    Abstract: The COVID-19 pandemic has highlighted that access to timely health spending data is crucial for informed policy-making. This Health Working Paper summarises and compares the methodologies applied in around half of OECD countries to estimate public and private health spending for the most recent year (i.e., t-1) as well as the approaches taken by the OECD Secretariat to fill existing data gaps for the remaining OECD countries. For the first time, the paper also explores the feasibility of nowcasting health spending for the current year (i.e., t) and examines data sources that could be potentially useful in such an exercise. While this review should help OECD countries that do not yet have experience in estimating health spending for year t-1 to improve the timeliness in their data reporting, the paper also analyses the applicability of the methods in low- and middle-income countries.

    Abstract: Assessing health system performance over time or across countries often means comparing populations with very different characteristics, including age structure. The share of the population aged 65 years and over ranges from less than 1 in 10 in some of the Latin American countries of the OECD to almost 3 in 10 in Japan. At the same time, populations are aging rapidly - on average across the OECD, there are 20% more people over 65 since 2015. Since risk of illness and ill-health generally increases with age, a population with an older demographic structure can expect higher mortality rates, greater incidence and prevalence of certain diseases, and thus higher demands for healthcare and, by consequence, higher spending on health. This working paper argues that the level of health spending depends not only on the size of the population (among other factors), but also on the demographic structure of the population. The paper reviews the international literature on age-adjusting health spending, and examines three methods of age-adjustment to report and compare health expenditure data between OECD countries and over time.

    Abstract: The pandemic resulted in a significant increase in the number of deaths in many OECD countries. With detailed data now available by age and sex, this OECD Health Working Paper examines the trends and differences in mortality patterns over the three-year span of the pandemic. While a simple comparison of the raw number of deaths with reference to a historical base period has proved to be an important and straightforward indicator to assess the overall impact of the pandemic, most OECD countries have undergone major changes in population size and structure. This paper reviews the methodology of calculating changes in mortality to take account of such demographic trends and, in producing a revised set of estimates using adjusted numbers of deaths, highlights some important variations in mortality across years, countries and age groups.

    Abstract: As a key component of health care, a full understanding of how much is spent on prescription medicines is increasingly important. Only a partial understanding of total expenditures across health systems is currently possible, as reporting is often limited to medicines dispensed in community pharmacies. However, spending on pharmaceuticals used elsewhere in the health sector, particularly in hospitals, constitutes a significant and growing proportion of the overall resources allocated to medicines. This report aims to improve the coverage and quality of data on total pharmaceutical spending across the whole health sector, by reviewing current practices, and recommending a set of definitions, concepts and guidance under the framework of A System of Health Accounts 2011. Countries are encouraged to apply these guidelines in their future reporting of pharmaceutical expenditures, as part of their annual health accounts data production.

    Abstract: COVID-19 is the most significant public health emergency in more than a century, causing a global economic crisis, and with long-term repercussions across society. COVID-19 continues to claim lives, many are suffering ill health (physical and/or mental) due to the virus, and health systems struggle to recover from the massive disruption. This unprecedented crisis has highlighted the urgent need for smart investments to strengthen health system resilience – to protect people’s underlying health, through enhanced preventive care and the ability to reinforce defences in acute times, to fortify the foundations of health systems by ensuring adequate core equipment and exploiting the potential of health information, and to bolster health professionals working on the frontline by building and maintaining sufficient numbers of doctors and nurses – thereby providing countries with the agility to respond not only to evolving pandemics but also to other health and societal shocks. This report identifies a set of priority investment areas needed to strengthen health system resilience. It then produces order-of-magnitude estimates of the expected costs of such investments, drawing extensively from existing OECD data and analytical studies.

    Abstract: Assessing the direct and indirect health impact of the COVID 19 pandemic is central in managing public health and other policy measures while learning to co-exist with the virus. Many countries are publishing statistics on COVID 19 related mortality. While the frequent and timely publication of such figures provides insights into the ongoing trends in a given country, differences in coding and reporting practices pose challenges for international comparisons. Looking at the number of total deaths can help to overcome some of these differences in national practices whilst also providing a better view of the overall impact of COVID 19, by taking into account not just the possible underreporting of COVID 19 deaths but also indirect mortality caused, for example, by health systems not being able to cope with other conditions – acute and chronic.

    Abstract: OECD countries face the multiple challenges of rapidly ageing societies with the associated rise in chronic diseases and the ever-present threat from new or evolving communicable diseases. This is within the context of seeking better value for money from the health sector. While a growing body of evidence shows that many health promotion and disease prevention measures can improve health outcomes at relatively low cost, less has been documented – in an internationally comparable way – on how much countries actually invest in such activities and the drivers of prevention spending over the years. This is particularly pertinent in the context of fiscal sustainability and tight public budgets. Using newly available data from across OECD countries, this study examines the differences in spending on prevention both at an aggregate and detailed level. This analysis brings a fresh perspective and raises questions as to the optimal resource allocations within the sector. Time series data is also scrutinised in conjunction with collated policy and public health developments from a number of countries to try to identify some of the drivers behind the observed prevention spending trends. In doing so, directions for further improvement in the underlying data as well as policy implications are discussed.

    Abstract: Across the OECD, healthcare spending has typically outpaced economic growth in recent decades. While such spending has improved health outcomes, there are concerns about the financial sustainability of this upward trend, particularly as healthcare systems are predominantly funded from public resources in most OECD countries. To better explore this financial sustainability challenge, many countries and international institutions have developed forecasting models to project growth in future healthcare expenditure. Despite methodological differences between forecasting approaches, a common set of healthcare spending drivers can be identified. Demographic factors, rising incomes, technological progress, productivity in the healthcare sector compared to the general economy (Baumol’s cost disease) and associated healthcare policies have all been shown to be key determinants of healthcare spending.

    Abstract: Norway is one of the top spenders on health care among OECD countries in per capita terms but much closer to the average when seen as a share of GDP. The question is to what extent these two key measures are compatible, and how Norway really measures up to other relevant high-income countries in health spending. In considering the latter, Norway allocates more to long-term care services than any other country. So how comparable are countries in the measurement of sectors such as long-term care and does this play a key role in determining overall spending estimates? Delving further, how does spending on the key sector of somatic specialist health care compared to other countries? If too much is spent, there is a risk that there is an over-emphasis on hospitals compared to primary care. On the other hand if there are too little resources in hospitals, there may be an over-expectation from the sector. However, estimates of spending based on inpatient care still mask a number of organisational and accounting differences, requiring adjustments to be made to the underlying figures. The resulting figures provide a new insight into cross-country comparisons and trends of somatic hospital spending. Finally, to determine what is explaining the different levels of spending, the appropriate use of international spatial deflators is discussed. Recent advances in the methodology to compile comparative price information for the health and hospital sectors are used to reveal to what extent spending across the comparator countries is the result of price or volume effects.

    Abstract: Across OECD countries, pharmaceutical spending reached around USD 800 billion in 2013, accounting for about 20% of total health spending on average when pharmaceutical consumption in hospital is added to the purchase of pharmaceutical drugs in the retail sector. This paper looks at recent trends in pharmaceutical spending across OECD countries. It examines the drivers of recent spending trends, highlighting differences across therapeutic classes. While the consumption of medicines continues to increase and to push pharmaceutical spending up, cost-containment policies and patent expiries of a number of top-selling products have exerted downward pressure on pharmaceutical expenditures in recent years. This resulted in a slower pace of growth over the past decade. The paper then looks at emerging challenges for policy makers in the management of pharmaceutical spending. The proliferation of high-cost specialty medicines will be a major driver of health spending growth in the coming years. While some of these medicines bring great benefits to patients, others provide only marginal improvements. This challenges the efficiency of pharmaceutical spending.

    Abstract: The gap between rich and poor keeps widening. Growth, if any, has disproportionally benefited higher income groups while lower income households have been left behind. This long-run increase in income inequality not only raises social and political concerns, but also economic ones. It tends to drag down GDP growth, due to the rising distance of the lower 40% from the rest of society. Lower income people have been prevented from realising their human capital potential, which is bad for the economy as a whole. This book highlights the key areas where inequalities are created and where new policies are required, including: the consequences of current consolidation policies; structural labour market changes with rising non-standard work and job polarization; persisting gender gaps; the challenge of high wealth concentration, and the role for redistribution policies.

    Abstract: In recent years, China has seen an unprecedented expansion of health insurance for its population in its quest to achieve universal health coverage. By 2011, 95% of the Chinese population was insured up from less than 50% in 2005 through public or employer-based insurance schemes. As part of this move, the structure of health care financing has shifted significantly, such that public sources in 2013 funded well over half of all health spending, compared with just over a third in the early 2000s. In that context, it is important to determine the main drivers of future growth in health spending in the medium term, to assess the possible impact on public budgets. Using a component-based health expenditure model developed at the OECD, future projections of public spending on health care and long-term care are made for OECD and key emerging economies, including China. The uniform cross-country framework allows for consistent international comparisons under different cost-pressure and cost-containment scenarios.

Se terminó a principios de noviembre de 2011 y refleja los avances hasta ese momento.

    Abstract: Almost four years since the onset of the global financial and economic crisis, unemployment and underemployment remain stubbornly high in many G20 countries, and many workers remain trapped in low paid, often informal, jobs with little social protection. Job creation has been anemic in many countries, too slow to fully reabsorb the mass of unemployed and underemployed or, particularly in some emerging market economies, to keep pace with labour force growth and the pressures of rural-urban migration. This raises concerns about the long-term negative effects on human capital, growing inequality and lower future output growth. The political pressures are high, and the risk of a drift towards protectionist measures aimed at 'keeping jobs at home' cannot be ignored. While there is substantial variation in national contexts, G20 countries can help minimize these risks through collective and collaborative work aimed at identifying and implementing credible policy reforms that will boost job creation, employment and the quality of jobs. The report aims at providing a preliminary review of countries' experiences against the backdrop of an evolving economic outlook and could form the basis of a more in-depth analysis, should Ministers request it. Improving labour market outcomes involves several challenges relating to both the quantity and quality aspects of job creation. There is a need in all countries to harness growth to generate labour market opportunities that correspond to labour force growth

    Abstract: In 2012, the Mexican Presidency of the G20 introduced inclusive green growth as a cross-cutting priority on the G20 development agenda. The second meeting of the G20 Development Working Group (DWG), hosted by the Government of the Republic of Korea, took place in Seoul the 19th and 20th of March 2012. As agreed during the first DWG meeting, this second meeting focused on the priorities for their presidency in the first half of 2012: infrastructure, food security and inclusive green growth (IGG). At its Seoul meeting, the DWG also agreed that IGG co-facilitators and relevant international organizations (IOs) should work together in 2012 to develop a nonprescriptive good practices guide/toolkit on enabling national policy frameworks for inclusive green growth to support countries who voluntarily wish to design and implement affordable and inclusive green growth policies, with the aim of achieving sustainable development and poverty alleviation. The toolkit is organized as follows. First, the necessity of applying the different tools in the context of a broad inclusive green growth strategy is stressed, and a harmonized framework combining approaches and tools identified by all four IOs is set forth. Second, the document offers an overview of key tools that can be mobilized to implement an inclusive green growth strategy. Quick technical descriptions of these tools are offered along with suggested sources for further details. Finally, capacity building and knowledge sharing initiatives are presented, with the Green Growth Knowledge Platform (GGKP) highlighted as a powerful collaborative tool to advance policies for inclusive green economies

    Abstract: This report examines how green growth and sustainable development policies can be incorporated into structural reform agendas. Indeed, as demonstrated in the report, many of these policies are closely linked and synergistic with the framework policies applied by G20 governments in their efforts to pursue strong and sustainable growth. The report, has been prepared in response to the request from G20 Finance Ministers and Central Bank Governors in their communication of 25-26 February 2012 that asked the Organization for Economic Co-operation and Development (OECD), with the World Bank and the United Nation (UN), to prepare a report that provides options for G20 countries on inserting green growth and sustainable development policies into structural reform agendas, tailored to specific country conditions and level of development. The report will be an input to the G-20 leader's summit in Los Cabos and provides a toolkit of policy options from which countries may draw-upon when designing their own green growth strategies. The G20 development working group has also tasked the International Organizations with the development of a non-prescriptive toolkit of policy options to support inclusive green growth in the context of sustainable development and poverty eradication in developing countries

    Abstract: A basic level of literacy and numeracy is essential for full participation in modern societies. While very few people in most of the advanced countries could be regarded as illiterate or innumerate, recent studies have shown that there are, in fact, significant numbers of people with poor skills, and that low levels of skills are associated with lower wages and greater chances of unemployment and disengagement from the labour market. At the same time, technological changes, particularly the increasing presence of information and communication technologies in all areas of life, have led to a growing demand for higher-level cognitive skills that involve understanding, interpreting, analysing and communicating complex information. In this context, policy makers need the most comprehensive and up-to-date information if they want to ensure that the supply of skills in the labour force matches labour-market demand. The Programme for the International Assessment of Adult Competencies (PIAAC), an international assessment of adult skills managed by the OECD, will be central in providing that information. Implemented by 25 countries in Europe, the Americas and Asia, PIAAC is compiling one of the richest sources of data regarding the skills that adults use in their work, home and communities, and related information, including family background, education, employment, income, civic participation and health. Results from the data, which are being collected from August 2011 to March 2012, will be available at the end of 2013. This report maps the development of the Programme, from determining what should be measured, to defining the meaning of PIAAC’s three core domains, “literacy”, “numeracy” and “problem solving in technology-rich environments”, to designing assessment tasks and determining how those tasks will be interpreted. It summarises the draft frameworks, developed by dedicated experts, for each of the assessment domains, and includes examples of the items and stimuli used to measure proficiency in the three domains. In essence, it provides an overview and a look at the underpinnings of PIAAC.

    Abstract: The costs of mental ill-health for the individuals concerned, employers and society at large are enormous. Mental illness is responsible for a very significant loss of potential labour supply, high rates of unemployment, and a high incidence of sickness absence and reduced productivity at work. In particular, mental illness causes too many young people to leave the labour market, or never really enter it, through early moves onto disability benefit. Today, between one-third and one-half of all new disability benefit claims are for reasons of mental ill-health, and among young adults that proportion goes up to over 70%. Indeed, mental ill-health is becoming a key issue for the well-functioning of OECD’s labour markets and social policies and requires a stronger focus on policies addressing mental health and work issues. Despite the very high costs to the individuals and the economy, there is only little awareness about the connection between mental health and work, and the drivers behind the labour market outcomes and the level of inactivity of people with mental ill-health. Understanding these drivers is critical for the development of more effective policies. This report aims to identify the knowledge gaps and begin to narrow them by reviewing evidence on the main challenges and barriers to better integrating people with mental illness in the world of work.

    Abstract: The book presents a major meta-analysis of 'value of a statistical life' (VSL) estimates derived from surveys where people around the world have been asked about their willingness to pay for small reduction in mortality risks. The analysis seeks to explain the differences in the estimates, for example across countries. Differences in incomes and the magintude of the risk reduction people have been asked to value were found to be the factors having the strongest impact on VSL, but a number of other policy-relevant factors are also important. Based on the meta-analysis, and a broad review of the literature, the book also presents clear advice on how VSL values best can be used in assessments of environmental, health and transport policies, such as in cost-benefit analyses. Using explicit VSL estimates to quantify the benefits to society of fatality risk reductions can play an important role in the development of more cost-effective public policies.

In response to the Paris Declaration call to “… develop and apply common approaches for ‘Strategic Environmental Assessment’ at sector and national levels” among donors and partners, the Guidance on Applying Strategic Environmental Assessment was endorsed in 2006 by members of the OECD Development Assistance Committee, representatives of developing countries receiving aid, the United Nations Development Programme, the United Nations Environment Programme, the World Bank and many other agencies. Since then, a growing number of countries at all levels of development have legislation or regulations prescribing the application of Strategic Environmental Assessment (SEA)  and many more are introducing it as part of their policy tools. This is creating unique opportunities for better policy making and planning by incorporating environmental considerations into high-level decision-making and opening new mechanisms to build consensus on development priorities within governments themselves and between governments and societies. Many development co-operation agencies and their partners are already making good progress in applying SEA. This publication presents the nine most interesting case studies of SEA in progress, selected from a total 100. These nine cases highlight that SEA can:• Safeguard environmental assets for sustainable poverty reduction and development;• Build public engagement in decision making;• Prevent costly mistakes by alerting decision-makers to potentially unsustainable development options at an early stage in the decision-making process;• Speed up implementation of projects and programmes;• Facilitate co-operation around shared environmental resources and contribute to conflict prevention.

    Abstract: This publication provides preliminary, quantitative estimates of direct budgetary support and tax expenditures supporting the production or consumption of fossil fuels in selected OECD member countries. The information has been compiled as part of the OECD's programme of work to develop a better understanding of environmentally harmful subsidies (EHS). It has been undertaken as an exercise in transparency, and to inform the international dialogue on fossil-fuel subsidy reform. It is also intended to inform the ongoing efforts of G20 nations to reform fossil-fuel subsidies. For each of the 24 OECD countries covered, the Inventory provides a succinct summary of its energy economy, and of the budgetary and tax-related measures provided at the central-government level (and, in the case of federal countries, for selected sub-national units of government) relating to fossil-fuel production or consumption. Many measures listed in this inventory are relative preferences within a particular country's tax system rather than absolute support that can be readily compared across countries, and for that reason no national totals are provided

    Abstract: Literacy for Life is the second report from the Adult Literacy and Life Skills Survey. It presents additional results on the nature and magnitude of the literacy gaps faced by OECD countries and how these gaps have evolved over the medium term. It offers new insights into the factors that influence the formation of adult skills in various settings – at home and at work – for the eleven countries participating in the first and last round of data collection between 2003 and 2008. The study offers comparative evidence on the impact of various factors on the supply of skill. The study offers a special focus on numeracy skills and problem solving skills. It explores the relationships between numeracy and key socio-demographic factors as well as labour market outcomes and earnings. It highlights the importance of problem solving skills by defining this foundational skill and by exploring its determinants as well as its relative role in influencing important labour market outcomes. The report offers also an analysis of performance across multiple skill domains. It investigates the skill profiles of various population groups defined in terms of the demographic and socioeconomic characteristics of those who score at levels deemed to be low in one or more skill domains and explores the resulting consequences. The report concludes by investigating the issue of skill mismatch in the labour market and its relationship to adult learning. The extent and distribution of mismatch between the day-to-day literacy related requirements of workers and the literacy skills they have obtained is an important issue that is being explored in this study.

    Abstract: This Test Guideline is designed to provide an evaluation of reproductive and developmental effects that may occur as a result of pre- and postnatal chemical exposure as well as an evaluation of systemic toxicity in pregnant and lactating females and young and adult offspring. In the assay, sexually-mature males and females rodents (parental (P) generation) are exposed to graduated doses of the test substance starting 2 weeks before mating and continuously through mating, gestation and weaning of their pups (F1 generation). At weaning, pups are selected and assigned to cohorts of animals for reproductive/developmental toxicity testing (cohort 1), developmental neurotoxicity testing (cohort 2) and developmental immunotoxicity testing (cohort 3). The F1 offspring receive further treatment with the test substance from weaning to adulthood. Clinical observations and pathology examinations are performed on all animals for signs of toxicity, with special emphasis on the integrity and performance of the male and female reproductive systems and the health, growth, development and function of the offspring. Part of cohort 1 (cohort 1B) may be extended to include an F2 generation; in this case, procedures for F1 animals will be similar to those for the P animals.

    Abstract: Questo rapporto costituisce una valutazione indipendente su alcuni problemi del turismo italiano condotta da responsabili ed esperti di altri paesi. E’ indicativo, in una qualche misura, di come gli altri vedono l’Italia e come il paese nel suo complesso si confronta con gli altri. L’analisi si è concentrata su cinque grandi questioni del turismo italiano: la sua competitività, la sua organizzazione istituzionale e il rapporto Stato-Regioni, la necessità di migliorare le informazioni statistiche, la qualificazione del capitale unamo, il suo affioramento e la sua formazione, l’attrattività e la promozione del brand Italia.
