Abstract: This report, part of a series covering around 20 OECD countries, contains a survey of the Finland's main barriers to employment for older workers, an assessment of the adequacy and effectiveness of existing measures to overcome these barriers, and a set of policy recommendations to further action by public authorities and social partners. These recommendations are designed to alleviate some of the pension and health care pressures governments are facing because of ageing populations.

Abstract: With one of the most rapidly ageing populations in the OECD area, over the longer term, Korea faces the possibility of labour force contraction and related economic problems. This book examines the situation of older workers in Korea and what reforms are needed to keep more of them in the work force. It is part of a series of reports looking at Ageing and Employment Policies in individual OECD countries.

Abstract: This report provides the first-ever comparative analysis of the role and performance of private health insurance (PHI) in OECD countries. It analyses PHI markets and identifies policy issues arising from their interdependence with publicly financed health coverage schemes. The report assesses the impact of PHI against health policy objectives, paying special attention to the challenges and benefits associated with different insurance mixes. The analysis identifies strengths as well as areas where private health insurance might pose challenges to health system performance. This report shows how governments can help ensure that PHI markets make a positive contribution to the performance of health systems. Examples of useful practices for developing more efficient and equitable health insurance markets are also presented.

Abstract: This paper presents results from an on-going joint European Commission / OECD project, aimed at monitoring the direct influence of tax and benefit instruments on household incomes. The project uses and extends OECD tax-benefit models to compute a range of work incentive indicators such as marginal effective tax rates on earned income. This paper provides a methodological background describing these extensions. It also discusses the usefulness of a range of indicators such as net replacement rates and marginal effective tax rates and to what extent they can be used to quantify possible work disincentives. The approaches are illustrated using detailed tax-benefit calculations for 2001 and comparing relevant indicators across 15 EU and 8 non-EU countries. The results presented in this paper permit the identification of family circumstances where (1) financial incentives to increase work are either small or missing altogether; or (2) resources provided by social transfers may be ...

Abstract: Most Western countries have a long tradition of employment service provision by public bodies and non-profit organisations, but not by for-profit organisations. The creation of a quasi-market arrangement is not a simple choice for government. This paper underscores the difficulties and discusses design options. It calls for a complex balancing act between regulation and the creation of room for market competition, between old and new instruments and procedures, and between efficiency and equity. It identifies as two key issues the need to actively create sufficient room for market competition, and the need for "positive creaming" which encourages providers to concentrate their efforts on the most disadvantaged target groups. The paper starts by stating the principles and preconditions for market competition, with a focus on possible quasi-market arrangements for strengthening service provision for the long-term unemployed in Flanders. It sets out four models for contracting with ...

Abstract: • Sound national systems of corporate governance are essential for all countries, including the poorest, to reap the benefits of globalisation. • “Corporate governance” comprises the institutions that govern the relationship between people who manage corporations and all others who invest resources in them. • The quality of local corporate governance critically affects a country’s ability to achieve sustained real productivity growth and the success of its long-term development efforts. • Pyramidal corporate-ownership structures, cross shareholdings and multiple share classes are widely used by corporate insiders in the developing world to extract corporate-control rents, exploit other investors and resist pressures to improve corporate governance. • The power of corporate insiders and their close relationship with those who exercise political power mean that sound corporate governance requires sound political governance, and vice versa.

Abstract: • Despite post-Monterrey donor initiatives, the Millennium Development Goals (MDGs) are underfinanced. • The revenue potential, the additionality and the speed of availability of new finance sources, and their political feasibility, are of particular importance. • On these criteria, it is unlikely that global taxes will be introduced in time. • The International Finance Facility, strengthened use of public guarantees and Global Premium Bonds, perhaps in combination, may stand a better chance of providing additional funds for the MDGs. • The most straightforward way to avoid underfunding of the Goals is to raise ODA further.

Abstract: The institutional framework of a country plays a determining role in the well-being of the women who live in it. This paper examines the cases of four case countries: India, Kenya, Sudan and Tunisia. In each of these case studies, the status of women has been analysed along with the reforms that have been implemented to improve it. Comparisons between the four countries have brought to light several obstacles to change in the institutional framework such as age-old traditions, religion, economic interests and parliamentary opposition. Where there are important initial obstacles, the government’s room for manoeuvre is limited, particularly in the short-term since magistrates, administrations, media and families often endeavour to postpone the application of new, more inclusive, laws. There are, however, examples of successful reform efforts. These successes are generally attributable to the presence of numerous women in parliament and government and in the legal service, the ...

Abstract: This study updates and extends a previous study on equity in physician utilisation for a subset of the countries analyzed here (Van Doorslaer, Koolman and Puffer, 2002). It updates results to 2000 for 13 countries and adds new results for eight countries: Australia, Finland, France, Hungary, Mexico, Norway, Switzerland and Sweden. Both simple quintile distributions and concentration indices were used to assess horizontal equity, i.e. the extent to which adults in equal need for physician care appear to have equal rates of medical care utilisation. With respect to physician utilisation, need is more concentrated among the worse off, but after “standardizing out” these need differences, significant horizontal inequity favoring the better off is found in about half of the countries, both for the probability and the total number of visits. The degree of pro-rich inequity in doctor use is highest in the US, followed by Mexico, Finland, Portugal and Sweden. In the majority ...

Abstract: The present level of ODA falls short of the amount needed to finance the Millennium Development Goals (MDGs). The figure of additional $50 billion per year, roughly the present total of ODA spent by DAC donors, is often quoted (e.g. by the Zedillo Report); it results from the sum of the fight against communicable diseases ($ 7-10 billion), primary schooling ($10 billion), infant and maternal mortality ($12 billion) and halving world poverty ($20 billion). The scarcity of public resources raises the importance of investing in international public goods as the cost of lifting one person out of income poverty, for example through agricultural research and global trade expansion, is estimated to be much lower than the cost of the same impact through traditional aid to poor countries. This raises important issues for donor strategies, in particular principles of aid allocation, which this paper aims to address. First, should aid be partly earmarked towards international public goods? ...

Abstract: Land dynamics are context specific and rapidly changing, and conflicts related to them do not systematically escalate into violence. One way of framing the discussion is to consider change in the structures of power governing the management of resources in rural areas as necessary to achieve greater efficiency and equity. Since such change will be opposed by beneficiaries of the system in place, the transformation of agriculture is bound to be marred by conflicts of various intensity levels. At the heart of these conflicts lies land because of its very high material and symbolic values. Evidence shows that (a) whether they result from pre-existing agrarian tensions or not, conflict situations in rural societies deeply affect the politics of land, and (b) whether it is at the heart of a conflict or gets dragged into it, land requires a careful approach by policy makers because it is a central element in the evolution of societies. As a result, policies pertaining to land can ...

Abstract: Monitoring changes in public awareness and attitudes in rich countries towards aid and other international development policy issues is extremely difficult: due to lack of systematic polling or monitoring, there is no reliable, comparable data across DAC member countries. This paper suggests a way to address this problem: a common questionnaire for all DAC countries to use in their national surveys, centred on the Millennium Development Goals. It sums up the process that led to the setting up of this questionnaire, provides a short analysis of the main methodological issues and brings together three versions, respectively for face-to-face, phone or mail processing. These should allow information and communication professionals, as well as policy makers, to improve the comparability of public opinion surveys in OECD DAC member countries on development and the MDGs, especially if — as proposed by the European Commission’s Directorate General for Development — parts of the ...

Abstract: The purpose of the System of Health Accounts Changes in health systems and concomitant health policy questions have been challenging the traditional system of health expenditure statistics over the last couple of decades. What are the major factors accounting for health expenditure growth? What factors explain the differences between countries in expenditure growth? How to ensure sustainable financing? What are the major factors accounting for the differences in the structure of health spending? How are the changes in health spending structure and the performance of health systems related? In order to answer such questions, reliable, comparable and appropriately detailed health expenditure data are required. The System of Health Accounts intends to provide the foundation for health statistics that are able to meet these challenges. Box 1 What is the System of Health Accounts? The System of Health Accounts (SHA) proposes an integrated system of comprehensive and internationally ...

Abstract: Dementia and its most common manifestation, Alzheimer’s disease, is a complex disorder that afflicts primarily the elderly, affecting an estimated 10 million people in OECD member countries. The complexity of the disease makes treating dementia extremely difficult, involving a wide variety of social and health care interventions. Typically, these two aspects of dementia care are examined separately. This paper adopts a conceptual model that examines both types of interventions and how they interact along the dementia care continuum. There are no effective health care treatments for stopping dementia, which is why the social care aspect plays an important role in treating the disease, with family members an integral part of this process. This paper shows that programs designed to help alleviate the burden of family members caring for a relative with dementia can have positive health benefits to both patient and family. In particular, the use of group-living, where dementia ...

Based on a review of existing indicators and an assessment of gaps left open by existing indicators, the experts set out to select indicators to cover clinical processes of diabetes care as well as proximal and distal outcomes of care. The review led to a recommendation of nine indicators: annual HbA1c testing, annual LDL cholesterol testing, annual screening for nephropathy, annual eye exam, HbA1c control, LDL cholesterol control, lower extremity amputation rates, kidney disease in persons with diabetes, and cardiovascular mortality in patients with diabetes.

Abstract: Until recently, an unemployed person in Sweden who participated in an active labour market programme earned entitlement to a further 60 weeks of unemployment insurance (UI) benefits. A "carousel" effect - cycling between periods of open unemployment and participation in active programmes - increasingly seen came to be seen as a factor making the active programmes less effective in promoting the transition to ordinary work. Following policy reforms in 2000 and 2001, participation in a programme no longer generates new UI entitlements. However, the public employment service (PES) can refer the unemployed to a new programme, the Activity Guarantee. This provides continuing income support, and engages participants in job-search and other activities in principle full-time until they find ordinary work. This paper summarises findings from two questionnaire surveys of the implementation of the Activity Guarantee. The first survey was addressed to PES case workers, who have direct ...

Abstract: The OECD regularly produces estimates of tax burdens and benefit entitlements for a range of “typical household” situations. The results of these calculations (published in the Benefits and Wages and Taxing Wages series) are frequently used to compare countries’ tax-benefit systems and to assess progress towards specific policy objectives. This paper presents information on particular aspects of the structure of household populations across countries in order to help in the interpretation of results based on such “typical” family situations. A range of internationally comparable data sources are used to assess how relevant household circumstances such as family structure, labour market attachment and benefit coverage vary across countries. The results are used as a basis for clarifying the scope of tax-benefit indicators based on synthetically constructed household typologies. “Typical household” calculations cannot be used to address essential distributional issues such as how ...

Local-currency interest rates in these countries are driven by those on rand-denominated transactions. Reducing the level and volatility of the rand premium would help reduce ?nancing costs in the region. . Policies should promote: enhancing ?nancial-market liquidity; easier access to South African ?nancial markets for African entities; domestic saving capacity; and the improvement of international perception of the rand. . Johannesburg could become a ?nancial “hub” for the region, channelling cheap resources to its neighbours.

Abstract: New Institutional Economics (NIE) takes a multidisciplinary approach to understanding economic phenomena like growth, efficiency and income distribution. The authors of this book attempt to provide an integrated methodology, hitherto lacking in NIE, that analyses both the impact of institutions on economic development and the determinants that shape institutional quality. Using the case of Argentina, they use the analytical framework developed and explained in the book to test their theoretical hypotheses. They find that their concept of institutional quality and their delineation between economic and political institutions work in practice. In addition, their consideration of particular institutions – democracy and autocracy – and their reflections on the impact of “traditional” legal and cultural frameworks add a dimension to the book that gives it a controversial, but stimulating timeliness. At a time when national governments and international institutions are seeking ways to improve governance and accelerate growth, particularly in the poorer countries, this book provides valuable insights for policy makers and those who advise them. It offers convincing evidence about the quality of institutions as a determining factor in development and suggests how such quality might be improved.

Abstract: This paper analyses the Irish private health insurance (PHI) market. It describes how PHI interacts with the public system, and assesses its contribution to equity, efficiency and responsiveness of the health system. The analysis identifies some of the factors affecting insurance market performance and its impact on the health system, including market characteristics, the regulatory and fiscal environment, health system organisation, and any actors’ incentives and behaviours. PHI plays a prominent role in Ireland. The health system is designed to offer comprehensive publicly funded health services to low-income groups, and universal public hospital coverage. Policies have encouraged the development of PHI to provide all individuals with a private alternative to the public system, as well as a means of funding cost-sharing and services not covered by the public system. With the implementation of the requirements of the Third EU Non-Life Directive, the PHI market, historically ...

Abstract: Private health insurance (PHI) is the sole source of primary health coverage for a third of the Netherlands’ population earning above a set income threshold. Social insurance (together with limited public (tax-based financing) is the main source of health coverage for the majority of the population. Most socially insured also purchase supplementary private health coverage. All citizens are eligible for a system of coverage for long-term care and care for the chronically ill. Thus, in the Netherlands, the source of health financing is determined according to the category of health risk, type of illness, as well as income level. Decisions have been made allocating the cost of more expensive long-term care and coverage of high-risk individuals and persons earning below a set level, to social or public insurance, or to PHI subsidised by a broader pool. From an equity perspective, the Dutch public/private financing mix appears to do well, although challenges remain. There appear to be ...

Abstract: Raising manufacturing productivity is of central importance to the developing world and an essential element of policy making. Overcoming Barriers to Competitiveness is about establishing the most reliable analysis of manufacturing productivity possible and helping policy makers set their priorities. The paper demonstrates that productivity rests on five elements of the economy: infrastructure, capital, trade, education and aggregate efficiency. These factors, when multiplied together, give a true picture of a country’s situation on the productivity “league table”. More than a simple comparison, this ranking system allows the identification of which elements in each particular national or regional case require most attention. This approach can be viewed as another way of addressing the so-called “competitiveness problem” of poor countries. It does not say, however, that other areas can be totally neglected; one of the main points of the paper is that all five elements have to be ...

They also do not capture differences in average ETRs facing different population sub-groups. I use EUROMOD, an EUwide tax-benefit model, to derive distributions of average and marginal ETR measures for fourteen countries. Results for each country show how many and which types of individuals face different ETR levels. Abstract: Citizens in OECD countries quite unambiguously support more solidarity and justice at the international level; however, despite the adoption of the Millennium Development Goals (MDGs) by their governments in 2000, they remain insufficiently informed and educated about global development challenges. With increased understanding of the issues, citizens could provide greater support for more efficient and coherent development policies as well as for more funding. This requires new efforts by national authorities to report on desirable outcomes and on the results and effectiveness of international development policies towards achieving them.

Abstract: This paper studies the development of indigenous insurance institutions set up to help cover the high costs of funerals, using evidence from rural areas in Tanzania and Ethiopia. Many of these institutions tend to co-exist within the same community and are based on well-defined rules and regulations, often offering premium-based insurance for funeral expenses, as well as, in many cases, other forms of insurance and credit to help address hardship. The paper argues that the characteristics and inclusiveness of these institutions make them well placed as models to broaden insurance provision and other development activities in these communities. In Ethiopia, there is some encouraging experience with using these institutions, as reviewed in this paper. However, the paper argues that their fragility as institutions is well illustrated by current pressures related to HIV/AIDS, as well as by their apparent resistance to engage more broadly with NGOs and government agencies. As a ...

Abstract: This report provides an overview of the limited empirical and theoretical research on take-up of welfare benefits, i.e. the extent to which people eligible for various types of benefits actually receive them. Focus is mainly on entitlement programmes, where take-up reflects both decisions of eligible individuals to apply for benefits and the accuracy of administrative decisions as to whether these individuals should get the benefit in question or not. Estimates of the extent of take-up of welfare benefits are based on a variety of approaches, and typically combine both administrative and survey data. Despite these methodological differences, and the very few OECD countries for which estimates are available, the evidence reviewed in this paper suggests that low take-up of welfare benefits occurs both across countries and programmes. Estimates typically span a range of between 40% and 80% in the case of social assistance and housing programmes, and between 60% and 80% for ...

Abstract: Donor agencies and policy makers tend to agree that increased access of women to education, health, credit, formal legal rights and employment opportunities, in conjunction with economic growth, will substantially improve the socio-economic role of women in developing countries. This paper challenges that view. It argues that these measures might not be sufficient if the institutional framework within a country constrains women from participating in economic activities. It finds that social institutions — laws, norms, traditions and codes of conduct — constitute the most important single factor determining women’s freedom of choice in economic activities. They have not only a direct impact on the economic role of women but also an indirect one through women’s access to resources like education and health care. The findings suggest that an institutional framework that disadvantages half of the adult population hinders development. To address gender inequalities effectively, policy ...

Abstract: Mit dem Beginn des 21.

Abstract: This paper analyses the Slovak health insurance system and the policy challenges it faces. It describes the structure of health coverage and health sector reforms being implemented by the Slovak government. It provides a preliminary assessment of the possible impact of such reforms, with a focus on the health insurance system and the possible introduction of private health insurance (PHI). It assesses how private health insurance would impact upon the health system, particularly equity, efficiency incentives facing providers and insurers, and responsiveness. The Slovak health system is based upon a mandatory Bismarck-style social health insurance system. Contributions are shared between employers and employees and the state contributes for the inactive population. Five non-profit and non-competing insurers operate nationwide, one of which covers two-thirds of the population. Individuals can freely enrol with any of the insurance companies and a risk equalisation system ...

Abstract: Natural disasters (droughts, earthquakes, epidemics, floods, wind storms) damage wellbeing, both in their immediate and long-term aftermath, and because the insecurity of exposure to disasters is in itself harmful to risk-averse people. As such, mitigating and coping with the risk of natural disasters is a pressing issue for economic development. This paper provides a conceptual framework for understanding natural disasters. Disasters, which imply tragic human costs, are distinguished from hazards, which are events like earthquakes or flooding: hazards only translate into disasters when societies are vulnerable to them. Consequently international development policy can play a role in reducing the costs of disasters by addressing vulnerability. A review of two recent disasters — the Turkish earthquakes of 1999, and Hurricane Mitch in 1998 — illustrates the importance of precarious urbanisation and environmental degradation for increased vulnerability to natural hazards. These cases ...

Abstract: While France has a universal public health insurance system, the coverage it provides is incomplete and the vast majority the French population has private complementary health insurance. Among OECD countries, the share of health care financed by private insurance is third highest behind the US and the Netherlands, two countries where private coverage is the primary source of payment for a large percentage of the population. France’s high rate of private insurance coverage is partly explained by historical factors and partly by the preferential tax treatment of employer-sponsored coverage. Because of the high rate of employerprovision – roughly half of all contracts are obtained through the workplace – coverage tends to vary with activity and industry classification. Historically, coverage was also positively related with income. In 2000, the French government introduced a new program, the Couverture Maladie Universelle (CMU), which extended eligibility for publicly funded ...

Abstract: Governments often look to private health insurance (PHI) as a possible means of addressing some health system challenges. For example, they may consider enhancing its role as an alternative source of health financing and a way to increase system capacity, or promoting it as a tool to further additional health policy goals, such as enhanced individual responsibility. In some countries policy makers regard PHI as a key element of their health coverage systems While private health insurance represents, on average, only a small share of total health funding across the OECD area, it plays a significant role in health financing in some OECD countries and it covers at least 30% of the population in a third of the OECD members. It also plays a variety of roles, ranging from primary coverage for particular population groups to a supporting role for public systems. This paper assesses evidence on the effects of PHI in different national contexts and draws conclusions about its ...

With cumulative proceeds of privatisation accounting for just $8 billion compared to $46 billion in transition economies over the same period, it is clearly still in its infancy. The report charts progress, outlines methods used, and questions whether the objectives have been achieved. It also examines the impact privatisation has had on alleviating poverty. It is for policy makers, advisers and donors and research institutes involved in the privatisation process.

Abstract: China has experienced spectacular economic growth since 1978, averaging 8 to 9% per year. As a result, on average, people’s standard of living is far higher than ever before in China’s history. However, economic disparities have also widened very significantly during this period, raising questions about the appropriateness and sustainability of existing policies. This book includes the papers from a seminar held in Paris on 20-21 October 2003 to explore the causes of China's growing economic disparities. The seminar was organised by the OECD and the National Development and Reform Commission (NDRC) of China. The papers by Chinese and OECD experts look at the driving forces behind these trends and discuss possible policy responses. The collection presents many comparisons with income inequality trends in OECD countries, including geographic disparities, and looks at ways to improve Chinese data on income distribution.

Abstract: This book presents the conclusions of a meeting of the Sahel and West Africa Club in Accra, Ghana, in May 2002 on the following topic : “Towards a Better Regional Approach to Development in West Africa”. Regional cooperation is still only approached from an institutional perspective whilst field dynamics, economic actions and their transactions and demographic changes are forces that define spaces that regional policies seldom take into account. Important questions emerged from the Accra debates: How to harmonise “real” spaces with institutional integration spaces? Do certain groups of actors have a constructive vision and can they contribute to the formulation of a credible regional project? Do these “real” spaces, especially those defined by “informal” exchanges, along with those targeted by multinational companies have positive or negative effects on the regional economy? Could they prefigure future official areas for integration? The book is for development economists, African researchers, NGOs and regional integration organisations.

Abstract: This paper reports on recent work on improving the effectiveness of aid allocations, and extends the scope of analysis beyond the aid-policy-growth-poverty linkages to include three new elements: a broader range of poverty-relevant objectives and effectiveness constraints of aid; practical experience in the application of quantitative analysis to allocations; and analytic approaches to determining shares of aid through multilateral channels and for global public goods. There is wide consensus on the main allocation criteria for effectiveness in reducing poverty through pro-poor growth: the level and incidence of poverty and the development performance of partner countries. Other variables that raise the impact of aid on growth are helping vulnerable countries adjust to shocks, debt relief and post-conflict reconstruction. Considering broader development and humanitarian objectives, aid should also be allocated to prevent violent conflict and to improve governance and social conditions in “difficult partnerships”. In addition, global public goods are severely under-funded, and there is some evidence of under-funding of multilateral programmes...
