, exposé de nombreuses personnes à une fluctuation de leurs revenus, un phénomène désigné sous le nom « d’instabilité des revenus ». Dans la mesure où la situation professionnelle et personnelle des individus peut changer à plusieurs reprises au cours d’une même année, ce rapport s’appuie sur de nouvelles techniques pour déterminer les personnes les plus exposées à l’instabilité des revenus dans les pays européens de l’OCDE, et étudie les retombées de cette instabilité sur la vie quotidienne, la mobilité sociale et les inégalités. L’instabilité des revenus peut être difficile à gérer pour les personnes qui ne disposent pas des ressources financières nécessaires pour lisser leurs revenus. Dans ce rapport, les personnes qui sont à la fois exposées à l’instabilité des revenus et vulnérables dans ce contexte sont considérées en situation d’insécurité économique. L’insécurité économique touche principalement les personnes qui ont des liens ténus avec le marché du travail et celles qui ne sont pas en situation de profiter des avantages tirés de la transformation numérique. Les personnes exposées au risque d’insécurité économique sont plus susceptibles que les autres d’être inquiètes quant à la possibilité de perdre leur emploi et, comme en attestent d’autres recherches, de connaître des problèmes de santé, de souffrir d’insécurité alimentaire et d’obtenir de mauvais résultats en termes de développement dans l’enfance, autant de freins potentiels à la mobilité sociale. Enfin, le rapport passe en revue une panoplie de mesures conçues pour améliorer la réactivité des systèmes de protection sociale, de sorte à mieux soutenir les personnes aux revenus très instables ; il étudie en outre différentes pistes d’action pour aider les plus exposés au risque d’insécurité économique à se constituer des réserves financières.

These results are a key input for evidence-based policies targeting inequality and fostering inclusive growth, providing insights into key dimensions of material well-being across household groups. The results complement existing inequality measures by including elements that are often lacking from micro statistics and by providing inequality measures consistent with macroeconomic aggregates, broadening the scope for analyses, while also capturing households and transactions that are typically underrepresented in micro data. Moreover, while the estimates do require a number of statistical choices and assumptions, they have a high degree of international comparability because of the common methodology and their alignment to national accounts results. The handbook provides an overview of the conceptual framework underlying the distributional results and discusses various aspects in relation to the compilation and presentation of the distributional results. It aims to assist compilers in deriving high-quality distributional results and to provide users with more insights into the main benefits of these results as well as into the way that the results have been derived.

    Kurzfassung: Finding sufficient funds to pay for more resilient health systems is challenging in the current economic context. COVID-19 has shown the need for additional targeted spending on public health interventions, the digital transformation of health systems, and bolstering the health workforce. Rising incomes, technological innovation and changing demographics put further upward pressure on health spending. This could result in health spending reaching 11.8% of GDP across OECD counties by 2040. This publication explores the policy options to finance more resilient health systems whilst maintaining fiscal sustainability. It finds that the scale of the additional health financing needs requires ambitious and transformative policy changes. Robust actions to encourage healthier populations and policies to reduce ineffective spending can put future health expenditure on a far gentler upward trajectory. These would enable spending to reach a more sustainable 10.6% of GDP in 2040. Better budgetary governance is critical. It improves how public funds for health are determined, executed and evaluated. Therefore, a focus of this report is on how good budgeting practices can increase the efficiency of current public spending, and also enable more ambitious policy changes in the medium to longer-term. Findings of this report are targeted at health and finance policy makers, with improved dialogue between health and finance ministries especially important when governments are operating in a constrained fiscal setting.

    Kurzfassung: Return migration has emerged as a critical policy concern for both destination and origin countries. While policy attention in destination countries has been focused on assisted voluntary return and reintegration (AVRR) programs, particularly for migrants with expulsion orders, these efforts represent only a fraction of broader return movements. This report, based on a project carried out by the OECD with support from the German Corporation for International Co-operation (Deutsche Gesellschaft für Internationale Zusammenarbeit GmbH – GIZ), commissioned by the Federal Ministry for Economic Co-operation and Development, analyses the scope and characteristics of different categories of return migration. The report draws on three workshops, held in Tunis, Rabat and Brussels, that discussed return migration in different contexts. It examines the multiple factors that influence migrants' decisions to return to their countries of origin and their reintegration at home, including the role of family and community. The report emphasises the pre-existing structures and networks of returning migrants in origin countries and their role in supporting different types of return migrants. The report also looks at potential re-migration.

Poleg tega je v izdaji za leto 2023 predstavljeno tematsko poglavje o stanju duševnega zdravja in z njim povezanih storitev v Sloveniji. Ta profil je rezultat sodelovanja OECD in Evropskega observatorija za zdravstvene sisteme in politike, ki je bil izveden v sodelovanju z Evropsko komisijo.

La présente synthèse résume les principales conclusions du rapport Un équilibre précaire ? Instabilité des revenus et insécurité économique en Europe, le premier produit par l’Observatoire de l’OCDE sur la mobilité sociale et l’égalité des chances. Ses auteurs ont fait appel à des techniques innovantes pour identifier les catégories de population en situation d’insécurité économique dans les pays européens de l’OCDE – autrement dit celles qui sont les plus exposées et les plus vulnérables à l’instabilité des revenus – et pour déterminer comment l’évolution des revenus et l’insécurité économique influencent les conditions de vie des individus, la mobilité sociale et les inégalités. Il passe également en revue une série de mesures propres à rendre la protection sociale plus réactive afin d’améliorer le soutien apporté à ceux dont les revenus sont instables et étudient des solutions qui permettraient d’aider ceux que l’insécurité économique guette plus particulièrement à constituer des réserves financières.

This document presents the OECD’s renewed health system performance assessment framework. It incorporates new performance dimensions, notably people-centredness, resilience, and environmental sustainability, and places increased emphasis on addressing inequalities, including those related to gender. This framework expands on existing OECD efforts in these domains and integrates the most recent advancements in health system performance assessment. By offering common definitions and fostering a shared understanding among policy makers, stakeholders and organisations, the updated framework will enhance international collaboration. Furthermore, it lays the foundation for developing future indicators, facilitating data collection, policy analysis, and the integration of knowledge.

    Kurzfassung: Early childhood education and care (ECEC) staff are central to promoting young children’s development, learning and well-being. Novice staff, defined as staff with up to three years of experience, account for 27% of staff in TALIS Starting Strong 2018. Novice staff are generally less educated than experienced staff in terms of the level of educational attainment and education or training with a focus specifically on working with children. More novice staff than experienced staff report a strong need for additional professional development in several areas, including “classroom/playgroup/group management” and “working with parents or families”. Policy approaches to support novice staff in ECEC can include ensuring that initial education prepares novice staff for their jobs by providing relevant knowledge specifically on working with children, supporting novice staff to engage in ongoing professional development, and clarifying how novice staff can progress professionally.

The report provides policy makers with fiscal, regulatory and health systems organisation tools, as well as examples of initiatives that can be undertaken in primary care, workplaces, and schools to better control cancer and counter inequalities.

Yet, for the digital transformation to be sustainable in the long term, it needs solid foundations, including adaptable governance arrangements, reliable and resilient digital public infrastructure, and a prospective approach to governing with emerging technologies such as artificial intelligence. This paper presents the main findings of the 2023 edition of the OECD Digital Government Index (DGI), which benchmarks the efforts made by governments to establish the foundations necessary for a coherent, human-centred digital transformation of the public sector. It comprises 155 data points from 33 member countries, 4 accession countries and 1 partner country collected in 2022, covering the period between 01 January 2020 and 31 October 2022.

    Kurzfassung: This paper builds on the results of the OECD Trust Survey in Portugal to identify the main areas for improvement and guidelines for action to help Portugal reinforce trust and democracy and enhance public governance to address future challenges. The Portuguese government’s response to the COVID-19 crisis drew on lessons learned from previous crises, notably the plunge in public trust after the 2008 financial crisis and the subsequent economic austerity measures which shook the political system and considerably impacted economic and social well-being. Public trust has traditionally been heralded as crucial for public governance. However, to build resilience in the face of multiple social, economic and environmental crises, Portugal will need to set public trust as an explicit policy objective and better understand what drives it. Investments in building a competent and trusted civil service will also be needed to develop more responsive and better-informed policy responses.

    Kurzfassung: La télémédecine, autrement dit les consultations cliniques à distance, n’était guère utilisée, dans la plupart des pays de l’OCDE, avant la pandémie de COVID-19, les contraintes réglementaires et la frilosité des patients et des prestataires de soins lui faisant frein. Au début de l’année 2020, alors que le COVID-19 bousculait totalement la prestation de soins en personne, les pouvoirs publics ont rapidement décidé de l’encourager. Le nombre de téléconsultations s’est alors envolé, ce qui a contribué de façon déterminante au maintien de l’accès aux soins, mais n’a qu’en partie compensé la contraction des soins en personne. Nous verrons dans la présente synthèse comment les pouvoirs publics ont amplifié les soins à distance durant la pandémie et quelles conséquences ce basculement de grande ampleur a eues sur le fonctionnement des systèmes de santé.

The report first investigates the life situation of LGBTI+ Germans by presenting the most up-to-date data on the share of Germans who self-identify as LGBTI+, evaluating the extent to which LGBTI+ Germans face discrimination and violence, and assessing how this population fares in terms of well-being, mental and physical health. The report then examines whether laws critical to achieving LGBTI+ equality have been passed and how LGBTI+ equality in Germany could be further improved through legislation. Finally, beyond laws, the report focuses on policy achievements towards LGBTI+ equality, by distinguishing between remedial policies, aimed at enforcing antidiscrimination and anti-violence laws, and preventive policies, aimed at fostering a culture of equal treatment of LGBTI+ individuals at school, in the workplace, and in healthcare.

Being physically active is one of the most important things people can do to improve their physical and mental health. It helps prevent a range of non-communicable diseases such as cardiovascular disease and cancer, and improves mental health and cognitive functioning, among other benefits. Nevertheless, too many Europeans are physically inactive. One in three European adults do not meet the WHO physical activity guidelines, and almost half never exercise or play sport. The report describes patterns and trends of insufficient physical activity in Europe. It reviews the detrimental impact that current physical inactivity levels have on population health and health expenditure. The report provides policy makers with options to address insufficient physical activity, drawing on case studies from across Europe.

Young women often reach higher levels of education than young men, but remain under-represented in fields with the most lucrative careers. Women spend more time on unpaid work, face a strong motherhood penalty, encounter barriers to entrepreneurship and fare worse in labour markets overall. They are also under-represented in politics and leadership positions in public employment. These elements permeate many policy areas and economic sectors - from international trade and development assistance to energy and the environment - in which policy often lacks a strong gender focus. Violence against women, the most abhorrent manifestation of gender inequality, remains a global crisis. This publication analyses developments and policies for gender equality, such as gender mainstreaming and budgeting, reforms to increase fathers' involvement in parental leave and childcare, pay transparency initiatives to tackle gender pay gaps, and systems to address gender-based violence. It extends the perspective on gender equality to include foreign direct investment, nuclear energy and transport. Advancing gender equality is not just a moral imperative; in times of rapidly ageing populations, low fertility and multiple crises, it will strengthen future gender-equal economic growth and social cohesion

The study also looks at social attitudes and how these might be affecting family formation and fertility trends. The final chapter projects demographic, economic, fiscal and social outcomes under different fertility trend scenarios.

    Kurzfassung: Despite strong economic performance and significant governance reforms over the past few decades, Lithuania has a higher share of its population at-risk-of poverty than other EU countries (21.4% in 2020 compared to an EU average of 16.3%), with some people more at risk than others, particularly people with multiple and complex needs in vulnerable situations who tend to rely more on public services. The provision of personalised services is fundamental to addressing the needs of people in vulnerable situations and to improving their well-being. This report is part of a joint project between the OECD and the European Commission to develop a more integrated approach to personalised services for people with disabilities, young people leaving care, and people leaving prison in Lithuania and to increase the involvement of non-governmental organisations in the design and delivery of those services. The report provides an analysis and assessment of the governance arrangements and NGO involvement in Lithuanian public service provision, the associated operating models and information technology (IT) infrastructure of employment and social services; and service design and delivery methods for the three groups. Finally, the report proposes recommendations to deliver effective, well-integrated public services to people in vulnerable situations in Lithuania.

Les femmes jeunes atteignent souvent un niveau d’études supérieur à celui de leurs homologues masculins, mais restent sous-représentées dans les secteurs offrant les rémunérations les plus élevées. Les femmes consacrent plus de temps au travail non rémunéré, sont fortement pénalisées lorsqu’elles ont des enfants, se heurtent à des obstacles en matière d’entrepreneuriat, et ont globalement de moins bons résultats sur le marché du travail. Elles sont aussi sous-représentées en politique et aux postes de direction dans le secteur public. On retrouve ces caractéristiques dans de nombreux domaines d’action et secteurs économiques (du commerce international à l’aide au développement en passant par l’énergie et l’environnement) où les questions de genre sont peu prises en compte dans l’action publique. La violence à l’encontre des femmes, expression la plus abominable des inégalités entre les genres, reste un enjeu de portée mondiale. Cette publication analyse les évolutions et les mesures en faveur de l’égalité des genres, comme la prise en compte de la dimension du genre dans les politiques et les budgets, les réformes visant à accroître la participation des pères aux congés parentaux et à la garde des enfants, les initiatives en faveur de la transparence salariale pour lutter contre les inégalités de rémunération entre les genres, ou les systèmes de lutte contre les violences basées sur le genre. Elle vise à élargir la vision de l’égalité entre les genres, afin d’inclure l’investissement direct étranger, l’énergie nucléaire et les transports. Faire progresser l’égalité entre les genres n’est pas seulement un impératif moral : en cette période marquée par un vieillissement démographique rapide, des taux de fécondité en berne et des crises qui se multiplient, les actions dans ce domaine contribueront à favoriser une croissance économique et une cohésion sociale plus respectueuses de l’égalité des genres à l’avenir. With support from the European Commission, the project combines a wide range of both quantitative and qualitative analysis. The former examines broad trends in manufacturing performance across OECD rural (TL3) regions between 2000 and 2019, with deeper dives that draw on more granular microdata in 14 OECD countries. Case studies were conducted across 12 regions in Slovenia, Germany, Italy, and France. They comprised interviews with over 300 local, regional, and national actors across government, private sector, universities, research institutes, NGOs and non-profit community organisations. The project also benefited from foresight and futures workshops conducted in January and July 2022 with experts and policymakers across OECD countries.

    Kurzfassung: Antimicrobial resistance (AMR) – the ability of microbes to resist antimicrobials – remains an alarming global health threat. This is despite the efforts made by OECD and EU/EEA countries to curtail it. Unless additional effective interventions are scaled up quickly, AMR rates are forecasted to increase in the next three decades across OECD and EU/EEA countries, with costs exceeding the healthcare expenditure on the COVID-19 pandemic. Using microsimulation and machine-learning techniques, this report analyses critical policy levers to inform the next generation of AMR initiatives. It shows that tackling the detrimental health and economic impact of AMR requires embracing a One Health framework – a collaborative, trans-disciplinary and multi-sectoral approach that promotes close co-operation and collaboration across human health, animal health, agrifood systems and the environment. This report identifies 11 One Health "best buys" that, if implemented systematically, would improve population health, reduce health expenditure and generate positive returns for the economy.

It plays a pivotal role in achieving climate neutrality and resilience but is also susceptible to certain risks. As infrastructure has a long lifespan, it is particularly vulnerable to the impacts of climate change over time. Poor planning and management of infrastructure assets can also increase dependency on fossil fuels and lock in climate risks. It is thus important that infrastructure is implemented in a way that helps address environmental and climate challenges. Developing effective strategies to meet environmental and climate objectives is one of the main priorities of the Irish Government. Project Ireland 2040 offers a unique opportunity to ensure that infrastructure investments support climate mitigation and adaptation efforts. Building on this impetus, the OECD has provided technical support to the Irish Government to strengthen climate and environmental considerations in its public infrastructure decision making process. This policy paper sets out a detailed overview of infrastructure planning, project appraisal and budgeting practice in Ireland, and identifies challenges and opportunities for environmental and climate considerations.

    Kurzfassung: Advancements in artificial intelligence (AI) are laying the groundwork for extensive and rapid transformations in society. Understanding the relationship between AI capabilities and human skills is essential to ensure policy responsiveness to ongoing and incoming changes. The OECD has tracked how well AI systems fare on tasks from the Programme for International Student Assessment (PISA), comparing AI performance to that of 15-year-old students in the test's core domains of reading, mathematics and science. Tests were conducted using the Generative Pre-Trained Transformer (GPT) family of large language models (LLMs), the AI behind ChatGPT, which took the world by storm after its public release in late 2022. Results show that both GPT versions outperform average student performance in reading and science. In addition, we observe rapid advances in mathematics where AI capabilities are quickly catching up with those of students. In November 2022, GPT-3.5 could answer 35% of a set of PISA mathematics tasks, a level of performance significantly below that of humans, who answer 51% of the tasks successfully on average. However, by March 2023, GPT-4 answered 40% of the tasks successfully. Policy implications of these results are discussed in this paper.

With close to two-thirds of all growth in productivity from 2010 to 2018 coming from better use of resources in remote rural areas, rural areas are critical to economic prosperity. In addition to the valuable heritage that they offer, rural areas in Scotland have had remarkable achievements over the past decade in terms of economic growth, social innovation and progress for climate mitigation. Unlike the demographic decline perceived in OECD countries, the decline in rural areas of Scotland is much slower. In the case of accessible rural areas, the population is still growing. In part, much of the progress of ensuring well-being in rural areas, is supported by mechanisms to link entrepreneurs with researchers, regional support for entrepreneurs and an established system of social entrepreneurship. This report sets the scene, reviews main policies and programmes impacting rural innovation, and explores the importance of social innovation in rural areas. It provides recommendations to reinforce some of the good practices in Scotland, which is particularly important as the government implements the new National Innovation Strategy.

Drawing on a representative sample of 27 000 respondents across 27 OECD countries, RTM illustrates respondents' perceived economic risks, levels of satisfaction with current social policies, and preferences for future government action on social protection. People are calling on governments to help with the cost-of-living crisis, to spend more on health post COVID-19, and to reinforce support for older people, including in long-term care. Based on a comprehensive cross-national survey of perceptions of social protection, this report offers lessons for the functioning of social programmes as countries emerge from COVID-19, manage the cost-of-living crisis, and plan for future challenges.

Local job creation has been driven by ample opportunities for highly-skilled workers whom the Brussels-Capital Region attracts from its surrounding regions, the European Union, and beyond. However, not all of the region's population has seen their labour market prospects improve equally. Many workers with lower levels of education continue to compete for a small number of vacancies, despite the increasingly tightening labour market. A future vision for the Brussels-Capital Region, therefore, needs to be built on recognising and addressing the multiple and often distinct challenges faced by its young and diverse population. In a labour market that offers high rewards for skills formation, tailoring continuous education and training offerings to different groups while removing barriers to participation in such programmes will need to take priority. Simplifying processes for accessing employment services, streamlining labour market policies, supporting the geographic mobility of less-educated jobseekers, and removing barriers to employment for its migrant population could serve as building blocks to increase labour force participation and improve the employment quality of all local talent.

The identification of socio-economic factors, in particular, influencing the demand for counterfeit and pirated goods is crucial to help policymakers develop appropriate policies, including better targeting of consumer awareness campaigns.

    Kurzfassung: Antofagasta is a world leader in copper and lithium production, with strategic importance for the global energy transition and for the economic development of Chile. Located in north Chile, Antofagasta is carved by the natural contours of the Atacama Desert and home to diverse Indigenous communities. Despite the wealth brought by mining, communities in Antofagasta lag on a number of well-being dimensions. At the same time, Antofagasta's mining industry is entering a new phase of development, driven by the expected surge in global demand for its minerals and the imperative to adapt to the green and digital transitions. Against this backdrop, a new development vision with a long-term strategy is warranted in the region to leverage mining benefits to improve well-being standards and take advantage of the opportunities brought by the digital and green transition in mining. This study presents the diagnosis, rationale and building blocks for a new mining strategy in the region of Antofagasta, Chile that prioritises well-being standards and opportunities for local communities. This medium and long-term strategy aims to create a new pact amongst different societal stakeholders to build trust and unite efforts for more inclusive and sustainable growth in the region.

    Kurzfassung: The 2021 OECD Survey on Drivers of Trust in Public Institutions provides a cross-country assessment of what influences people’s trust in government and public institutions, understanding people’s perception of the functioning of democratic institutions. Most of the drivers in the Trust Survey refer to government competences and values that help countries take concrete steps to rebuild trust. Both at the individual and country-level trust may vary due to cultural, economic and social factors. This paper analyses the open-ended survey responses in 16 OECD countries to understand two questions. First can the answers provide insights into cultural differences and extreme attitudes of distrust? Second, what dimensions influence trust that are not considered in the theoretical OECD Trust Framework underpinning the Trust Survey? The results of the topic model resemble closely the drivers in the Trust Framework, but respondents name not just government inputs, outputs, and processes, but also socioeconomic vulnerabilities, intergenerational and global challenges. While many respondents write that ''corruption'', ''money'' and ''power'' drives their trust in public institutions, these feelings are not associated with government transparency and accountability. These findings show the advantage of qualitative measurements (open-ended survey questions) to understand the complex relationship between trust and public governance.

    Kurzfassung: Sweden is undergoing a major reform of its public employment service (PES) Arbetsförmedlingen, shifting its main role from providing in-house services towards monitoring of providers and working with different stakeholders in guiding and implementing labour market policies. At the same time, the PES is undergoing a significant restructuring, resulting in a downscaling of physical presence across the country and an increased digitalisation of services. To support this reform and services to jobseekers across urban and rural settings, this report a describes the main features of the Swedish labour market and employment system and analyses the challenges of the reform from a local perspective. In light of international examples, it outlines policy options for contracting services to ensure coverage in all places and for all jobseekers, managing the balance between physical and digital services, and coordinating services at the local level.

This study aims to support these efforts by analysing the judicial workforce and relevant support structures and processes currently employed by the Irish courts. In particular, the study seeks to contribute to the deliberations of the Irish Judicial Planning Working Group, which was established to identify reform initiatives and evaluate staffing needs to enhance the efficient administration of justice over the next five years.

Poté analyzuje celou řadu zásadních a prioritních oblastí veřejné správy včetně zapojování občanů, koordinace a strategického plánování vedených z centra vládnutí, informovaného rozhodování (EIDM) ve veřejné správě, veřejné správy na úrovni krajů a obcí, řízení lidských zdrojů ve veřejné správě a využívání elektronických nástrojů veřejné správy. Součástí Přehledu je i případová studie o opatřeních a předpisech v oblasti veřejné správy během pandemie onemocnění covid-19. Přehled o stavu veřejné správy v ČR obsahuje doporučení pro veřejnou správu v ČR, která jí pomohou s uskutečněním a se splněním cílů koncepce rozvoje veřejné správy s názvem Klientsky orientovaná veřejná správa 2030 a v konečném důsledku také s realizací dlouhodobé vize a závazků udržitelného rozvoje.
