    Abstract: This report presents a quantitative assessment of how the presentation of news has changed over the past 30 years and how it varies across platforms. Using RAND-Lex, a suite of tools that combine machine learning and text analysis, the researchers considered such linguistic characteristics as social attitude, sentiment, affect, subjectivity, and relation with authority for four comparisons: newspapers before and after 2000 (through 2017), broadcast television news before and after 2000 (through 2000), broadcast news and prime-time cable programming for the period from 2000 to 2017, and newspapers and online journalism during the 2012-2017 period. Over time, and as society moved from "old" to "new" media, news content has generally shifted from more-objective event- and context-based reporting to reporting that is more subjective, relies more heavily on argumentation and advocacy, and includes more emotional appeals. These changes were observed across platforms, appearing least significant in the evolution of print journalism and most stark in comparisons of broadcast news with prime-time cable programming and of print journalism with online journalism. The report quantifies the sizes of observed changes and provides examples of what these changes look like in context. It also includes a discussion of the implications of these trends for the changing media ecosystem and for Truth Decay-the term RAND uses to refer to the diminishing role of facts and analysis in political discourse

    Abstract: In recent years, the number of U.S. service members treated for psychological health conditions has increased substantially. In particular, at least two psychological health conditions - posttraumatic stress disorder (PTSD) and major depressive disorder (MDD) - have become more common, with prevalence estimates up to 20 percent for PTSD and 37 percent for MDD. Delivering quality care to service members with these conditions is a high-priority goal for the military health system (MHS). Meeting this goal requires understanding the extent to which the care the MHS provides is consistent with evidence-based clinical practice guidelines and its own standards for quality. To better understand these issues, RAND Corporation researchers developed a framework to identify and classify a set of measures for monitoring the quality of care provided by the MHS for PTSD and MDD. The goal of this project was to identify, develop, and describe a set of candidate quality measures to assess care for PTSD and MDD. To accomplish this goal, the authors performed two tasks: (1) developed a conceptual framework for assessing the quality of care for psychological health conditions and (2) identified a candidate set of measures for monitoring, assessing, and improving the quality of care for PTSD and MDD. This document describes their research approach and the candidate measure sets for PTSD and MDD that they identified. The current task did not include implementation planning but provides the foundation for future RAND work to pilot a subset of these measures

    Abstract: Sleep disturbances are a common reaction to stress and are linked to a host of physical and mental health problems. Given the unprecedented demands placed on U.S. military forces since 2001, there has been growing concern about the prevalence and consequences of sleep problems for servicemembers. Sleep problems often follow a chronic course, persisting long after servicemembers return home from combat deployments, with consequences for their reintegration and the readiness and resiliency of the force. Therefore, it is critical to understand the role of sleep problems in servicemembers' health and functioning and the policies and programs available to promote healthy sleep. This report provides the first comprehensive review of sleep-related policies and programs across the U.S. Department of Defense (DoD), along with a set of actionable recommendations for DoD, commanders, researchers, and medical professionals who treat U.S. servicemembers. This multimethod study also examined the rates and correlates of sleep problems among post-deployed servicemembers, finding negative effects on mental health, daytime impairment, and perceived operational readiness. The research reviewed evidence-based interventions to treat sleep disturbances among servicemembers and veterans and exposed several individual- and system-level barriers to achieving healthy sleep. Implementing evidence-based treatments is just one step toward improving sleep across the force; as the research recommendations highlight, it is equally important that policies and programs also focus on preventing sleep problems and their consequences

This report provides the first comprehensive review of sleep-related policies and programs across the U.S. Department of Defense (DoD), along with a set of actionable recommendations for DoD, commanders, researchers, and medical professionals who treat U.S. servicemembers. This multimethod study also examined the rates and correlates of sleep problems among post-deployed servicemembers, finding negative effects on mental health, daytime impairment, and perceived operational readiness. The research reviewed evidence-based interventions to treat sleep disturbances among servicemembers and veterans and exposed several individual- and system-level barriers to achieving healthy sleep. Implementing evidence-based treatments is just one step toward improving sleep across the force; as the research recommendations highlight, it is equally important that policies and programs also focus on preventing sleep problems and their consequences

    Abstract: The U.S. Department of Defense (DoD) has been struggling with increasing rates of suicide among military personnel for the past decade. As DoD continues to implement new programs and examine its policies in an effort to prevent military personnel from taking their own lives, it is important to assess its current responses to suicide and to identify opportunities to enhance these programs and policies. Unfortunately, there is little scientific evidence on how best to respond to suicides, how to ensure that surveillance activities are managed appropriately and that loss survivors are given sufficient support to grieve, how additional suicides can be prevented, and how to honor and respect the decedent and his or her loved ones. At the same time, there are many resource guides intended to provide recommendations for organizations (mostly schools) in responding to suicides. A review of the existing scientific evidence on postvention (responses to prevent additional suicides in the aftermath of a suicide) and guidance for other types of organizations provides potential insights for DoD, however. Complemented by the perspectives of those most intimately touched by military suicide - the family and friends of those who have died - these sources may help DoD formulate its guidance in a practical and sensitive way

    Abstract: The ability to predict future health care costs reasonably accurately is critical to planning for the Centers for Medicare and Medicaid Services (CMS). The models used for such projections to date, however, are limited in terms of their capacity to take into account the complex array of factors likely to affect future spending. To improve CMS's ability to map the effects on spending of such factors as medical breakthroughs and demographic trends, RAND Health developed the Future Elderly Model (FEM), a demographic-economic model framework of health spending projections that enables the user to answer "what-if" questions about the effects of changes in health status and disease treatment on future health care costs. What distinguishes the FEM from other models is its inclusion of a multidimensional characterization of health status, which allows the user to include a richer set of demographic controls as well as comorbid conditions and functional status. This report describes the development of the FEM and its application in four clinical areas: cardiovascular disease, the biology of aging and cancer, neurological disease, and changes in health care services. Beside those involved in planning at the Centers for Medicare and Medicaid Services, it should be of interest to health policy planners and health economists

    Abstract: How is the U.S. Army changing to fulfill its role in light of the new national security strategy? How must it change further to better accomplish its manifold and varied missions?     Abstract: In its FY02 Defense Appropriations Conference Report, Congress expressed concerns about reduced competition resulting in a decline in innovation in the U.S. fixed-wing military aircraft industry. Drawing on primary and secondary sources on the aircraft industry, this report provides a brief survey of industry structure, innovation, and competition in the U.S. fixed-wing combat aircraft industry from its earliest days to the present. It supports a much larger research effort that examines the future of the U.S. military aircraft industrial base in response to the above congressional concerns. The study suggests that it is possible to identify at least five distinct technology eras over the history of fixed-wing, heavier-than-air combat aircraft, each of which began with a period of revolutionary innovation, high rates of technology advancement, and significant improvement in performance. The historical evidence suggests, but does not prove, that an industrial structure that includes numerous prime contractors is conducive to encouraging the onset of periods of higher innovation when demand changes and market conditions are right. Without such an industry structure, new Defense Department initiatives may be necessary to promote high levels of innovation. This is a companion volume to a report on the future viability of the combat aircraft industry: Competition and Innovation in the U.S. Fixed-Wing Military Aircraft Industry (Santa Monica, Calif.: RAND, MR-1656-OSD, 2003) by John Birkler, Anthony G.     Abstract: This report grew out of concerns about the weak levels of mathematical proficiency among U.S. citizens and the inequities in the achievement of students of different ethnic, cultural, and social groups. Debates concerning the value of specific curricula, requirements for teacher knowledge, and standards that students should meet are poorly informed because research evidence is lacking. The report proposes a long-term, strategic program of research and development in mathematics education with the twin goals of raising the level of mathematical proficiency and removing inequalities. The program addresses issues directly related to teaching and learning by focusing on three domains: developing teachers' mathematical knowledge for teaching; teaching and learning mathematical practices--the mathematical know-how that characterizes expertise in learning and using mathematics; and teaching and learning algebra in grades K-12. To gain the necessary political support for the program, the report recommends that work begin in three key areas: studies providing evidence for necessarily political decisions concerning standards of mathematical proficiency to be met by students; studies of current instructional practice and curriculum in U.S. classrooms; and studies that assemble existing measures of mathematical performance or develop new ones that can be used in the proposed program

    Abstract: In the Balanced Budget Act of 1997, Congress mandated that Health Care Financing Administration (HCFA) implement a Prospective Payment System (PPS) for inpatient rehabilitation. The Centers for Medicare and Medicaid Services (CMS, the successor agency to HCFA) issued the final rule governing such a PPS on August 7, 2001 and the system went into effect on January 1, 2002. This report details the analyses that RAND performed to support HCFA's efforts to design, develop, and implement the PPS.     Abstract: In the Balanced Budget Act of 1997, Congress mandated that Health Care Financing Administration (HCFA) implement a Prospective Payment System (PPS) for inpatient rehabilitation. The Centers for Medicare and Medicaid Services (CMS, the successor agency to HCFA) issued the final rule governing such a PPS on August 7, 2001 and the system went into effect on January 1, 2002. This report details the analyses that RAND performed to support HCFA's efforts to design, develop, and implement the PPS. It describes RAND's research on new function-related groups, comorbidities, unusual cases, facility-level adjustments, outlier payments, facility-level adjustments, and assessment instruments.     Abstract: This report analyzes special payments that Medicare has been making to rural providers. These special payments are intended to support the rural health care infrastructure to help ensure access to care for Medicare beneficiaries. The research provides a comprehensive overview of these payments, including documentation of the supply of providers, trends in payments, and Medicare costs per beneficiary. Four types of special payments were examined: (1) payments to sole community hospitals, Medicare-dependent hospitals, and rural referral centers; (2) reimbursements to rural health clinics and federally qualified health centers; (3) bonus payments to physicians in rural health professional shortage areas; and (4) capitation payments in rural counties

As it continues to play a critical part in determining Indonesia's future development, the military itself is undergoing profound change. The authors of this book explore the role of the military in politics and society since the fall of President Suharto in 1998. They examine key research issues that are central to the strategic interests of the United States in Asia: Will the Indonesian military be a constructive force supporting democratic processes or will it opt for authoritarian solutions? What are some realistic goals for further progress on military reform? And how can the United States engage the Indonesian military most effectively to help bring about positive change? To answer these questions, the authors present several strategic scenarios for Indonesia, each of which has important implications for U.S.-Indonesian relations, and propose goals for Indonesian military reform and elements of a U.S. engagement policy
