    Abstract: In the backdrop of the COVID-19 pandemic, ensuring the safety of health care services remains a serious, ongoing challenge. This once-in-a-century global health crisis exposed the vulnerability of healthcare delivery systems and the subsequent risks of patient harm. Given the scale of the occurrence and costs of preventable patient safety events, intervention and investment are still relatively modest. Good patient safety governance focuses on what leaders and policy makers can do to improve system performance and reduce the financial burden of avoidable care. Moreover, it is essential in driving progress in improving safety outcomes. This report examines how patient safety governance mechanisms in OECD countries have withstood the test of COVID-19 and provides recommendations for countries in further improving patient safety governance and strengthening health system resilience.

Die Kosten der vermeidbaren, auf medikationsbedingte Ereignisse zurückzuführenden Hospitalisierungen sowie der längeren Krankenhausaufenthalte aufgrund von vermeidbaren, im Krankenhaus auftretenden medikationsbedingten Schäden belaufen sich im OECD-Raum insgesamt auf mehr als 54 Mrd. USD. Der vorliegende Bericht umfasst vier Teile. Im ersten Teil werden die menschlichen und wirtschaftlichen Kosten medikationsbedingter sicherheitsrelevanter Ereignisse im OECD-Raum untersucht.     Abstract: Electronic Health Records (eHR) represent a significant digital transformation in the healthcare sector. A 2021 OECD survey of 27 countries revealed a growing adoption of eHRs. However, system fragmentation remains a concern: only 15 countries have a nationally unified system. Twenty-four countries have adopted a minimum data set for standardized core health information. While patient access to eHRs has notably increased since 2016, obstacles such as provider resistance, technical barriers, and legal hurdles continue to exist. The COVID-19 pandemic underscored the pivotal role of eHRs, particularly in vaccine tracking and post-market surveillance, highlighting the pressing need for international cooperation to maximize the benefits of eHRs in healthcare. Furthermore, as eHRs integrate with artificial intelligence, new governance challenges arise.

    Abstract: The OECD's Patient-Reported Indicator Surveys (PaRIS) initiative aims to measure outcomes and experiences of healthcare as part of an effort to improve the value of health system investments. The PaRIS survey, a survey of people living with chronic conditions, is currently being implemented in twenty countries. The PaRIS survey has been developed together with government officials, patients, providers, and researchers. However, the extent of stakeholder involvement varies between countries. This paper reports on the stakeholder engagement in design, development and implementation of the PaRIS survey Field Trial in seventeen countries. Engagement strategies were analysed by target group (patients, providers, or other stakeholders), and engagement level (co-designing, involving, consulting, and informing). The results provide valuable lessons for the implementation of the full PaRIS survey in 2023 and illustrate how stakeholders could be more actively engaged in health services research and policymaking.

Health data governance reforms, in particular, have been an important aspect of countries responses. Improvements in the quality, coverage, completeness, and capacity for data sharing in regard to existing national personal health datasets were widely reported. Countries have also made significant investments in digital tools, systems for public health monitoring, assessments of resource use and availability, and data to monitor the status of non-COVID related health needs.

As many as 1 in 10 hospitalizations in OECD countries may be caused by a medication-related event and as many one in five inpatients experience medication-related harms during hospitalization. Together, costs from avoidable admissions due to medication-related events and added length of stay due to preventable hospital-acquired medication-related harms total over USD 54 billion in OECD countries. This report includes four components; it 1) assess the human impact and economic costs of medication safety events in OECD countries, 2) explores opportunities to improve prescribing practices 3) examines the state-of-the art in systems and policies for improving medication safety, and 4) provides recommendations for improving medication safety at the national level.

    Abstract: Improving patient safety culture (PSC) is a significant priority for OECD countries as they work to improve healthcare quality and safety—a goal that has increased in importance as countries have faced new safety concerns connected to the COVID-19 pandemic. Findings from benchmarking work in PSC show that there is significant room for improvement. Across included survey findings from OECD countries, less than half (46% ) of surveyed health workers believe that important patient care information is transferred across hospital units and during shift changes. Just two-in-five surveyed health workers in OECD countries believe the staffing levels at their workplace are appropriate for ensuring patient safety (40%) or that mistakes and event reports would not held against them (41%). International benchmarking is a feasible and useful addition to exiting measurement initiatives on safety culture and may help accelerate necessary improvements in patient safety outcomes.

In der Covid-19-Krise wurde in besonderem Maße deutlich, wie wichtig es ist, eine sichere Gesundheitsversorgung zu gewährleisten – für die Patient*innen ebenso wie für die Gesundheitskräfte. Ein ausreichendes Angebot an qualifizierten Arbeitskräften ist Grundvoraussetzung für ein resilientes Gesundheitssystem. Daher muss die Politik nun Wege finden, ein adäquates Arbeitskräfteangebot aufzubauen und zu fördern, damit das Gesundheitssystem für künftige Schocks gewappnet ist. Dies gilt nicht nur für den Krankenhaussektor, sondern auch für Gemeindesettings, die Pflege und die Primärversorgung. Außerdem sollten geeignete Mechanismen zum Schutz der Patient*innen und Gesundheitskräfte geschaffen werden, die eine sichere Schutzausrüstung, genügend medizinisches Material und eine angemessene Personalausstattung, Weiterbildung und Unterstützung am Arbeitsplatz gewährleisten. Besonders wichtig sind solche Mechanismen, wenn politisch Verantwortliche gesundheitliche, sicherheitsbezogene und wirtschaftliche Anforderungen miteinander in Einklang bringen müssen.

    Abstract: Osteoarthritis impacts 7% of the global population, affecting more than 500 million people worldwide. As populations of OECD countries age, an increasing number of hip and knee replacement surgeries calls for further work on assessment of quality of care, particularly from patients’ point of view. Thirteen programmes from nine countries participated in the PaRIS Hip and Knee PROMs comparative reporting in 2020-21 by collecting and submitting data by generic and condition-specific PROMs. All programmes showed improvements in patient outcomes though the relative improvement varied. Crosswalks from SF-12 to EQ-5D provided valuable lessons on conversion errors. Results of this work call for improving the use of data for comparative reporting as well as further collaboration on utilising patient-reported metrics in quality-of-care improvement and policymaking.

    Abstract: Patient-reported measures are a critical tool for improving policy and practice in mental health care. However, to date, the use of patient-reported measures in mental health care is limited to a small number of countries and settings—and there is a pressing need, both within and across countries, to consistently and effectively measure the effects and impact of care for patients who use mental health care services. The PaRIS pilot data collection on mental health included 15 data sources from 12 countries, collected over the course of 2021. While the scope of included data varied, the results demonstrate increased adoption of national and subnational efforts to capture patient-reported information in mental health care systems. Analysis of data collected through the PaRIS mental health pilot documents, in general, positive patient-reported experiences of mental health care. The results also suggest improvement in patient-reported outcomes for those receiving mental health care services.

Diese Entwicklung dürfte sich in Anbetracht der zunehmenden Bevölkerungsalterung im OECD-Raum fortsetzen. Die Kosten vermeidbarer Krankenhauseinweisungen aus Langzeitpflegeeinrichtungen beliefen sich 2016 auf knapp 18 Mrd. USD. Dies entspricht 2,5 % der Ausgaben für die stationäre Krankenhausversorgung bzw. 4,4 % der Ausgaben für die Langzeitpflege. Forschungsarbeiten zeigen, dass mehr als die Hälfte der unerwünschten Ereignisse in der Pflege und 40 % der Krankenhauseinweisungen aus Pflegeeinrichtungen vermeidbar sind. Den Grundursachen dieser Ereignisse kann durch bessere Prävention, Sicherheitsmaßnahmen und Personalentwicklung – insbesondere im Hinblick auf den Kompetenzmix und die Ausbildung – begegnet werden. Durch gezielte Investitionen in Schlüsselbereichen können signifikante Effekte erzielt und die wichtigsten Kostentreiber unerwünschter Ereignisse in der Langzeitpflege reduziert werden.

    Abstract: Health care settings are inherently hazardous places, with very unpredictable and complex working environments. These hazards and risks not only result in a range of injuries and ill-health among workers but also jeopardise the safety of patients. The COVID-19 crisis has amplified the importance of ensuring that the health care that is provided is safe—for patients and health workers alike. A sufficient, and capable, workforce, is the foundation of resilient systems. Policy makers need to focus now on how to build and support an appropriate workforce to respond to future shocks. This includes health workers beyond the hospital—including those in community, long-term, and primary care. The safety of both patients and health workers should be protected through appropriate mechanisms to ensure the safety of protective equipment and sufficient supplies, appropriate staffing levels, training and support at the workplace. These governance mechanisms are even more relevant when policy makers face trade-offs between health, safety and economic concerns.

    Abstract: Long-term care (LTC) institutions are now providing care to a greater number of people, and more residents with chronic conditions and multiple co-morbidities, than ever before. Trends suggest this strain will continue to increase as OECD populations continue to age. The total cost of avoidable admissions to hospitals from LTC facilities in 2016 was almost USD 18 Billion, equivalent to 2.5% of all spending on hospital inpatient care or 4.4% of all spending on LTC. Research shows that over half of the harm that occurs in LTC is preventable, and over 40% of admissions to hospitals from LTC are avoidable. The root causes of these events can be addressed through improved prevention and safety practices and workforce development—including skill-mix and education. Targeted investments in a number of key areas can have a significant impact by mitigating the main cost drivers of adverse events in LTC.

The report advises a set of policies that can help countries to improve diagnosis, strengthen access to care services, improve the quality of care, and support the families and carers of people living with dementia. Measuring and comparing the services that are delivered to people with dementia and the outcomes they achieve is a crucial part of improving dementia care. Most health systems have very poor data on dementia care and countries should work to strengthen the measurement of quality and outcomes of dementia care.

This long-run increase in income inequality not only raises social and political concerns, but also economic ones.     Abstract: The book presents a major meta-analysis of 'value of a statistical life' (VSL) estimates derived from surveys where people around the world have been asked about their willingness to pay for small reduction in mortality risks. The analysis seeks to explain the differences in the estimates, for example across countries. Differences in incomes and the magintude of the risk reduction people have been asked to value were found to be the factors having the strongest impact on VSL, but a number of other policy-relevant factors are also important. Based on the meta-analysis, and a broad review of the literature, the book also presents clear advice on how VSL values best can be used in assessments of environmental, health and transport policies, such as in cost-benefit analyses. Using explicit VSL estimates to quantify the benefits to society of fatality risk reductions can play an important role in the development of more cost-effective public policies.

    Abstract: The costs of mental ill-health for the individuals concerned, employers and society at large are enormous. Mental illness is responsible for a very significant loss of potential labour supply, high rates of unemployment, and a high incidence of sickness absence and reduced productivity at work. In particular, mental illness causes too many young people to leave the labour market, or never really enter it, through early moves onto disability benefit. Today, between one-third and one-half of all new disability benefit claims are for reasons of mental ill-health, and among young adults that proportion goes up to over 70%. Indeed, mental ill-health is becoming a key issue for the well-functioning of OECD’s labour markets and social policies and requires a stronger focus on policies addressing mental health and work issues. Despite the very high costs to the individuals and the economy, there is only little awareness about the connection between mental health and work, and the drivers behind the labour market outcomes and the level of inactivity of people with mental ill-health. Understanding these drivers is critical for the development of more effective policies. This report aims to identify the knowledge gaps and begin to narrow them by reviewing evidence on the main challenges and barriers to better integrating people with mental illness in the world of work.

Venezia, con i suoi elevati livelli occupazionali e di crescita, si situa tra le aree urbane più dinamiche e produttive all’interno dell’ocse. Sviluppatasi sul modello delle piccole imprese e dei distretti industriali, sta ora affrontando una profonda trasformazione economica. Lo storico nucleo urbano di Venezia, erede di secoli di sviluppo economico costruito sui mari, vede ogni giorno accentuarsi l’esplosione nella terra ferma circostante di una struttura spaziale assai articolata capace di integrare in una unica area urbana altre antiche presenze come Padova e Treviso e la consistente rete di città minori. Il modello della city-region assunto dall’ocse per Venezia (uno spazio urbano entro cui si muove quotidianamente la gran parte dei lavoratori e degli studenti per la maggior parte dei giorni dell’anno) deve confrontarsi con sfide ambientali sempre più impegnative, conseguenza dell’aumento del traffico e dei crescenti costi infrastrutturali esacerbati dai fenomeni di dispersione urbana. Anche la sua struttura demografica sta mutando in seguito all’invecchiamento della popolazione, alla presenza di immigrati e al progressivo spopolamento del centro storico della città. Questo rapporto offre un’analisi comparativa di questi temi e, utilizzando il database metropolitano ocse, si concentra nello specifico su produttività e crescita; considerando l’economia regionale, la pianificazione urbana, gli studi sul sistema dei trasporti e i problemi idrologici evidenzia i cambiamenti all’interno della città-regione. Alla luce del programmato sviluppo delle connessioni ferroviarie tra le città, questo studio richiama l’esigenza di programmi atti ad aumentare le sinergie economiche tra Venezia e le città vicine; prende in esame strumenti chiave per promuovere la crescita economica e la governance metropolitana, proponendo inoltre un migliore coordinamento delle politiche riguardanti l’uso del territorio, dei servizi aggiuntivi per lo sviluppo delle piccole e medie imprese e del potenziamento dell’innovazione legata alle università. Il rapporto valuta la qualità della governance delle acque e la potenzialità che Venezia ha di diventare un punto di riferimento essenziale in relazione alle strategie di adattamento al cambiamento climatico. The role of total factor productivity and innovation in driving growth was significant in the 1990s and declined in the early part of this decade, but appears to be on the rise again thanks in part to significant increases in public and private investment in innovation. The Basque Country has begun a transition from a model of incremental innovation in manufacturing to a model increasingly based on science and other forms of knowledge. Through a diagnostic of the innovation system and the policy mix, the review offers some policy and governance recommendations to achieve the region’s desired transition in light of global trends in the innovation process and innovation policy.

    Abstract: On the 50th anniversary of the OECD, we examine the unique work the organisation performs in regulating and rationalising governments’ use of export credits in support of exports, jobs, economic growth and national interests more broadly. This work is part of a global post war effort to emphasise multilateral co operation and sound economic policies to promote co operation, efficiency and prosperity rather than destructive competition, controversy and conflict. OECD export credits work is one of the basic building blocks of the ever growing structure of global trade agreements that aim to maintain open and efficient markets. The objective is to eliminate subsidies and unfair practices in the economic competition that forms the foundation of a healthy and dynamic global economy. The elimination of official financing subsidies in global trade is only a part of the broader trade policy agenda, but it is a vital part, and has been delegated to the OECD by the WTO. Since financing is the life blood of trade flows, specialised OECD housed work allows trade to flow efficiently for aircraft and other capital goods while other trade policy work and litigation continue at the WTO. The export credits work at the OECD is described in this collection of essays. However it is about much more than the series of agreements described herein. It is more fundamentally about the governments and their people - policy makers and experts - who gather at the OECD to build collectively a system of export credits disciplines that is fair, transparent, adaptable and effective. It is therefore as much about people and ideas as anything else. The export credit secretariat pictured above represents only the latest in a long line of OECD staff committed to facilitate and advise this work. The OECD’s motto on its 50th anniversary is “Better Policies for Better Lives.” This reminds us that in the end, it is policies that are at the centre of human well being. And export credits work is about promoting these better policies by developing “smart rules” that open markets and maintain a level playing field and by bringing people and governments together to this end.

    Abstract: The North Atlantic (NORA) region is a transnational area comprising the Faroe Islands, Greenland, Iceland, and the coastal counties of Norway. These territories are linked by shared characteristics and challenges, as well as by historical, institutional and cultural links. Improving accessibility to the region, ensuring sustainable development of its fisheries, enlarging and diversifying its economic base, and meeting the challenges of climate change are key issues. Strengthened regional co-operation can help these territories address them by exchanging know-how and best practices, pooling resources and reaching economies of scale, improving the efficiency of public sector provision, and increasing the “voice” of the region. However, transnational co-operation in the NORA region faces some barriers, as it involves territories that compete in their main economic activities, are separated by large distances, and have strong institutional and economic links with other countries and regions. In order to get the most from transnational co-operation, this report recommends that the NORA territories: focus co-operation efforts on targeted themes and issues; draw up a regional development strategy; promote greater awareness of the benefits of co-operation; develop a “variable geometry” approach to regional co-operation; and enlarge and refine the role of the NORA institution as a facilitator of co-operation.

Zur Verbesserung der regionalen Wirtschaftsleistung wurde 2008, im Anschluss an den OECD-Prüfbericht 2002, die neue Schweizer Regionalpolitik (NRP) eingeführt. Die NRP stellt einen klaren Paradigmenwechsel dar, da sie sich nicht mehr auf den Bau von Infrastrukturen und die Gewährung von Finanzhilfen konzentriert, sondern die Wirtschaftsförderung zur gezielten Mehrwertschöpfung in den Regionen in den Mittelpunkt stellt. Dieser Bericht enthält Empfehlungen, wie das Ergebnis der NRP durch die Ausweitung des Wirkungsbereichs auf das gesamte Land, engere interkantonale Zusammenarbeit und besser koordinierte Sektoralpolitiken optimiert werden kann. Ferner befasst der Bericht sich mit der regionalen Innovationsförderung. In diesem Rahmen wird empfohlen, die Aufgaben der einzelnen Staatsebenen klarer zu definieren, Forschung und Technologietransfer auf Landesebene vom Bund finanzieren zu lassen und die Kantone mit der Innovationsförderung in den Funktionalräumen zu beauftragen. Der OECD-Prüfbericht Raumentwicklung Schweiz 2011 ist in das breiter gefasste Raumentwicklungsprogramm des OECD Territorial Development Policy Committee eingebettet.     Abstract: Rural England plays a significant role in the economy of the United Kingdom, but an even larger social and cultural role. And it is unique among OECD regions, in that it is geographically compact, with rural inhabitants generally no more than a half hour’s drive from an urban area. There is thus a vast amount of interaction between rural and urban populations in England. England’s rural population is, on average, doing better than the urban population across a broad range of socio-economic indicators. Nevertheless, rural England is also struggling with pockets of poverty and social exclusion, difficulties in maintaining access to high quality public services, an ageing population, and, most importantly, a widespread shortage of affordable housing. The government has adopted mainstreaming as its rural policy strategy. The objective of mainstreaming is to ensure that people in rural England have access to the same policies and programmes as those available in urban England. While mainstreaming is an attractive policy approach, especially in a country with strong rural-urban interactions such as England, it has proved challenging to implement for different reasons. This report examines the mainstreaming policy response as applied to rural England and suggests ways to increase its effectiveness. The report will interest academics and policy makers alike as it includes a discussion on governance structures and decentralisation; delivering public services; economic development; and the importance of improving connectivity in the context of rural areas. While the focus is on rural England, other OECD member countries will also benefit from the insights provided.

It acknowledges that policies do not operate in a vacuum and that they must take account of the contexts that influence the development and diffusion of eco-innovation, such as market structures. It explores links between eco-innovation policies and related fields such as industry, competition, and international cooperation. This work builds on an OECD inventory of eco-innovation policies in OECD countries and in China. It also draws on studies of select environment-friendly innovations, highlighting different patterns of development across countries. It also incorporates extensive international consultation on the topics of eco-innovation and green growth. The results from this publication will contribute to the Green Growth Strategy being developed by the OECD as a practical policy package for governments to harness the potential of greener growth. It offers new insights into the factors that influence the formation of adult skills in various settings – at home and at work – for the eleven countries participating in the first and last round of data collection between 2003 and 2008. The study offers comparative evidence on the impact of various factors on the supply of skill. The study offers a special focus on numeracy skills and problem solving skills. It explores the relationships between numeracy and key socio-demographic factors as well as labour market outcomes and earnings. It highlights the importance of problem solving skills by defining this foundational skill and by exploring its determinants as well as its relative role in influencing important labour market outcomes. The report offers also an analysis of performance across multiple skill domains. It investigates the skill profiles of various population groups defined in terms of the demographic and socioeconomic characteristics of those who score at levels deemed to be low in one or more skill domains and explores the resulting consequences. The report concludes by investigating the issue of skill mismatch in the labour market and its relationship to adult learning. The extent and distribution of mismatch between the day-to-day literacy related requirements of workers and the literacy skills they have obtained is an important issue that is being explored in this study.

    Abstract: Attractiveness for investment in innovation is high on the policy agenda in many countries as innovation is a key factor of growth and competitiveness in OECD countries. Virtually all governments are keen to attract international investment by multinational enterprises (MNEs) as a means to promote growth and employment, create new jobs and bring in new technologies. While all countries and regions have some policy measures in place that are aimed at increasing their attractiveness for innovation, it is less clear if these policies are effective. This report analyses the current trends in international investment in innovation and the attractiveness policies already implemented. These are often based on the more traditional instruments for attracting international investment. The book also explores in more detail the role of investment incentives that governments tend to give to international investors: their rationale, their impact and their usefulness. The evidence presented in this report raises clear policy issues and questions existing policies. A number of policy principles are formulated to guide policy makers.

    Abstract: Today’s global policy climate underlines the importance of better addressing non-economic dimensions of well-being and social progress such as health, social engagement, political interest and crime. Education plays an important role in shaping indicators of progress. However, we understand little about the causal effects, the causal pathways, the role of contexts and the relative impacts that different educational interventions have on social outcomes. This report addresses challenges in assessing the social outcomes of learning by providing a synthesis of the existing evidence, original data analyses and policy discussions. The report finds that education has the potential to promote health as well as civic and social engagement. Education may reduce inequalities by fostering cognitive, social and emotional skills and promoting healthy lifestyles, participatory practices and norms. These efforts are most likely to be successful when family and community environments are aligned with the efforts made in educational institutions. This calls for ensuring policy coherence across sectors and stages of education.

    Abstract: Migrants contribute to the economic growth of their host countries in many ways, bringing new skills and competencies with them and helping to reduce labour shortages. An aspect that has received only limited attention up to now is migrants’ contribution to entrepreneurial activity and employment creation in their host countries. In OECD countries, entrepreneurship is slightly higher among immigrants than natives and the total number of persons employed in migrant businesses is substantial, although the survival rate of these businesses is often lower than that of their native counterparts. Migrant entrepreneurship has gone beyond traditional ethnic businesses, into a wide range of sectors and innovative areas. Greater knowledge of migrant entrepreneurship is essential if policy makers are to better support migrant enterprises and their role in economic growth and job creation. In addition, increasing awareness of the positive role that migrants can play as entrepreneurs could contribute to a more balanced public debate on immigration. Taking a cross-country perspective, this publication sheds light on these issues and more, discussing policy options to foster the development and success of migrant businesses. It is a compilation of papers presented at a June 2010 conference organised by the OECD Secretariat, with the financial support of the Swedish and Turkish authorities, and the Dutch-Turkish Businessmen Association (HOTIAD).

There is a pressing need for policy makers to address the recent “medicalisation” of labour market problems, a phenomenon that appears to underlie much of the difficulties countries find in disability policy making. This report is an assessment of the Canadian situation, albeit through the lens of the federal government and the provinces of Québec, British Columbia and Manitoba. It looks at the current state of play following a decade of various reforms and preceding a period where further revisions are likely.

    Abstract: OECD's review of rural policy in Quebec. It finds that in rural Québec, both population and personal income are growing, on average, and the province’s economic base continues to diversify. Land occupancy is more homogenous than in the rest of Canada, due to the presence of denser networks of small and medium-sized communities. However, mirroring the situation at the national level, the province displays large regional disparities. The sustainability of some rural communities, especially if remote and resource-based, is challenged by demographic and economic decline. In this context, Québec has developed one of the most advanced rural policy approaches in the OECD, closely in line with the framework suggested in the OECD’s New Rural Paradigm. The province’s rural policy does not have a sectoral focus, and aims at community empowerment and land occupancy. To maximize returns on its rural policy investments, Québec needs to integrate social development more strongly with economic and entrepreneurial development, and further strengthen the supra-local level of government as the centre for rural and territorial development strategies. This should be combined with stabilisation measures in lagging areas, through the accumulation of human capital and enhanced access to land in predominantly rural territories. To address environmental challenges, natural resources should be protected both in the outskirts of metropolitan zones and in remote areas.

Regional Policies in OECD Countries responds to this need. It is the first systematic, comparative analysis of OECD countries’ regional policies. The report addresses fundamental regional policy concerns, such as: problem recognition; the objectives of regional policy; the legal/institutional framework; the urban/rural framework; budget structures; and the governance mechanisms linking national and sub-national governments as well as sectors. It begins with an overview of the regional policy today. This is followed by country profiles covering the 31 OECD members. The profiles share a common conceptual framework, allowing countries to see how their experiences measure up. The report also contains several annexes, which cover some of the countries that are candidates for accession to the OECD or with which the OECD has enhanced engagement. The annexes also cover the key topics of cross-border cooperation and trends in urban-rural linkages, especially efforts to control urban sprawl. Are surgeons in some countries more likely to leave “foreign bodies” behind after operations or make accidental punctures or lacerations rates when performing surgery? The need for answers to these kinds of questions and the value of measuring the quality of health care are among the issues addressed in this publication. Many health policies depend on our ability to measure the quality of care accurately. Governments want to increase “patient-centeredness”, improve co-ordination of care, and pay providers of high-quality care more than those who underperform. However, measuring the quality of health care is challenging. The OECD’s Health Care Quality Indicator project has overcome some of the problems, though many remain. If policy makers are serious about improving the body of evidence on the quality of care, they need to improve their health information systems. This publication describes what international comparable quality measures are currently available and how to link these measures to quality policies such as accreditation, practice guidelines, pay-for-performance, national safety programmes and quality reporting.

    Abstract: This review of regional innovation assesses how to improve Catalonia's current strategy and actions in order to boost its innovation system through both its own programmes and those of Spain and the European Union. It finds that with over seven million inhabitants and a GDP of around EUR 204 billion, Catalonia is not only an important region within Spain, but within the OECD as well. Indeed, its economic output is similar to countries like Portugal and Norway. The region experienced massive population growth over the past decade, due to immigration, which in part drove GDP growth. However, Catalonia’s productivity is slipping, relative to other OECD regions, necessitating the transition to a productivity-driven growth model through a stronger regional innovation system. The region has successfully strengthened its research base, with investments in R&D having increased four-fold over the past decade. Catalonia is now mobilising actors across the innovation system in regional centres, such as Barcelona, to improve productivity and address social challenges.

The Core Principles cover seven key areas: i) Conditions for effective regulation and supervision; ; ii) Establishment of pension plans, pension funds, and pension fund managing companies; iii) Pension plan liabilities, funding rules, winding up, and insurance; iv) Asset Management; v) Rights of members and beneficiaries and adequacy of benefits; vi) Governance; and vii) Supervision. Each of the seven core principles consists of a main text and a set of implementing guidelines. In the case of the core principle of supervision, the supporting guidelines integrate the IOPS Principles of Private Pension Supervision. The Methodology provides a structured approach for evaluating a jurisdiction’s occupational pension regulatory system in relation to the OECD Core Principles of Occupational Pension Regulation.

    Abstract: The report analyses the retirement income systems of 18 Asian countries, including Australia, China, India, Indonesia, Pakistan, the Philippines and Vietnam. It says that reform is needed because: coverage of formal pension systems is relatively low; withdrawal of savings before retirement is very common; pension savings are often taken as lump sums and often do not provide people with adequate income over their lifetime; pensions payments are not automatically adjusted to reflect changes in the cost of living. In OECD countries, an average of 70% of the working-age population are eligible for a pension. However in South Asia, just 7.5% of the working-age population are eligible and in East Asia 18%. Furthermore, few countries in Asia/Pacific have social pensions to provide safety-net retirement incomes for people who are not members of formal schemes. Only in India are social pensions significant, with around 10 to 15% of older people covered.

    Abstract: Few other countries feature the combination of very low population densities and difficult topography that hinders communication, in addition to a variety of contrasting climates. But the “Nordic welfare model” strives, with a good degree of success, to offer equal living conditions to all citizens by providing proper access to quality public services across the country. This comes, however, at great cost. This publication asks whether such a model can be sustainable in the long run, when population ageing and the reduction of petroleum reserves will reduce the leeway that the rapidly growing economy offers. It examines whether competitiveness and innovation could be further developed, given the high share of resource-based and traditional activities and whether urban policy could be better integrated into regional policy so as to better harness the energy of regional growth engines in different areas of the country, including the northern most parts. Lastly, it looks at whether impending regional reform could facilitate the necessary adaptations by transferring more power to regional councils.

Yucatán es también una tierra de contrastes. Es una región rezagada pero en crecimiento, que ofrece una alta calidad de vida y vastos recursos naturales, pero que a la vez padece problemas de sustentabilidad. Sus atracciones turísticas se localizan en zonas rurales que no se benefician de ellas. Cuenta con servicios médicos estatales y peninsulares, pero su cobertura de servicios de salud es desequilibrada. Yucatán es un centro de educación superior en la península y, sin embargo, sus graduados no consiguen empleo. Tienen varias comunidades marginadas en organismos administrativos fragmentados y, si bien los estados que pertenecen a la península comparten un legado y un atractivo culturales comunes, sus instituciones no cooperan entre sí.Resulta claro que Yucatán no aprovecha por completo sus múltiples recursos y, de hecho, los desafíos que prevalecen en la región amenazan con debilitar los bienes locales. Entre estos desafíos se encuentra la necesidad de convertir determinadas actividades en procesos de innovación y diseño de un valor agregado más alto, así como fomentar el que las actividades primarias lleguen a los mercados internacionales. Pese a que el gobierno estatal ha propuesto programas para estimular la formalización de la economía informal, medidas como una mejor reglamentación, la reducción de la burocracia y la oferta de oportunidades de empleo y el acceso al crédito formal podrían causar un mayor impacto. La falta de una visión a largo plazo compartida y coherente es un reto fundamental para mejorar la competitividad regional y la cohesión social en Yucatán. Las recomendaciones de la OCDE sólo pueden ser parte de una estrategia de mayor envergadura para desarrollar una visión colectiva del futuro del estado.

    Abstract: Istanbul has established itself as the industrial, financial and logistics centre of the country, producing almost one-third of the national output and absorbing the bulk of foreign direct investment. And, on the international scale, Istanbul ranks among the fastest growing OECD metro-regions. This review finds, however, that Istanbul faces challenges that could hamper its ambition to become a Eurasian hub for finance, logistics, culture and tourism, as well as its development in general. Its economy is changing from one driven by labour-intensive activities to one based on knowledge industries, while traditional and labour-intensive sectors (e.g. textiles and its supply chain) are shifting only gradually and slowly to other complementary industry segments. Constraints on human capital development and the informal sector have hindered productivity levels and increased income disparities. Over-migration is putting a burden on Istanbul’s transport, public infrastructure and housing, and earthquake risk management. The scale and variety of these challenges necessitates improving local public management and implementing a national strategy to reduce regional disparities and to limit migration flows towards the megalopolis.

    Abstract: Sustainable development depends on maintaining long-term economic, social, and environmental capital. In failing to make the best use of their female populations, most countries are underinvesting in the human capital needed to assure sustainabilitly. This market and systems failure is discussed in this publication in terms of gender constraints, which are based on the socially-constructed and historically developed roles of men and women. It also illuminates how female contributions can be better realized at present and how strategies can be developed for meeting the needs of future generations. This report is a contribution by the OECD to the UN Commission on Sustainable Development and its cross-cutting work on gender.

    Abstract: This Framework provides policy makers with a practical and accessible guide to best practice evaluation methods for SME and entrepreneurship policies and programmes. It uses case studies from a wide range of OECD countries to examine the benefits of evaluation and how best to address common issues that arise when commissioning and undertaking SME and entrepreneurship evaluations. Key evaluation principles are set out, including the ‘Six Steps to Heaven’ approach. These principles are illustrated with examples of evaluations of national, regional and local programmes that can be explored further by the reader. The publication focuses not only on the evaluation of individual policies and programmes, but also on peer review evaluations and assessment of the impact of SMEs and entrepreneurship of mainstream programmes that do not have business development as their principal aim. This book complements other existing guidelines, as it focusses particularly on problem situations and exceptions to the general rules.

    Abstract: To catch up with more advanced economies, Serbia urgently needs to improve the functioning of its labour market. This report reviews labour market trends and the principle challenges to labour market policy, making a series of recommendations. Despite many reforms, new business growth until now has been far too slow to compensate for job losses elsewhere. Recent reforms of labour law should be followed up by further efforts to improve the climate for business and productive work. Labour regulations must be flexible, but they should also be enforced more consistently. For all this to happen, it is essential that an effective social dialogue can take place and that it encompasses expanding and declining segments of the labour market.
