Ce document présente le nouveau cadre d'évaluation de la performance des systèmes de santé de l'OCDE. Il intègre de nouvelles dimensions de la performance, notamment la question des soins centrés sur la personne, la résilience et le développement durable, et met davantage l'accent sur la lutte contre les inégalités, y compris celles liées au genre. Ce cadre s'appuie sur les efforts déjà déployés par l'OCDE dans ces domaines et intègre les avancées les plus récentes en matière d'évaluation de la performance des systèmes de santé. En proposant des définitions communes et en favorisant une compréhension partagée entre décideurs, parties prenantes et organisations, le cadre renouvelé facilitera la collaboration internationale. En outre, il permettra l'élaboration de futurs indicateurs, facilitera la collecte de données, l'analyse des politiques et l'intégration des savoirs.

    Abstract: Les mutations économiques et technologiques des dernières décennies ont rendu les perspectives économiques individuelles plus incertaines. Si les formes d’emploi atypiques et la transformation numérique ont ouvert le champ des possibles, elles ont aussi, comme les grands chocs économiques récents, exposé de nombreuses personnes à une fluctuation de leurs revenus, un phénomène désigné sous le nom « d’instabilité des revenus ». Dans la mesure où la situation professionnelle et personnelle des individus peut changer à plusieurs reprises au cours d’une même année, ce rapport s’appuie sur de nouvelles techniques pour déterminer les personnes les plus exposées à l’instabilité des revenus dans les pays européens de l’OCDE, et étudie les retombées de cette instabilité sur la vie quotidienne, la mobilité sociale et les inégalités. L’instabilité des revenus peut être difficile à gérer pour les personnes qui ne disposent pas des ressources financières nécessaires pour lisser leurs revenus. Dans ce rapport, les personnes qui sont à la fois exposées à l’instabilité des revenus et vulnérables dans ce contexte sont considérées en situation d’insécurité économique. L’insécurité économique touche principalement les personnes qui ont des liens ténus avec le marché du travail et celles qui ne sont pas en situation de profiter des avantages tirés de la transformation numérique. Les personnes exposées au risque d’insécurité économique sont plus susceptibles que les autres d’être inquiètes quant à la possibilité de perdre leur emploi et, comme en attestent d’autres recherches, de connaître des problèmes de santé, de souffrir d’insécurité alimentaire et d’obtenir de mauvais résultats en termes de développement dans l’enfance, autant de freins potentiels à la mobilité sociale. Enfin, le rapport passe en revue une panoplie de mesures conçues pour améliorer la réactivité des systèmes de protection sociale, de sorte à mieux soutenir les personnes aux revenus très instables ; il étudie en outre différentes pistes d’action pour aider les plus exposés au risque d’insécurité économique à se constituer des réserves financières.

    Abstract: Parental emotional support, alongside material and temporal support, is an important determinant of children's subjective well-being and academic success. However, not all children benefit from the same level of parental support, and there are major differences depending on families' socio-economic status and child gender. Using the PISA 2018 surveys, this paper examines differences in parental support reported by 15-year-olds both within countries according to social status and between girls and boys, and between countries. We show that differences in parental emotional support by parents' education level and child gender are substantial. Some of these differences are (largely) explained by other characteristics such as family wealth, country of origin, and school urbanicity and private/public status. Greater parental emotional support is also found to be associated with higher PISA test scores and greater subjective wellbeing, with little variation by parental education. On the whole, our findings suggest that a significant enhancement in parental support and related child outcomes, especially in countries with lower average levels of parental emotional support, can be attained through a combined effort on several fronts: by addressing monetary and material poverty within families, by facilitating parents in balancing work and taking care of their children, by promoting greater parental involvement in their children's school life, and by offering appropriate services to assist families with special needs and facing greater challenges.

Novice staff, defined as staff with up to three years of experience, account for 27% of staff in TALIS Starting Strong 2018. Novice staff are generally less educated than experienced staff in terms of the level of educational attainment and education or training with a focus specifically on working with children. More novice staff than experienced staff report a strong need for additional professional development in several areas, including “classroom/playgroup/group management” and “working with parents or families”. Policy approaches to support novice staff in ECEC can include ensuring that initial education prepares novice staff for their jobs by providing relevant knowledge specifically on working with children, supporting novice staff to engage in ongoing professional development, and clarifying how novice staff can progress professionally.

    Abstract: In many countries, public expenditure, including transfers, plays a major role in reducing income inequality. The report reviews the various ways that budgeting can be used to this end. A first includes taking a broad approach to results-based budgeting, taking social and distributional goals into consideration. A second relies on integrating distributional impact analysis directly into the budget process. The report discusses the concrete experience of eight OECD countries in this area, analysing how they are integrating distributional impact assessment in spending and budgeting decisions. Finally, it discusses the tools, frameworks and data that are needed to take distributional considerations into account as part of evidence-informed policy making.

    Abstract: Return migration has emerged as a critical policy concern for both destination and origin countries. While policy attention in destination countries has been focused on assisted voluntary return and reintegration (AVRR) programs, particularly for migrants with expulsion orders, these efforts represent only a fraction of broader return movements. This report, based on a project carried out by the OECD with support from the German Corporation for International Co-operation (Deutsche Gesellschaft für Internationale Zusammenarbeit GmbH – GIZ), commissioned by the Federal Ministry for Economic Co-operation and Development, analyses the scope and characteristics of different categories of return migration. The report draws on three workshops, held in Tunis, Rabat and Brussels, that discussed return migration in different contexts. It examines the multiple factors that influence migrants' decisions to return to their countries of origin and their reintegration at home, including the role of family and community. The report emphasises the pre-existing structures and networks of returning migrants in origin countries and their role in supporting different types of return migrants. The report also looks at potential re-migration.

    Abstract: Finding sufficient funds to pay for more resilient health systems is challenging in the current economic context. COVID-19 has shown the need for additional targeted spending on public health interventions, the digital transformation of health systems, and bolstering the health workforce. Rising incomes, technological innovation and changing demographics put further upward pressure on health spending. This could result in health spending reaching 11.8% of GDP across OECD counties by 2040. This publication explores the policy options to finance more resilient health systems whilst maintaining fiscal sustainability. It finds that the scale of the additional health financing needs requires ambitious and transformative policy changes. Robust actions to encourage healthier populations and policies to reduce ineffective spending can put future health expenditure on a far gentler upward trajectory. These would enable spending to reach a more sustainable 10.6% of GDP in 2040. Better budgetary governance is critical. It improves how public funds for health are determined, executed and evaluated. Therefore, a focus of this report is on how good budgeting practices can increase the efficiency of current public spending, and also enable more ambitious policy changes in the medium to longer-term. Findings of this report are targeted at health and finance policy makers, with improved dialogue between health and finance ministries especially important when governments are operating in a constrained fiscal setting.

The results complement existing inequality measures by including elements that are often lacking from micro statistics and by providing inequality measures consistent with macroeconomic aggregates, broadening the scope for analyses, while also capturing households and transactions that are typically underrepresented in micro data. Moreover, while the estimates do require a number of statistical choices and assumptions, they have a high degree of international comparability because of the common methodology and their alignment to national accounts results. The handbook provides an overview of the conceptual framework underlying the distributional results and discusses various aspects in relation to the compilation and presentation of the distributional results. It aims to assist compilers in deriving high-quality distributional results and to provide users with more insights into the main benefits of these results as well as into the way that the results have been derived.

    Abstract: The OECD Income Distribution Database (IDD) plays a leading role in providing evidence, and in monitoring and analysing international income distribution statistics to inform policy debate. In most OECD countries, official income distribution statistics are usually delivered with time lags varying from two to three years. This paper examines the growing use by statistical offices of nowcasting techniques based on microsimulation models to produce more timely provisional estimates, and examines the advantages and challenges associated with these techniques.     Abstract: This paper lays out an approach, and a research agenda, for assessing the impact of carbon pricing on household budgets. It relies on a rich set of available data and policy models and combines them in a way that is informative for mapping the gains and losses at the household level in the short term as countries transition to a low-carbon economy. After accounting for direct burdens from higher fuel prices, indirect effects from higher prices of goods other than fuel, and households’ behavioural responses, overall burdens are only mildly regressive. Recycling carbon-tax revenues back to households allows considerable scope for avoiding or cushioning losses for large parts of the population, and existing policy models can be used to design compensation measures that facilitate majority support for carbon tax packages.

Different service delivery providers such as health, justice, housing and social protection must work together seamlessly – across governmental and non-governmental providers – to provide an effective response. This report presents a stocktaking of OECD governments’ efforts to integrate service delivery to address the most prevalent form of gender-based violence against women: intimate partner violence. It presents an overview of different strategies for coordinating key services commonly offered in OECD countries: healthcare, justice, housing, child services, income support, and preventative programmes to stop the reoccurrence of violence. Based on extensive feedback from 35 out of 38 OECD countries and a consultation with non-governmental service providers, this report identifies best practices and investigates the barriers to resolving one of the most pressing human rights issues in OECD countries today.

    Abstract: At a time of rising cost of living, wide wage inequalities and widespread in-work poverty, the demand for a living wage has heightened. The concept of a “living wage” has some limitations, including that it is operationalised in a variety of ways. This variety may serve the purpose of making it a more relevant instrument, typically by providing information on the cost of living that firms and social partners may embed in their wage-setting processes; however, the variety can also increase a lack of transparency. The paper reviews some of the most common methodologies, by identifying points of convergence and divergence. Living wage estimates produced by the Fair Wage Network are then put into context by benchmarking them against internationally comparable wage metrics and poverty lines. Finally, the paper presents a number of critical steps to strengthen the concept of a living wage. This paper does not assess the economic cost or feasibility of living wages, not at the firm level or at the broader industry and economy level. This paper advises using the living wage as one of the pieces of information that – when properly contextualised – could inform wage negotiations and wage policies set in consultation with social partners.

    Abstract: There is ample evidence that exposure to various chemicals can increase the probability of children to be born with low or very low birth weight. Infants born with very low birth weight have a higher risk of suffering from neurosensory problems, issues related to behavioural and social competencies, and learning disabilities than infants born with normal birth weight. Authorities face challenges in regulating chemical substances through actions such as bans and prohibitions, because of the difficulty in explicitly considering the economic benefits and costs of such regulations. Moreover, existing Values of a Statistical Case (VSC) of very low birth weight are rare and cannot be directly applied to the cost benefit analysis of chemical management options for a wide range of countries. This paper is part of the series of large scale willingness to pay (WTP) studies resulting from the Surveys to elicit Willingness to pay to Avoid Chemicals related negative Health Effects (SWACHE) project that intends to improve the basis for doing cost benefit analyses of chemicals management options and environmental policies in general. The present paper details a stated preference survey estimating WTP to reduce the risk of very low birth weight, filling an important gap in the valuation literature and addressing a need for applied benefits analysis for chemicals regulation. The SWACHE very low birth weight survey was fielded in 9 countries: Canada, the Czech Republic, Italy, Mexico, the Netherlands, Switzerland, Türkiye, the United Kingdom, and the United States.

    Abstract: The health system performance assessment (HSPA) framework for the Czech Republic is an initiative designed to help the Czech health system improve policy planning, monitoring, and decision taking. This report describes the HSPA framework for the Czech Republic, its development process, governance structure and implementation roadmap. It also details the Czech HSPA framework domains, populated by indicators selected through a comprehensive review process. As such, the framework enables the assessment of strengths and weaknesses of the Czech health system. Its implementation will increase the accountability of national authorities and principal healthcare stakeholders, improve public involvement, smooth flow of information across the health sector, and allow reform planning and monitoring.

This paper explores integration of employment services with other services (e.g. social, health and housing) at the local level as one promising way for governments to provide more effective support that is holistic and person-centred and be more efficient in public spending. The paper analyses different forms of local service integration, the possible benefits, and the particular roles subnational governments can play in service integration reforms – ranging from national public employment service-led reforms to more locally driven reforms. The paper also analyses the barriers to service integration and proposes future work to strengthen local service integration.

    Abstract: This paper documents the development and piloting of the OECD Skills Profiling Tool. The OECD Skills Profiling Tool assesses three types of skills: occupation-specific skills; foundational skills (literacy, numeracy and digital skills); and a set of noncognitive skills using academically validated self-reported tests. After completion of the assessment, the OECD Skills Profiling Tool generates two sets of results: a personalised skill profile, which can be benchmarked against other users; and a list of suggested occupations that make use of those skills. In December 2021, 270 users and 38 career guidance counsellors in Chile, Colombia, Mexico and Peru tested the OECD Skills Profiling Tool. This paper describes the selection of assessment instruments, the methodology used to generate the results provided by the OECD Skills Profiling Tool and the outcomes of the piloting phase of the tool.

    Abstract: This report provides an overview of the policies and procedures for addressing child sexual exploitation and abuse (CSEA) material across the global top-50 online content-sharing services. It finds that only 10 of the 50 services define CSEA with sufficient detail to understand what is prohibited on their services, and only 20 of the services issue a transparency report on CSEA. Even among those services, there are significant variations in what behaviour is captured in their definitions, and the metrics, methodology and frequency of transparency reports differ across platforms. While good practices exist, the report reveals a fragmented response to this complex and evolving problem, which limits comparability and makes it challenging to conduct a thorough assessment of the overall impact of platforms' efforts to combat CSEA.

    Abstract: Advancements in artificial intelligence (AI) are laying the groundwork for extensive and rapid transformations in society. Understanding the relationship between AI capabilities and human skills is essential to ensure policy responsiveness to ongoing and incoming changes. The OECD has tracked how well AI systems fare on tasks from the Programme for International Student Assessment (PISA), comparing AI performance to that of 15-year-old students in the test's core domains of reading, mathematics and science. Tests were conducted using the Generative Pre-Trained Transformer (GPT) family of large language models (LLMs), the AI behind ChatGPT, which took the world by storm after its public release in late 2022. Results show that both GPT versions outperform average student performance in reading and science. In addition, we observe rapid advances in mathematics where AI capabilities are quickly catching up with those of students. In November 2022, GPT-3.5 could answer 35% of a set of PISA mathematics tasks, a level of performance significantly below that of humans, who answer 51% of the tasks successfully on average. However, by March 2023, GPT-4 answered 40% of the tasks successfully. Policy implications of these results are discussed in this paper.

The objective of this publication is to describe experiences in purchasing arrangements and payment methods and how they have been used to attain quality of care and health outcomes for chronic conditions. The publication builds on the existing body of empirical evidence and newly commissioned case studies from Australia, Canada, Chile, China, Germany, Indonesia, South Africa, and Spain to better understand the design of different purchasing arrangements that aim to promote quality for chronic disease care. We identify lessons learned in alignment of payment methods with service delivery models, key design issues in quality and payment, supporting purchasing policies, and monitoring and evaluation. Recognising that no single model is applicable to all settings, the publication aimed to share lessons learned and best practices and identify areas for future research, particularly in low- and middle-income settings.

Selected mental health initiatives across OECD countries are reviewed to illustrate how policy makers have been realigning action across government agencies; redesigning policy formulation to address the joint factors influencing mental health; refocusing efforts towards the promotion of positive mental health; and reconnecting with societal stakeholders beyond government, including those with lived experience, youth, civil society and research institutions.     Abstract: The demand for help with daily activities – so-called long-term care – is set to increase in Croatia. The population is ageing at a faster rate than the EU average, and by 2050, about one-third of the population is projected to be aged 65 years and over. In addition, with one of the highest poverty rates among older people in EU countries, at 30%, most older people cannot afford long-term care without public support. However, the long-term care system is both fragmented, with multiple benefits and services across different providers, and underfunded with public expenditure among the lowest across EU countries. As a result, long-term care remains unaffordable for most people even after receiving public support, leading to gaps in access, inequities, and a strong reliance on relatives to provide the bulk of long-term care. This report suggests avenues to improve access and equity of long-term care and proposes policy recommendations to enhance the support for family carers.

    Abstract: The OECD review of Gender Equality in Colombia: Towards a Better Sharing of Paid and Unpaid Work is the third in a collection of reports focusing on Latin American and the Caribbean countries, and part of the series Gender Equality at Work. The report compares gender gaps in labour and educational outcomes in Colombia with other countries. Particular attention is put on the uneven distribution of unpaid work, and the extra burden this places on women. It investigates how policies and programmes in Colombia can make this distribution more equitable. The first part of the report reviews the evidence on gender gaps and on what causes these, including the role played by attitudes. The second part develops a comprehensive framework to address these challenges, presenting a broad range of options to reduce the unpaid work burden falling on women, and to increase women's labour income. Earlier reviews in the same collection have looked at gender equality policies in Chile (2021) and Peru (2022).

    Abstract: New OECD surveys of employers and workers in the manufacturing and finance sectors of seven countries shed new light on the impact that Artificial Intelligence has on the workplace —an under-researched area to date due to lack of data. The findings suggest that both workers and their employers are generally very positive about the impact of AI on performance and working conditions. However, there are also concerns, including about job loss—an issue that should be closely monitored. The surveys also indicate that, while many workers trust their employers when it comes to the implementation of AI in the workplace, more can be done to improve trust. In particular, the surveys show that both training and worker consultation are associated with better outcomes for workers.

    Abstract: The COVID 19 pandemic has disproportionately hit some vulnerable population groups. Those living in deprived areas, migrant population, and ethnic minorities are at higher risk of catching and dying from the virus than other groups, and they also face significant indirect health impacts of the COVID-19 pandemic - both mental health impacts and disruption of routine care. The working paper gathers evidence on the direct and indirect health impacts of the COVID-19 on the poor population and the ethnic minorities. It reviews factors underlying these inequalities, and maps policy interventions adopted by OECD countries to help address the disproportionate impacts of the COVID-19 pandemic on vulnerable population groups.

The number of teleconsultations skyrocketed, playing a vital role in maintaining access to care, but only partly offsetting reductions in in-person care. This brief describes how governments scaled up remote care during the pandemic and explores the impact that this massive shift to remote care has had on health care system performance.

    Abstract: This paper provides an overview of the work of the Expert Group on the Joint Distribution of Income, Consumption and Wealth at Micro Level (EG ICW) set up by Eurostat and the OECD. It discusses the challenges of producing joint income, consumption and wealth estimates, assesses their quality, and presents selected experimental results. Although the analysis reveals large differences between countries, a number of general patterns emerge. First, income, consumption and wealth are partially correlated, with the association being stronger in the tails of the joint distribution than around its middle. Second, risk of poverty goes beyond income, with asset and consumption risk of poverty being widespread, especially among some population groups. Third, a large share of households spend more than they earn. This is corroborated by negative median saving rates for households in the bottom income quintile. Fourth, inequalities are significantly higher when using a comprehensive measure of material living standards than a distributional analysis of disposable income would suggest. Looking ahead, this paper calls for further efforts to improve the robustness of the results.

    Abstract: With more chronic conditions and an ageing population, a growing share of the population will need end-of-life care, reaching close to 10 million people by 2050. While end-of-life care services help improve quality of life through relieving pain and other symptoms, currently, there are substantial gaps in the provision of services. More than half of deaths still occur in hospital across OECD countries, despite home generally being the preferred place of death, because of lack of appropriate services and poor care co-ordination. There are marked inequalities in access with people from higher education twice as likely to receive end-of-life care services than those with low education. Planning, recording of wishes and shared decision-making fall short of what is desirable, with only one-quarter of wishes being recorded in the form of advance directives. People at the end of life do not always receive appropriate symptom relief, while others suffer from overtreatment. This report suggests avenues to improve care for the dying by enhancing measurement and monitoring of quality and rethinking funding and governance models of care. End-of-life care could also be improved by increasing knowledge among health care staff across different care settings, and making communication around the topic more person-centred.

    Abstract: This report calls on policy makers to step up the policy response to increase physical activity. Being physically active is one of the most important things people can do to improve their physical and mental health. It helps prevent a range of non-communicable diseases such as cardiovascular disease and cancer, and improves mental health and cognitive functioning, among other benefits. Nevertheless, too many Europeans are physically inactive. One in three European adults do not meet the WHO physical activity guidelines, and almost half never exercise or play sport. The report describes patterns and trends of insufficient physical activity in Europe. It reviews the detrimental impact that current physical inactivity levels have on population health and health expenditure. The report provides policy makers with options to address insufficient physical activity, drawing on case studies from across Europe.
