    Abstract: On 15 October 2020, in the middle of the COVID-19 pandemic, Dr. Souha Kanj received a Radboud honorary doctorate in recognition of her achievements in the field of infection control and prevention, efforts in antimicrobial stewardship, and research on various infectious diseases, particularly antimicrobial resistant (AMR) bacteria. This edition includes the laudatio and the speech of our honorary doctor. Kanj published the first overview of antibiotic resistance in Arab countries and discovered differences between neighboring countries in the epidemiology of antimicrobial resistance. She also developed infection prevention monitoring and education programs for healthcare workers, and private organizations in the Arab world. Thanks to her efforts, Lebanon has signed the WHO pledge for hand hygiene. Kanj is not only commended for her expertise, didactic skills, and contribution to medicine, but also for her resilience and strong belief in making a positive impact, despite the difficult journey she has been through, having studied during the Lebanese civil war and being faced with numerous challenges that hit Lebanon and the region after that

    Abstract: Preeclampsia is a hypertensive disorder of pregnancy, diagnosed after the 20th week of gestation in women experiencing new-onset hypertension along with symptoms affecting the liver, kidneys, or brain. In some cases, women with preeclampsia develop novel seizures or unexplained coma, at which time they are diagnosed with eclampsia. The mechanisms contributing to preeclampsia and eclampsia are not fully elucidated, although the placenta seems to play a critical role. Previous studies suggest that improper placentation stimulates mitochondrial dysfunction and the exaggerated release of placental-derived molecules including inflammatory cytokines, anti-angiogenic factors, reactive oxygen species, and cell-free nucleic acids into the maternal circulation that cause systemic vascular dysfunction. These, along with maternally derived molecules, act in concert, leading to hypertension and target organ damage during pregnancies complicated by preeclampsia and eclampsia.In this reprint, we present the original research articles and review papers published as part of the Special Issue: "The Pathophysiology of Preeclampsia and Eclampsia" in Cells

    Abstract: When adopting remote sensing techniques in precision agriculture, there are two main areas to consider: data acquisition and data analysis methodologies. Imagery and remote sensor data collected using different platforms provide a variety of information volumes and formats. For example, recent research in precision agriculture has used multispectral images from different platforms, such as satellites, airborne, and, most recently, drones. Although these secondary causes of osteoporosis are the most frequently observed causes of unexpected bone loss, they can only be diagnosed via a high degree of suspicion and clinical experience and by performing the appropriate investigations. In inflammatory disorders such as rheumatoid arthritis or chronic inflammatory bowel diseases, as well as vascular diseases, T-cell activation, and consequently pro-inflammatory cascades, trigger the increased expression of T-cell-derived RANKL. In addition, a new biomarker signature of bone-related miRNAs is promising in certain clinical features. Glucocorticoids, often used to control disease activity, decrease the number and function of osteoblasts and inhibit OPG expression. The ubiquitous occurrence of disease-related secondary changes in bone metabolism implies that numerous medical disciplines need to interact. Screening for secondary causes of osteoporosis and the search for new modes of action should present a substantial aspect of osteoporosis management. However, persistent inflammatory stimuli or the disregulation of the mechanisms of the resolution phase can lead to chronic inflammation and inflammatory-based diseases. Nowadays, commercially approved anti-inflammatory drugs are represented by nonsteroidal anti-inflammatory drugs (NSAID); glucocorticoids (SAID); and, in some cases, immunosuppressant and/or biological drugs. These agents are effective for the relief of the main inflammatory symptoms. However, they induce severe side effects, and most of them are inadequate for chronic use. Starting from these premises, the demand for new, effective, and safe anti-inflammatory drugs has led research in new therapeutic directions. The recent and emerging scientific community slant is oriented towards natural products/compounds that could represent a boon for the discovery of new active molecules and for the development of new drugs and potentially useful therapeutic agents in different inflammatory-related diseases. We hope that it will stimulate the interest of the scientific community involved in studying the effects of natural and synthetic compounds in different fields of interest such as acute and chronic inflammation, inflammatory pain, inflammation-related diseases (e.g., autoimmune diseases), amongst others

    Abstract: Europe’s zero-pollution agenda should depart from an understanding of how the European citizens’ bodies are polluted by synthetic chemicals and prioritize the reduction of the chemical body burden and associated health impacts. In this context, Human Biomonitoring (HBM) is an important and valuable tool for assessing the internal exposure of humans resulting from aggregated exposure to chemicals. Though many obstacles still hinder the use of HBM data, the growing availability of HBM data offers an opportunity for improving and refining Human Risk Assessment. This Special Issue intends to illustrate, using case studies, how HBM data could be used to better estimate internal exposure and resulting risks. Case studies on exposure from consumer products (cosmetic products, non-food products, etc.) or exposures via food or water in the general population or among workers contributed to better identifying the obstacles hindering a broader use of HBM data in RA. A better understanding of population exposure and the exposure of vulnerable groups against health-based human biomonitoring guidance values also provides the basis for effective risk management to reduce impacts on health.     Abstract: Thrombosis is a pathophysiological phenomenon observed in acute and chronic coronary syndrome, stroke, and peripheral artery disease. Exposure of the Tissue Factor (TF) to flowing blood is followed to its binding to other coagulation factors leading to thrombin formation. Together with platelet activation, TF ultimately promotes thrombus formation. Recent advances in platelet pathophysiology have shown that these cells are able to regulate their gene/protein expression, engage in de novo protein synthesis, and release different mediators that may interfere with different cell functions. Several clinical trials have clearly shown the efficacy of anticoagulation and/or anti-platelet aggregation in different thrombotic disorders. Real-world practice clearly indicates that antithrombotic strategies need to be personalized according to patient characteristics. The combination of multiple antithrombotic drugs represents a challenging scenario and was the focus of multiple recent randomized controlled trials, with more still to come. As knowledge about platelets and the coagulation system has substantially evolved within the last few years, so should our approach to antithrombotic treatment; focused on current hurdles of antithrombotic treatments in patients with cardio- and cerebrovascular diseases, starting from the molecular mechanisms involved, proposing practical solutions to compelling clinical scenarios using a pathophysiology-oriented approach on the basis of current clinical evidence

    Abstract: Asthma is a prevalent chronic inflammatory disease of the airways with complex etiology and mechanistic pathways and variable clinical presentation. Such heterogeneity renders the search for targeted therapies difficult. Currently, the best-known disease phenotype with tailored biological treatment is eosinophilic asthma; however, even in some of these patients, anti-IL5 treatment is ineffective. Furthermore, about a quarter of patients, especially those with a severe phenotype, develop progressive and irreversible airway obstruction, leading to persistent symptoms despite optimized treatment. Therefore, new therapeutic approaches are still needed, particularly in severe asthma. The Special Issue presents asthma research focusing on modern personalized therapeutic approaches. Particular interest is directed to novel asthma biomarkers, including molecular and epigenetic ones, shining new light on pathogenesis, phenotyping, and customized therapy in various airway inflammatory patterns of the disease. Principally, resection, transplantation and local ablation are most commonly performed in patients with early-stage HCC tumors, while TACE and systemic therapy are, respectively, the preferred treatment options for intermediate- and advanced-stage tumors. However, the HCC tumor immune microenvironment (TME) affects the response to current anti-PD-1/PD-L1 immunotherapies. Therefore, an enhanced understanding of the immunobiology of TME is essential for the development of predictive biomarkers of patient stratification and strategies of drug combinations to improve therapeutic efficacy, especially for patients with tumors that are unresponsive to anti-PD-1/PD-L1 therapy.     Abstract: The war in Syria has lasted for many years and continues to cast a dark shadow over its people, including healthcare workers caring for those injured and in distress. During the war, healthcare workers have been exposed to unprecedented challenges, becoming targets of bombing, killing, siege, arrest, and torture. Moreover, healthcare workers continue to experience difficult situations that require them to make decisions with no clear or easy moral choice. The present study aimed to understand the experiences of healthcare workers and the difficulties and challenges that hinder applying existing ethical frameworks and codes for disasters within Syria’s context of revolution and war. Qualitative analysis of interviews led to classification of the ethical challenges experienced by healthcare workers into four groups: the risks from providing care; stewardship of resources and work challenges; corruption and organizational pressure; and psychological, emotional, and social stress. The study also showed that both Syrian healthcare workers and the developers of ethical frameworks adopt many similar values and principles. Yet, the participants adopted several unique moral values not identified in ethical frameworks as a result of navigating their professional duties under the circumstances of the Syrian war. At last, recognizing the challenges and precarious tasks healthcare workers cope with during wars and other similar disasters could help volunteers, medical personnel, and humanitarian organizations deciding to work in Syria better understand the specific context and obstacles for ethical decision-making. Moreover, attracting attention for and support of Syrian healthcare workers locally and globally presents a long-lasting benefit

    Abstract: Toll-like receptors (TLRs) represent a powerful system for the recognition and elimination of pathogen-associated molecular patterns (PAMPs) from bacteria, viruses, and other pathogens and damage-associated molecular patterns (DAMPs) released from dying cells. Typical PAMPs include bacterial cell wall components, viral pathogens, or pathogenic nucleic acids, including viral RNA and DNA. Activation of TLRs leads to the production of proinflammatory cytokines and type I interferons which are important for the induction of the host immune response against bacterial and viral infections. However, dysregulation and overstimulation can be detrimental, leading to hyper-inflammation, sepsis, and loss of tissue integrity. The involvement of TLRs in inflammation and bacterial infection has been recognized for a long time. There is an increasing number of reports demonstrating the involvement of TLR activation in a variety of viral infections, associated with protective immunity, but also immune hyper activation and even viral replication. Recent data show the involvement of TLR activation in various acute respiratory viral infections, including SARS-CoV-2 and indicate an essential role in COVID-19 pathology.     Abstract: Arthritis has a high prevalence globally and includes over 100 types, the most common of which are rheumatoid arthritis, osteoarthritis, psoriatic arthritis, and inflammatory arthritis. All types of arthritis share common features of the disease, including monocyte infiltration, inflammation, synovial swelling, pannus formation, stiffness in the joints, and articular cartilage destruction. The exact etiology of arthritis remains unclear, and no cure exists as of yet. Anti-inflammatory drugs (NSAIDs and corticosteroids) are commonly used in the treatment of arthritis. However, these drugs are associated with significant side effects, such as gastric bleeding and an increased risk of heart attack and other cardiovascular problems. It is therefore crucial that we continue to research the pathogenesis of arthritis and seek to discover novel modes of therapy. This reprint summarizes the themes of the 29 articles published in our Special Issue “Research of Pathogenesis and Novel Therapeutics in Arthritis 2.0”.     Abstract: Billions of people suffer from dental problems. Paradoxically, the deteriorating state of teeth is accompanied by the ever-increasing desire to preserve the best facial appearance, which is significantly influenced by teeth aesthetics. This favors the development of dental materials and manufacturing technologies for dental prosthetics, needed to achieve expected effects of clinical treatment.This reprint focus on enhancing antimicrobial and mechanical properties of polymeric materials and composites for dentistry. In recent years, special attention has been focused on the possibility of giving materials new or improved properties by the introduction of nano or submicron size additives, fibers or whiskers. Using agents such as natural oils to enhance antimicrobial properties remains an exciting idea. Another area of research is the application of antibacterial monomers, which can be copolymerized in resins to kill oral pathogenic microflora. The use of new monomers or new compilations of various monomers to improve mechanical properties has also aroused interest. In addition, we are currently looking for new data regarding colonization of dental materials by pathogenic microbes and their influence on the other properties. Further, there are many new commercially available materials which should be investigated to verify their properties, which is important from the point of view of clinical practice. Original new research and reviews related to any of the topics mentioned above indicate the current directions of development of dental materials. We hope that the presented work will be of interest to readers

The field emerged during the 1980s, aligning with the approval of the first regulatory-agency-approved nanomedical oncological drugs. Additionally, nanotechnology has played a pivotal role in the development of mRNA vaccines utilized during the COVID-19 pandemic, further establishing its enduring significance in the domains of science and biomedical innovation. The reasons for the use of nanotechnology in biomedicine can vary but are mostly the protection and/or delivery of bioactive molecules to target tissues. The general idea is to create nanoscopic platforms that can interact differently with biological systems, either through pharmacokinetic modifications or through the preferential activation of some biological pathway. The intricate biomolecular interactions that underlie the functioning of the biological system take place within the nanoscale. So, nanoparticles can be designed to be similar in size to key biological structures like large biomolecules or small organelles, enabling them to interact with biological systems in unique ways, which can improve the effects of drugs used in the treatment of certain diseases. In other words, highly distinct and improved biological effects can emerge from such interactions

    Abstract: Assessing the risk of difficult-to-treat bacterial superinfections, such as multidrug-resistant (MDR) Gram-negative bacilli, according to comorbidities, lung abnormalities or systemic diseases, the severity of pneumonia and the risks of drug-related effects are crucial points for the clinical management. Despite the wide—and not always appropriate—use of broad-spectrum antimicrobials, specific data about bacterial and fungal co-infection in COVID-19 patients are lacking, and confirmed evidence is urgently required to build prospective evidence and to support the development of antimicrobial policy and appropriate stewardship interventions.
