    Kurzfassung: The ability to predict future health care costs reasonably accurately is critical to planning for the Centers for Medicare and Medicaid Services (CMS). The models used for such projections to date, however, are limited in terms of their capacity to take into account the complex array of factors likely to affect future spending. To improve CMS's ability to map the effects on spending of such factors as medical breakthroughs and demographic trends, RAND Health developed the Future Elderly Model (FEM), a demographic-economic model framework of health spending projections that enables the user to answer "what-if" questions about the effects of changes in health status and disease treatment on future health care costs. What distinguishes the FEM from other models is its inclusion of a multidimensional characterization of health status, which allows the user to include a richer set of demographic controls as well as comorbid conditions and functional status. This report describes the development of the FEM and its application in four clinical areas: cardiovascular disease, the biology of aging and cancer, neurological disease, and changes in health care services. Beside those involved in planning at the Centers for Medicare and Medicaid Services, it should be of interest to health policy planners and health economists

    Kurzfassung: This report grew out of concerns about the weak levels of mathematical proficiency among U.S. citizens and the inequities in the achievement of students of different ethnic, cultural, and social groups. Debates concerning the value of specific curricula, requirements for teacher knowledge, and standards that students should meet are poorly informed because research evidence is lacking. The report proposes a long-term, strategic program of research and development in mathematics education with the twin goals of raising the level of mathematical proficiency and removing inequalities. The program addresses issues directly related to teaching and learning by focusing on three domains: developing teachers' mathematical knowledge for teaching; teaching and learning mathematical practices--the mathematical know-how that characterizes expertise in learning and using mathematics; and teaching and learning algebra in grades K-12. To gain the necessary political support for the program, the report recommends that work begin in three key areas: studies providing evidence for necessarily political decisions concerning standards of mathematical proficiency to be met by students; studies of current instructional practice and curriculum in U.S. classrooms; and studies that assemble existing measures of mathematical performance or develop new ones that can be used in the proposed program

    Kurzfassung: In its FY02 Defense Appropriations Conference Report, Congress expressed concerns about reduced competition resulting in a decline in innovation in the U.S. fixed-wing military aircraft industry. Drawing on primary and secondary sources on the aircraft industry, this report provides a brief survey of industry structure, innovation, and competition in the U.S. fixed-wing combat aircraft industry from its earliest days to the present. It supports a much larger research effort that examines the future of the U.S. military aircraft industrial base in response to the above congressional concerns. The study suggests that it is possible to identify at least five distinct technology eras over the history of fixed-wing, heavier-than-air combat aircraft, each of which began with a period of revolutionary innovation, high rates of technology advancement, and significant improvement in performance. The historical evidence suggests, but does not prove, that an industrial structure that includes numerous prime contractors is conducive to encouraging the onset of periods of higher innovation when demand changes and market conditions are right. Without such an industry structure, new Defense Department initiatives may be necessary to promote high levels of innovation. This is a companion volume to a report on the future viability of the combat aircraft industry: Competition and Innovation in the U.S. Fixed-Wing Military Aircraft Industry (Santa Monica, Calif.: RAND, MR-1656-OSD, 2003) by John Birkler, Anthony G. Brower, Jeffrey A. Drezner, Gordon Lee, Mark Lorell, Giles Smith, Fred Timson, William P.G. Trimble, and Obaid Younossi. It should be of interest to members of Congress, congressional staff members, industry executives, and others in the civilian and uniformed defense policy community interested in the future viability of the U.S. military aircraft industrial base
    Kurzfassung: In its FY02 Defense Appropriations Conference Report, Congress expressed concerns about reduced competition resulting in a decline in innovation in the U.S. fixed-wing military aircraft industry. Drawing on primary and secondary sources on the aircraft industry, this report provides a brief survey of industry structure, innovation, and competition in the U.S. fixed-wing combat aircraft industry from its earliest days to the present. It supports a much larger research effort that examines the future of the U.S. military aircraft industrial base in response to the above congressional concerns. The study suggests that it is possible to identify at least five distinct technology eras over the history of fixed-wing, heavier-than-air combat aircraft, each of which began with a period of revolutionary innovation, high rates of technology advancement, and significant improvement in performance. The historical evidence suggests, but does not prove, that an industrial structure that includes numerous prime contractors is conducive to encouraging the onset of periods of higher innovation when demand changes and market conditions are right. Without such an industry structure, new Defense Department initiatives may be necessary to promote high levels of innovation. To answer these questions, the authors present several strategic scenarios for Indonesia, each of which has important implications for U.S.-Indonesian relations, and propose goals for Indonesian military reform and elements of a U.S. engagement policy

This report details the analyses that RAND performed to support HCFA's efforts to design, develop, and implement the PPS. It describes RAND's research on new function-related groups, comorbidities, unusual cases, facility-level adjustments, outlier payments, facility-level adjustments, and assessment instruments. In addition, it presents RAND's recommendations concerning the payment system and discusses the researchers' plans for further research on the monitoring and refinement of the PPS

    Kurzfassung: This report analyzes special payments that Medicare has been making to rural providers. These special payments are intended to support the rural health care infrastructure to help ensure access to care for Medicare beneficiaries. The research provides a comprehensive overview of these payments, including documentation of the supply of providers, trends in payments, and Medicare costs per beneficiary. Four types of special payments were examined: (1) payments to sole community hospitals, Medicare-dependent hospitals, and rural referral centers; (2) reimbursements to rural health clinics and federally qualified health centers; (3) bonus payments to physicians in rural health professional shortage areas; and (4) capitation payments in rural counties
