    Abstract: Sri Lanka's health system has a long track record of strong performance. This case study describes the main features and achievements of Sri Lanka's high-performing health system, to distill lessons for the rest of the world. UNICO case studies focus on a particular health coverage program. In Sri Lanka, the selected health coverage program is the government's national health service (NHS) that was established in the 1930s and has remained broadly the same since then. Thus, the entire government health system is taken as the health coverage program, without narrowing the focus to a specific program. The relationship between the government system and the private health sector in Sri Lanka is explored in the case study. Looking forward, the pending agenda related to addressing the health needs of an aging population is also discussed

    Abstract: Metropolitan areas drive economies, yet the same elements that contribute to economic growth, such as industrialization accompanied by migrant influx, result in overcrowding and poor housing and sanitation. These factors, coupled with intensive international connectivity, make cities extremely vulnerable to pandemics. Experiences from New York and Sno Paulo show that complex administrative structures; conflicting messages from federal, state, and city governments; human resource shortages; supply chain mismanagement; weak coordination between hospitals and public health systems; and poor linkages with the private sector are all limiting factors of a comprehensive pandemic response. COVID-19 has seriously impacted the delivery of essential health services, especially in cities, where the private sector and public hospitals deliver a significant share of primary care. COVID-19 also presents a major public mental health challenge both for health professionals and the public

    Abstract: Las enfermedades no transmisibles (ENT) son la principal causa de muerte en los paises de ingresos medios-bajos, medios-altos y altos; para 2015 lo seran tambien en los paises de ingresos bajos. En adicion a la mortalidad, las ENT representan cerca de la mitad de la carga de enfermedad medida en anos de vida ajustados por discapacidad (DALY) en los paises de ingresos bajos y medios, y se proyectan grandes aumentos en los DALY relacionados con las ENT. Para responder a este reto, los gobiernos deberan disenar e implementar politicas economicas, sociales y de salud con las cuales se trabaje en las relaciones existentes entre las ENT y la pobreza, y minimizar las perdidas economicas y en salud de la poblacion. Las politicas publicas y el reto de las enfermedades cronicas no transmisibles ofrece un marco de referencia util para los gobiernos en la formulacion de sus estrategias. Los autores senalan que la respuesta politica mas efectiva sera dual: desarrollar programas para evitar tanto como sea posible la amenazadora carga de enfermedad de las ENT-por ejemplo, mediante intervenciones en salud publica y mejores servicios de salud- y simultaneamente prepararse para atender el sistema de salud y las presiones economicas que se originaran con el incremento de las ENT a causa del envejecimiento de las poblaciones.     Abstract: Bangladesh allocates a much smaller share of the government budget to health than countries of similar income level. Reallocation is politically challenging but could offer the biggest potential source of fiscal space for health, larger than the space that economic growth can bring. How those additional resources are used, is also important to improve financial protection. Improving the readiness of the public health care network to provide quality care, particularly at primary health care level, could reduce the use of alternative providers, thereby reducing the need for OOP payments. Ensuring the functionality of community clinics, union level facilities, and upazila health complexes, including a revision of their opening hours would also contribute towards this goal. Ensuring access to pharmaceutical products, particularly for essential NCD-related drugs, at affordable prices could also contribute to a reduction in households OOP payments. In addition to these supply-side interventions, the government could consider demand-side programs where resources follow the patient. For instance, given the large needs and the little resources available, an important pathway to reduce OOP payments while decreasing inequalities would be better targeting public subsidies to the poor and vulnerable. Finally, improving public financial management, policies, and governance will also contribute to the effective use of existing and any additional resources for health

    Abstract: Robust public financial management (PFM) systems are crucial to ensure the efficacy and integrity of public health spending, thereby contributing to improved service coverage and financial protection, as required for achieving universal health coverage. A weak PFM system has impeded implementation of the Bangladesh health care financing strategy 2012-2032. This paper aims to identify and document major PFM challenges in relation to the interventions outlined in this strategy document, on the grounds that relaxing these constraints will strengthen implementation. Further, the study examines PFM barriers in service delivery, such as delays in fund availability and procurement and the lack of operational funds at the facility level. The paper points to a number of obstacles, including the absence of a legal framework for implementing a social health protection scheme, no laws to retain user fees at health facilities or to change financial rules to introduce flexible cash at facilities, district health managers without delegated financial power, noncompliance with audit observations, and need for PFM capacity strengthening. Short-, medium- and long-term actions are presented to address these PFM issues. Removing these barriers will not require significant additional resources, but will offer the potential to significantly enhance value for money for Bangladesh's government health budget

    Abstract: While high levels of political commitment to improve health outcomes in the state have ensured that Andhra Pradesh (AP) ranks fourth among the category of twenty-one large states in the NITI Aayog Health Index, the health sector is challenged by an epidemiological transition with rising incidence of noncommunicable diseases (NCDs). The state has made significant achievements in the last five years in almost all key maternal and child health indicators. Despite these improvements, prevalence of diarrhea and acute respiratory infections (ARIs) among children under five years appears to be on the rise. Wide inter district variations in the health outcomes indicate challenges in equity and access. Since tax-financed government revenues are essential for progress toward universal health coverage (UHC), and efficient allocation and use of such revenues is more critical in resource-constrained environments, the state government agreed to undertake an assessment of public financial management (PFM) systems within the health sector. In recognition of this agenda, an assessment of the potential for achieving better alignment between the PFM and health systems was undertaken in August 2022. AP was the first state in India for the assessment
