    Abstract: The clinical spectrum of systemic lupus erythematosus (SLE) is highly heterogeneous, ranging from mild disease, which can be limited to skin and joint involvement, to life-threatening conditions with renal impairment, severe cytopenias, central nervous system disease, and thromboembolic events. Apart from the host genetics, several environmental factors, such as sunlight, infections, drugs, and probably hormonal factors, can trigger the onset of symptoms related to SLE. Despite significant advances in our understanding of the pathophysiology and optimization of medical care, patients with SLE still have significant rates of premature mortality and many patients experience severe disease with increased risk of sustaining organ damage and having a reduced health-related quality of life. The development of effective drugs that can induce remission or low disease activity, the unanimous use of definitions of remission and low or high disease activity, flare, and response to therapy, the identification of non-invasive biomarkers of disease activity and long-term outcomes, and the implementation of SLE patients’ perspectives as an integral part of the clinical assessment constitute only a few of the many unmet needs in the field of SLE

    Abstract: An integral part of homeostasis includes daily rhythms in physiology, behavior, and metabolism. The circadian timing system enables organisms to adapt their biological functions to the cyclic nature of the Earth. Sleep is a fundamental physiological function, playing a crucial role in physical, mental and emotional health. Alterations of sleep quality and quantity caused by modern lifestyle and behavior as well sleep disorders are associated with an increased risk for chronic diseases. Chronic misalignment of endogenous rhythms can also lead to metabolic dysfunction. Research is starting to demonstrate the complex interrelationship between nutrition, sleep and the circadian system. For example, some foods and nutrients may impact the availability of substances that influence sleep. On the other hand, inadequate sleep is associated with an unhealthy nutrient intake and altered metabolism. Nutrients can act as drivers of circadian rhythms; they have the ability to influence circadian regulation and alter biological functions in specific tissues. A lacuna still exists in the knowledge surrounding the interplay of nutrition with sleep and the circadian system. In light of this, novel opportunities are disclosed for developing tailored nutritional recommendations with beneficial effects on sleep and circadian rhythmicity so as to improve life quality and prevent disease. The purpose of this Special Issue is therefore to collect contributions regarding this interplay and the related clinical implications

    Abstract: Tryptophan is a rate-limiting essential amino acid and a unique building block of peptides and proteins. This largest amino acid serves as the precursor for the important endogenous indoleamines serotonin, N-acetylserotonin, and melatonin that act as neurotransmitters, neuromodulators, and neurohormones. Kynurenic acid is the most potent endogenous antiexitotoxic agent. Other highly relevant pathways of tryptophan are the reversible transamination to indole-3-pyruvate with formation related indolic acids that act as potent antioxidant agents. Tryptophan metabolites, such as melatonin, and structurally related agents, such as indole-3-propionic acid, act as potent catalytic antioxidants and bioenergetic agents that facilitate regeneration and protection against stress and aging. Several indole compounds act as uremic toxins since these agents can induce radical formation that is associated with enhanced oxidative stress and damage. The exploration of the effects of these protective and toxic tryptophan derived agents has revealed important molecular mechanisms and mediators of adaptation and aging. Research on tryptophan in nutrition and health can facilitate the development of new approaches to extend human health and life span. Amino acids are the building blocks of life that enable repair, as well as recycling and regeneration.     Abstract: Asthma is a prevalent chronic inflammatory disease of the airways with complex etiology and mechanistic pathways and variable clinical presentation. Such heterogeneity renders the search for targeted therapies difficult. Currently, the best-known disease phenotype with tailored biological treatment is eosinophilic asthma; however, even in some of these patients, anti-IL5 treatment is ineffective. Furthermore, about a quarter of patients, especially those with a severe phenotype, develop progressive and irreversible airway obstruction, leading to persistent symptoms despite optimized treatment. Therefore, new therapeutic approaches are still needed, particularly in severe asthma. The Special Issue presents asthma research focusing on modern personalized therapeutic approaches. Particular interest is directed to novel asthma biomarkers, including molecular and epigenetic ones, shining new light on pathogenesis, phenotyping, and customized therapy in various airway inflammatory patterns of the disease.     Abstract: Coronavirus disease 2019 (COVID-19) is a new respiratory disease caused by infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that caused a global pandemic. COVID-19 patients often develop acute respiratory distress syndrome (ARDS), which involves acute and lethal respiratory failure. In a meta-analysis, the median incidence of ARDS in COVID-19 patients was found to be 35 (2-79)%, while the mortality rate in COVID-19-associated ARDS patients was found to be 39 (12.5-73.1)% (Hasan, et al. Expert Rev Repoir Med. 2020; 14: 1149). As of 12 December 2021, 270,373,764 cases had been confirmed as COVID-19 and 5,321,321 cases had resulted in death, according to the World Health Organization webpage. Various symptoms such as persistent fatigue, myalgia, and insomnia can persist after recovery from COVID-19; these symptoms are called “long COVID-19”. Patients with COVID-19 who survive but develop pulmonary fibrosis may experience persistent dyspnea upon exertion, a decrease in pulmonary function, and/or a radiological abnormal shadow. Antifibrotic drugs including nintedanib and pirfenidone are candidate drugs for the treatment of pulmonary fibrosis due to COVID-19; randomized controlled trials are ongoing. However, there are limited studies about respiratory disease caused by COVID-19.     Abstract: Arthritis has a high prevalence globally and includes over 100 types; the most common types are rheumatoid arthritis, osteoarthritis, psoriatic arthritis, and inflammatory arthritis. All types of arthritis share common features, including monocyte infiltration, inflammation, synovial swelling, pannus formation, stiffness in the joints, and articular cartilage destruction. The exact etiology of arthritis remains unclear, and no cure exists as of yet. Anti-inflammatory drugs (NSAIDs and corticosteroids) are commonly used for the treatment of arthritis. However, these drugs are associated with significant side effects, such as gastric bleeding and an increased risk of a heart attack and other cardiovascular problems. Therefore, it is crucial that we continue to research the pathogenesis of arthritis and novel modes of therapy. This reprint summarizes and discusses the themes of 19 articles published in our Special Issue “Research of Pathogenesis and Novel Therapeutics in Arthritis 3.0”.     Abstract: Cannabis has a history of medical use dating back millennia; however, the use of cannabis for medicinal purposes fell out of favor in the middle of the last century. There is currently a renewed interest in the potential medicinal value of this plant, as well as individual compounds produced by Cannabis (as well as synthetic analogs). Currently, nearly 50 countries permit the medical use of cannabis, and over a dozen other countries have legalized cannabis-based products (CBD, Sativex, Dronabinol, etc.) for medical use. While there exists a plethora of anecdotal reports indicating that cannabis can treat a variety of diseases, there is a limited amount of strong, evidence-based scientific data to support these claims. This Special Edition of Biomedicines, “Therapeutic Potential for Cannabis and Cannabinoids”, was dedicated to providing evidence for the medicinal value of cannabis to treat human illness and disease. Topics included, but were not limited to, preclinical animal models examining the efficacy of cannabis and cannabinoid derivatives on pain, anxiety, cancer, and other diseases; and cell and molecular models of cannabinoid pharmacokinetics, drug–drug interactions, and disease treatment.     Abstract: The advent of optical coherence tomography (OCT) has arrived in an era in which the in vivo, cross-sectional imaging of both the retina and choroid is possible. Infra-red rays of light are used to image the chorioretinal layers with an image resolution reaching 10-15 microns. During the last three decades, OCT technology has undergone several improvements, such as spectral domain OCT (SD-OCT), swept source OCT (SS-OCT), enhanced depth imaging (EDI-OCT), wide field OCT, intraoperative OCT, OCT angiography (OCTA), and adaptive optics OCT. These enhancements help in both the qualitative and quantitative analysis of chorioretinal parameters in pathologies such as macular edema, neurosensory detachment, retinoschisis, macular holes, epiretinal membranes, and choroidal tumors. An added advantage is the role of OCT in the evaluation of optic disc disorders, including optic neuritis and glaucoma, via the quantitative analysis of the retinal nerve fiber layer. Moreover, OCT being non-invasive provides an opportunity for serial imaging during follow-up visits. Overall, OCT imaging provides tissue details, closely mimicking the histopathology, and is an invaluable asset in the armamentarium of retina physicians. A great number of different fatty acids (saturated, mono- and di-unsaturated, branched, halogenated, hydroxylated, and methoxylated), as well as the major polyunsaturated fatty acids (PUFAs) such as eicosapentaenoic (EPA) and docosahexaenoic (DHA) acids, occur in marine organisms. In addition, marine organisms are a rich source of metabolites with unusual structures that often exhibit interesting pharmacological properties. Moreover, it is worth emphasizing that the nutritional value of fish and marine invertebrates directly depends on their lipid composition and, in particular, their fatty acid profile, and this represents the basis of the trade of most international marine products.     Abstract: Optic neuropathies are conditions in which there is damage to the optic nerve (ON) caused by a variety of causes, including glaucoma, inflammation, gene abnormalities, ischemia, trauma, and toxicity. ON damage triggers a process of axon degeneration, inflammatory cytokine upregulation, breakdown of the blood–optic nerve barrier, and, eventually, the induction of apoptosis of retinal ganglion cells (RGCs), resulting in optic atrophy. To date, there is no effective treatment for most optic neuropathies; however, because the damage initially is axogenic, there may exist a window of therapeutic opportunity before the death of RGCs.     Abstract: Inflammation is a complex biological response to injury as a result of different stimuli such as pathogens, damaged cells, or irritants. Inflammatory injuries induce the release of a variety of systemic mediators, cytokines, and chemokines that orchestrate cellular infiltration, consequentially bringing about the resolution of inflammatory responses and the restoration of tissue integrity. However, persistent inflammatory stimuli or the disregulation of the mechanisms of the resolution phase can lead to chronic inflammation and inflammatory-based diseases. Nowadays, commercially approved anti-inflammatory drugs are represented by nonsteroidal anti-inflammatory drugs (NSAID); glucocorticoids (SAID); and, in some cases, immunosuppressant and/or biological drugs. These agents are effective for the relief of the main inflammatory symptoms. However, they induce severe side effects, and most of them are inadequate for chronic use. Starting from these premises, the demand for new, effective, and safe anti-inflammatory drugs has led research in new therapeutic directions. The recent and emerging scientific community slant is oriented towards natural products/compounds that could represent a boon for the discovery of new active molecules and for the development of new drugs and potentially useful therapeutic agents in different inflammatory-related diseases. We hope that it will stimulate the interest of the scientific community involved in studying the effects of natural and synthetic compounds in different fields of interest such as acute and chronic inflammation, inflammatory pain, inflammation-related diseases (e.g., autoimmune diseases), amongst others

    Abstract: Europe’s zero-pollution agenda should depart from an understanding of how the European citizens’ bodies are polluted by synthetic chemicals and prioritize the reduction of the chemical body burden and associated health impacts. In this context, Human Biomonitoring (HBM) is an important and valuable tool for assessing the internal exposure of humans resulting from aggregated exposure to chemicals. Though many obstacles still hinder the use of HBM data, the growing availability of HBM data offers an opportunity for improving and refining Human Risk Assessment. This Special Issue intends to illustrate, using case studies, how HBM data could be used to better estimate internal exposure and resulting risks. Case studies on exposure from consumer products (cosmetic products, non-food products, etc.) or exposures via food or water in the general population or among workers contributed to better identifying the obstacles hindering a broader use of HBM data in RA. A better understanding of population exposure and the exposure of vulnerable groups against health-based human biomonitoring guidance values also provides the basis for effective risk management to reduce impacts on health.     Abstract: In the last decade, the incidence of cancer in young patients has increased in several cancer types according to the US Surveillance, Epidemiology and End Result (SEER) data and other worldwide registries. Recently, Sung and colleagues published an extensive pooled analysis from the US in Lancet Public Health, which revealed a dramatic increase in cancer incidence among younger adults (aged 25–49 years), mainly in “obesity-related” cancers, most of which occur in the gastrointestinal (GI) tract. Several studies over the past decade have indicated a significant increase in colorectal cancer incidence among young adults below the age of 50 years. Current evidence suggests that the majority of early-onset GI cancers are sporadic and harbor no familial etiology, indicating the potentially significant role of environmental and behavioral factors in their pathogenesis. Furthermore, the gut microbiome has been proposed to play a key role in CRC carcinogenesis. The scope of this Special Issue of Cancers is to highlight perspectives regarding biology, etiology, late-term toxicities and quality of life which are unique and relevant to young-onset GI cancer cases

This perspective began to change in the early 1990s due to seminal publications from the Adventist Health Study, which showed that nut consumption significantly improved heart health-related outcomes. Many randomized clinical trials, epidemiological studies, and in vitro/in vivo mechanistic studies have since explored the role of eating nuts and its relation to health. Similarly, dried fruits have been scrutinized due to their concentrated sugar content despite their non-sodium micronutrient and fiber density. Due to accumulating evidence on nut and dried fruit intake and health outcomes, an international scientific meeting was organized for leading experts to examine and recapitulate in detail what is well known and established and what avenues of knowledge are still lacking in nut and dried fruit research.     Abstract: Over the last decades, interest in forensic toxicology has continually increased. Consequently, the discipline assumed a leading role, becoming one of the reference sciences for elucidating events of judicial importance. The need to reconcile analytical problems (e.g., relating to the qualitative/quantitative analysis of substances of abuse in complex biological matrices) with judicial requirements (i.e., providing data that can be used as documentary evidence in proceedings) makes the discipline unique among all "analytical" sciences. Thus, the availability of validated procedures, the constant monitoring of analytical performance through quality assurance protocols, the availability of certified “in matrix” standards, and the analytical problems underlying the analysis of new psychoactive substances in the blood and other biological fluids represent the challenges of modern forensic toxicology. The correct interpretation of analytical data, especially to elucidate the extent of impairment induced by a certain substance, and “old” issues related to the correct interpretation of post-mortem data (given the impossibility of translating the therapeutic ranges defined in vivo) represent further areas of research and debate among scientists. Finally, the possibility of interacting with other disciplines (such as proteomics and social sciences) allows for extending the concept of analytical data acquisition/interpretation beyond the classical limits of forensic toxicology (for example, considering the influence of gender when interpreting the effect of a substance)

There are several types of medical sensors that can be utilized for various applications, such as temperature probes, force sensors, pressure sensors, oximeters, electrocardiogram sensors that measure the electrical activity of the heart, heart rate sensors, electroencephalogram sensors that measure the electrical activity of the brain, electromyogram sensors that record electrical activity produced by skeletal muscles, and respiration rate sensors that count how many times the chest rises in a minute. The output of these sensors used to be interpreted by humans, which was time consuming and tedious; however, such interpretations became easy with advances in artificial intelligence (AI) techniques and the integration of the sensor outputs into computer-aided diagnostic (CAD) systems. This reprint presents some of the state-of-the-art AI approaches that are used to diagnose different diseases and disorders based on the data collected from different medical sensors. The ultimate goal is to develop comprehensive and automated computer-aided diagnosis by focusing on the different machine learning algorithms that can be used for this purpose as well as novel applications in the medical field

Crucially, chronic pain is more than a mere by-product of sensory input; it entails the activation of a complex brain network intertwined with biopsychosocial factors. As such, its management emerges as a formidable clinical challenge. Within the scope of this Special Issue, focused attention is given to postoperative pain (POP) and chronic postsurgical pain (CPSP). POP, characterized by its predictability, intensity, and short duration, contrasts starkly with the potentially persistent and troublesome nature of CPSP, which can endure for over three months if not adequately addressed. The prevention of CPSP takes on paramount importance, with regional anesthesia techniques emerging as promising tools for intervention. The pivotal role of multimodal analgesia and combined therapeutic strategies is underscored. The Special Issue also delves into the intriguing realm of gender differences in pain perception, advocating for further research to illuminate these disparities. Risk factors contributing to the development of chronic pain following surgery are meticulously identified. Lastly, this Special Issue explores the potential of new technologies in the field of telemedicine and artificial intelligence to optimize care delivery, opening new avenues for enhanced patient outcomes and treatment strategies

    Abstract: The huge prevalence of type 2 diabetes is a leading human and economic concern worldwide. The diagnosis code of type 2 diabetes includes multiple clinical phenotypes showing different degrees of obesity, fat distribution patterns, types of dyslipidemia, and characteristics of insulin resistance syndrome. In addition, subjects with type 2 diabetes could develop different clinical trajectories, with age being a critical factor in progression. The chronic complications of type 2 diabetes are a major cause of mortality and disability, and are ranked in the top places in western countries’ scores. The pathophysiological heterogeneity of type 2 diabetes is not captured by the current classifications and guidelines. The potential for diabetes progression and complications developing is different depending on the type 2 diabetes cluster, which is mostly regardless of the degree of glycemic control. However, the impact on quality and life expectancy with the current therapeutic approach is not always the same for every individual. Clinical research is the key to reducing the burden of diabetes. New basic, digital, and clinical approaches are obtaining useful knowledge and designing more individualized interventions to prevent, track, and treat diabetic complications. This Special Issue is a clear reflection of this methodological evolution

    Abstract: Childhood cancer or pediatric cancer is one of the leading causes of death among children. Treatment strategies for pediatric cancers include chemotherapy, surgery, radiation, and stem cell transplantation. These therapeutic regimens are well known to develop highly toxic and long-lasting side-effects including secondary cancers in the future lifespan of pediatric patients. Despite these intensive therapeutic regimens, cancer may still relapse as refractory and metastatic cancer. Therefore, further understanding the causes of different pediatric cancers, mechanisms of drug-resistance, and causes of relapse and metastasis is required for developing and advancing less toxic and more effective therapeutic strategies for pediatric cancers.
