    The Operationalisation of Sex and Gender in Quantitative Health–Related Research: A Scoping Review (2022)

    Abstract: Current trends in quantitative health research have highlighted the inadequacy of the usual operationalisation of sex and gender, resulting in a growing demand for more nuanced options. This scoping review provides an overview of recent instruments for the operationalisation of sex and gender in health-related research beyond a concept of mutually exclusive binary categories as male or masculine vs. female or feminine. Our search in three databases (Medline, Scopus and Web of Science) returned 9935 matches, of which 170 were included. From these, we identified 77 different instruments. The number and variety of instruments measuring sex and/or gender in quantitative health-related research increased over time. Most of these instruments were developed with a US-American student population. The majority of instruments focused on the assessment of gender based on a binary understanding, while sex or combinations of sex and gender were less frequently measured. Different populations may require the application of different instruments, and various research questions may ask for different dimensions of sex and gender to be studied. Despite the clear interest in the development of novel sex and/or gender instruments, future research needs to focus on new ways of operationalisation that account for their variability and multiple dimensions.

    Abstract: Background: Evidence on the relationship between socioeconomic position (SEP) and infections with SARS-CoV-2 is still limited as most of the available studies are ecological in nature. This is the first German nationwide study to examine differences in the risk of SARS-CoV-2 infections according to SEP at the individual level. Methods: The ‘CORONA-MONITORING bundesweit’ (RKI-SOEP) study is a seroepidemiological survey among a dynamic cohort of the German adult population (n=15 122; October 2020–February 2021). Dried blood samples were tested for SARS-CoV-2 antibodies and oral-nasal swabs for viral RNA. SEP was measured by education and income. Robust logistic regression was used to examine adjusted associations of SARS-CoV-2 infections with SEP. Results: 288 participants were seropositive, PCR positive or self-reported a previous laboratory-confirmed SARS-CoV-2 infection. The adjusted odds of SARS-CoV-2 infection were 1.87-fold (95% CI 1.06 to 3.29) higher among low-educated than highly educated adults. Evidence was weaker for income differences in infections (OR=1.65; 95% CI 0.89 to 3.05). Highly educated adults had lower odds of undetected infection. Conclusion: The results indicate an increased risk of SARS-CoV-2 infection in low-educated groups. To promote health equity in the pandemic and beyond, social determinants should be addressed more in infection protection and pandemic planning.

    Abstract: Care and the City is a cross-disciplinary collection of chapters examining urban social spaces, in which caring and uncaring practices intersect and shape people’s everyday lives. While asking how care and uncare are embedded in the urban condition, the book focuses on inequalities in caring relations and the ways they are acknowledged, reproduced, and overcome in various spaces, discourses, and practices. This book provides a pathway for urban scholars to start engaging with approaches to conceptualize care in the city through a critical-reflexive analysis of processes of urbanization. It pursues a systematic integration of empirical, methodological, theoretical, and ethical approaches to care in urban studies, while overcoming a crisis-centered reading of care and the related ambivalences in care debates, practices, and spaces. These strands are elaborated via a conceptual framework of care and situated within broader theoretical debates on cities, urbanization, and urban development with detailed case studies from Europe, the Americas, and Asia. By establishing links to various fields of knowledge, this book seeks to systematically introduce debates on care to the interconnecting fields of urban studies, planning theory, and related disciplines for the first time.

    Abstract: Explanatory models (EMs) for illness are highly relevant for patients, and they are also important for clinical diagnoses and treatment. EMs serve to capture patients' personal illness narratives and can help reveal how culture influences these narratives. While much research has aimed to understand EMs in the Western hemisphere, less research has been done on other cultures. Therefore, we investigated local causal attributions for mental illness in Cuba because of its particular history and political system. Although Cuban culture shares many values with Latin American cultures because of Spanish colonization, it is unique because of its socialist political and economic context, which might influence causal attributions. Thus, we developed a qualitative interview outline based on the Clinical Ethnographic Interview and administered interviews to 14 psychiatric patients in Havana. We conducted a thematic analysis to identify repeated patterns of meaning. Six patterns of causal attribution for mental illness were identified: (1) Personal shortcomings, (2) Family influences, (3) Excessive demands, (4) Cultural, economic, and political environment in Cuba, (5) Physical causes, and (6) Symptom-related explanations. In our sample, we found general and Cuba-specific patterns of causal attributions, whereby the Cuba-specific themes mainly locate the causes of mental illness outside the individual. These findings might be related to Cubans' socio-centric personal orientation, the cultural value of familismo and common daily experiences within socialist Cuban society. We discuss how the findings may be related to social stigma and help-seeking behavior.

    Abstract: Building on the works of sociologists of health and illness that have highlighted the effects of visible difference and stigmatisation since Goffman, this article examines the ambivalence of visibility experienced by people with cystic fibrosis (CF), a fatal chronic disease and the artful tactics they employ in carving out a habitable space in an ableist world. Dealing with the ambivalence of being at once inherently ill and apparently healthy is a process of giving constant care and attention to one’s body and its presence in public, and if successful, enabling those affected by it to acquire a new – albeit temporary – healthy self with the help of therapy.

    Abstract: China has seen a rapid decline of the traditional multi-generational household and an increase in rural-to-urban migration, raising concerns about a possible breakdown of the informal support system. Against this background, the paper looks at family care-giving (or the absence thereof) to parents in three different living arrangements: with any child or child-in-law (co-resident); independent with at least one child living in the same community (networked); and without any children in either the household or the community (isolated). It also compares the care-giving arrangements of single elders to those living with a spouse. The sample, which is derived from the comprehensive China Health and Retirement Longitudinal Study (CHARLS), contains data on 887 functionally impaired individuals aged 60 and above. The findings suggest that married parents are mostly cared for by their spouse, even if they co-reside with adult children. Proximity to children is particularly important for single elders, who are more likely to lack a care-giver when living independently. There appears to be a hierarchy in family care responsibilities, where children step in as care-givers only when the spouse is no longer able to fulfil this role. While these findings imply a significant deviation from traditional practices and norms of ‘filial piety’, they can be interpreted as a rational adaptation to the changed economic circumstances in rural China.

    Abstract: This paper reports on a co-laborative laboratory ethnography in a molecular biology laboratory conducting research on environmental epigenetics. It focuses on a single study concerned with the material implications of social differentiation. The analysis briefly raises biopolitical concerns. Its main concern lies with an understanding of the human body as local in its working infrastructure or "inner laboratory", an understanding that emerges from the co-laborative inquiry between biologists and anthropologist. This co-laborative mode of inquiry raises productive tensions within biology as to the universal or local nature of human nature and within anthropology as to the status of human biology within social theory. The paper cannot resolve this tension. Rather it explores it as an epistemic object in the context of interdisciplinarity, ontography and co-laboration. In concluding, it specifies co-laboration as temporary, non-teleological joint epistemic work aimed at producing new kinds of reflexivity.

    Abstract: In recent years, the employment rates of people of pension age have increased considerably. However, longitudinal evidence on the effects of this employment on wellbeing which might contribute to an evaluation of this late-life work is scarce. Based on empirical findings so far and on theoretical approaches to wellbeing, work and retirement, both negative and positive effects of post-retirement work on life satisfaction are plausible. In this paper, we investigate the effects of taking up work again between the ages of 65 and 75 on life satisfaction in different occupational classes in Germany and the United Kingdom. We expect that not only the heterogeneous conditions and experiences of working are crucial for the consequences that post-retirement work has for life satisfaction, but also the institutional arrangements surrounding this form of work. We use data from the German Socio-Economic Panel and the British Household Panel Survey, covering the 1990s and 2000s. Based on fixed-effects regression modelling, we find positive effects of working in both countries, although not all effects are significant. Differentiating by the class of the job in which the older person works, we find mainly positive effects and no significant differences between those who work in a lower-class job and all others. In addition, we find that the positive effect of working on life satisfaction is partly explained by increased satisfaction with household income for those working in a lower-class job in the United Kingdom. We conclude that many of the pessimistic assumptions about people working after pension age cannot be confirmed for our time of observation. However, there are several reasons for believing that the results will be different in the future or for differently defined populations of people working past pension age.

    Abstract: From the 1970s onwards, reproductive medicine made available interventions and treatments that were readily taken up by an increasing number of people “to make kin” in western and “non-western” societies alike. Instead of an unfortunate fate or social problem unwanted childlessness today is seen as a pathological condition that requires medical attention. While there is broad consensus that reproductive medicine is beneficial in principle, new techniques (like pre-implantation diagnosis) regularly trigger heated controversies at the time of their implementation: in public discourses, in legal, political or medical debates as well as in perceptions of patients. Reproductive interventions are perceived as challenging “normal” conception, as disrupting “natural” ontologies, and as questioning ethical consensus. Drawing on ethnographic data from Turkey and Germany, the article comparatively analyzes how different actors respond to these challenges. Using the method of “extended cases” we inquire from a processual perspective how patients use and experience reproductive medicine, how they create “assisted reproductive biographies”; we ask what kinship practices they develop and how they seek support of patient organisations; and we analyze how they modify body images, acquire medical knowledge, reconfigure intimate practices, create new relations and moral obligations. On the basis of these observations the article sets out to theoretically frame normalization practices on an individual, familial, institutional and discursive level.

    Abstract: The molecular biological field of epigenetics has recently attracted attention not only in biology, but also in the broader scientific community and the popular press. Commentators paint a very heterogeneous picture with some arguing that epigenetics is nothing but another aspect of gene regulation, and others enthusiastically proclaiming a paradigmatic shift in developmental biology. This article analyses a particular approach to environmental epigenetics – a subfield of epigenetics that is central to the recent excitement. The focus lies on an ethnographic analysis of research practices that enable a particular lab group to study the impact of different levels of context, for example, changes in the social and material environment, on epigenetic modification and thus phenotypic variation. The article argues that changes in the practice of doing epigenetic biology contribute to a molecularisation of biography and milieu, suggest the configuration of somatic sociality and produce a different concept of the body: the embedded body. This article concludes with a brief discussion of customary biology as a potential new research agenda at the interface of material and social inquiry.

    Abstract: This special issue of Science, Technology, & Human Values critically explores a new stage in which the life sciences and biomedical practices have entered. This new stage is marked by postgenomic developments and an increased interest of life sciences in the everyday lives of people outside laboratories and clinical settings. Furthermore, particular attention is given to many chronic and degenerative disorders such as cardiovascular disease, Alzheimer’s disease, or developmental disorders. These developments coincide—or have become entangled—with a new set of interests that an anthropologically inclined science and technology studies (STS) is bringing to the analyses of biomedical practices. An increased interest is observed in the anthropologically inclined STS in studying phenomena on different scales and in exploring fields that are not readily dominated by technoscientific rationality in practice. The introduction to the special issue examines briefly these developments and situates them in a broader genealogy of different movements that have taken place in the anthropologically inclined subfield of STS since the late 1970s and early 1980s.

    Abstract: Cardiovascular diseases present the leading cause of death worldwide. Over the last decade, their preventio has become not only a central medical and public health issue but also a matter of political concern as well as a major market for pharma, nutrition, and exercise. A preventive assemblage has formed that integrates diverse kinds of knowledges, technologies, and actors, from molecular biology to social work, to foster a specific healthy lifestyle. In this article, the authors analyze this preventive assemblage as a heterogeneous engineer, that is, as an attempt to order complex everyday life into an architecture of modernism. This article draws on research conducted as part of the interdisciplinary research cluster ‘‘preventive self’’ (2006-2009) bringing together analyses from social anthropology, history, linguistics, sociology of knowledge, and medicine. The authors report here primarily from ethnographic investigations into biomedical research, primary care, and educational practices in kindergartens. The authors conclude that the preventive assemblage largely fails to install any kind of singular order. Instead, it is translated into existing orderings producing heterogeneity of a different nuance.

    Abstract: Recent findings in neuroscience have shown differential patterns in brain activity in response to similar stimuli and activities across cultural and social differences. This calls for a framework to understand how such differences may come to be implemented in brains and neurons. Based on strands of research in social anthropology, we argue that human practices are characterized by particular patterns, and that participating in these patterns orders how people perceive and act in particular group- and context-specific ways. This then leads to a particular patterning of neuronal processes that may be detected using e.g. brain imaging methods. We illustrate this through (a) a classical example of phoneme perception (b) recent work on performance in experimental game play. We then discuss these findings in the light of predictive models of brain function. We argue that a 'culture as patterned practices' approach obviates a rigid nature-culture distinction, avoids the problems involved in conceptualizing 'culture' as a homogenous grouping variable, and suggests that participating as a competent participant in particular practices may affect both the subjective (first person) experience and (third person) objective measures of behavior and brain activity.

    Abstract: The rapid development of biomedicine demands a trustworthy, proactive regulatory regime that is able to manage progress with genuine regard for ethical, social and legal concerns. With its recent past of eugenics and euthanasia, Germany is particularly concerned with setting up a fair and transparent approach, able to respond quickly to scientific developments as well as societal concerns. This article reports on the development, implementation and evaluation of a citizen scenario workshop as a tool of participatory prognostics, integrating elements from participatory technology assessment and forecasting. In 7 days of highly structured work and expert support, 24 German participants developed four scenarios on "The Relationship of Biomedicine and the Economy in the Year 2014." Results and evaluation both show that the process (1) leads to scenarios that provide a useful perspective beyond expert opinion; (2) enriches the public and political discourse; and (3) offers a social learning opportunity appreciated by nonprofessionals and experts alike. We are confident in recommending this technique as a useful addition to existing foresight and horizon scanning activities.

    Abstract: The article analyzes knowledges and options, produced by the life sciences, and focuses on emergent social forms and divergent cultural modes in which they are integrated into everyday routines and vernacular thinking. Taking the Cypriot bone marrow database as a case in point, the operation of this „biomedical platform“ is explored as a prototypical experimental and experiental scape. The article argues that it not only brings into existence new biological objects but also disseminates new forms of instituted altruism and de-/re-politicized solidarity. Applying the concept of multiple modernities, that was developed in post-colonial studies and cultural anthropology, and drawing on perspectives from science and technology studies, the article calls for comparative inquiries of the complex dynamics and interdependencies of vernacular culture and recent advances in the production of genomic knowledge.
