    Abstract: The large and growing human and financial cost of dementia provides an imperative for policy action. It is already the second largest cause of disability for the over-70s and it costs $645bn per year globally, and ageing populations mean that these costs will grow. There is no cure or effective treatment for dementia, and too often people do not get appropriate health and care services, leading to a poor quality of life. Our failure to tackle these issues provides a compelling illustration of some of today’s most pressing policy challenges. We need to rethink our research an innovation model, since progress on dementia has stalled and investment is just a fraction of what it is for other diseases of similar importance and profile. But even then a cure will be decades away, so we need better policies to improve the lives of people living with dementia now. Communities need to adjust to become more accommodating of people with dementia and families who provide informal care must be better supported. Formal care services and care institutions need to promote dignity and independence, while coordination of health and care services must be improved. But there is hope: if we can harness big data we may be able to address the gaps in our knowledge around treatment and care.

    Abstract: Variations in health care use within a country are complicated. In some cases they may reflect differences in health needs, in patient preferences or in the diffusion of a therapeutic innovation; in others they may not. There is evidence that some of the observed variations are unwarranted, signalling under- or over-provision of health services, or both. This study documents geographic variations for high-cost and high-volume procedures in select OECD countries. It finds that there are wide variations not only across countries, but within them as well. A mix of patient preferences and physician practice styles likely play an important part in this, but what part of the observed variations reflects over-provision, or whether there are unmet needs, remain largely unexplained. This report helps policy makers better understand the issues and challenges around geographic variations in health care provision and considers the policy options.

Authors are international experts in their fields, and the book contains the most up-to-date information on the issue of genital cutting of infants and children from medical, legal, bioethical, and human rights perspectives.
    Abstract: Every year, across the globe, an estimated 13.3 million boys and 2 million girls are involuntarily subjected to genital cutting. Both male and female genital cutting persist, generating a multi-billion-dollar-a-year industry that is defended by its proponents with dubious studies, manipulated statistics, and an appeal to “custom.” Physicians and parents alike have been misled into believing that these mutilations are beneficial, necessary, and harmless. Today, flawed studies have allowed the promotion of circumcision as a way of combating HIV/AIDS in Africa, an experiment that failed in the USA, where a half-million circumcised males have succumbed to AIDS. These facts notwithstanding, the public and legal outcry against these abuses is increasing. For instance, the high court in Cologne, Germany ruled in 2012 that circumcision harms the child, that the harm is irreversible, that it denies the child the right to his own body, and that circumcision denies the individual the right to choose his own religion. Thus, the issue of circumcision has expanded beyond the arena of medicine and is firmly established as a human rights and legal problem. The contributors to this volume, an international panel of experts in the fields of medicine, law, ethics, anthropology, sociology, history, religion, and politics, thoroughly examine and elucidate this violation of human rights

    Abstract: Confidence in a country’s health care system requires indemnification and deterrence systems that adequately cover liability, provide fair compensation for injury, and deter medical malpractices. Over the last years, in many OECD countries, these systems have experienced difficulties resulting in high-risk specialty physicians and surgeons leaving the practice and the development of expensive and useless—if not risky—defensive medicine. This publication surveys and assesses various types of mechanisms and reforms that best limit and indemnify medical accidents. Reasons for difficulties faced by some compensation and prevention regimes, given the specificities of national circumstances and in particular of health care systems, are examined. The study offers a series of unique and focused policy options for establishing more efficient indemnification and deterrence systems to cope with medical accidents.

    Abstract: How is it that, at the beginning of the twenty-first century, it is still possible for males and females to be denied their inherent right to keep all the body parts with which they were born? Circumcision is a cultural phenomenon that affects 15.3 million children and young adults annually. In terms of gender, 13.3 millions boys and 2 million girls are subjected to the involuntary removal of part or all of their external sex organs every year. Few people, however, ask why such practices persist or how modern societies can tolerate this inherent violation of human rights. The problem of female circumcision is being addressed on an international level, while male circumcision remains a subject many academics are reluctant to fully or impartially examine. This book explores the problem of male and female circumcision in modern society from religious, anthropological, psychological, medical, legal, and ethical perspectives. Bodily Integrity and the Politics of Circumcision: Culture, Controversy, and Change illuminates the vulnerability of human society to medical, economic, and historical pressures.     Abstract: Over one million people world-wide have benefited from successful tissue and organ transplants and survival rates have dramatically improved. But transplantation has become a victim of its own success. The demand for human organs can no longer be met and deaths of persons on waiting lists have more than doubled since 1988. A number of alternatives have been proposed to fill the gap between the supply and demand of organs and the past few years have seen the development of various approaches derived from recent advances in biotechnology. Among these technologies is xenotransplantation - the transplantation of viable cells, tissues and organs from one animal species to another. Xenotransplantation of cells and tissues has been approved for clinical trials in a number of OECD countries.
