    Abstract: In 2007, on a net basis, the United States imported 58 percent of the oil it consumed. This book critically evaluates commonly suggested links between these oil imports and U.S. national security. The major risk to the United States posed by reliance on oil is the economic costs of a major disruption in global oil supplies. On the other hand, the study found no evidence that oil exporters have been able to use embargoes or threats of embargoes to achieve key political and foreign policy goals. Oil revenues are irrelevant for terrorist groups' ability to launch attacks. The study also assesses the economic, political, and military costs and benefits of potential policies to alleviate challenges to U.S. national security linked to imported oil. Of these measures, the adoption of the following energy policies by the U.S. government would most effectively reduce the costs to U.S. national security of importing oil: (1) Support well-functioning oil markets and refrain from imposing price controls or rationing during times of severe disruptions in supply. (2) Initiate a high-level review of prohibitions on exploring and developing new oil fields in restricted areas in order to provide policymakers and stakeholders with up-to-date and unbiased information on both economic benefits and environmental risks from relaxing those restrictions. (3) Ensure that licensing and permitting procedures and environmental standards for developing and producing oil and oil substitutes are clear, efficient, balanced in addressing both costs and benefits, and transparent. (4) Impose an excise tax on oil to increase fuel economy and soften growth in demand for oil. (5) Provide more U.S. government funding for research on improving the efficiency with which the U.S. economy uses oil and competing forms of energy.--Publisher description

This volume documents a study of how relations between the two powers have unfolded in the Persian Gulf, Iraq, Lebanon, and Palestine from 2003 through January 2009. Wehrey et al. detail the complex and multidimensional relationship between Saudi Arabia and Iran and its implications for regional stability and U.S. interests. In doing so, the authors challenge conventional thinking about Saudi-Iranian relations, arguing, for example, that Sunni-Shi'a distinctions are not the key driver in dealings between the two nations, that the two states have a tendency to engage on areas of common interest, and that the notion of a watertight bloc of Gulf Arab states opposing Iran is increasingly unrealistic. The study concludes with U.S. policy recommendations for leveraging the Saudi-Iranian relationship, particularly in the context of a U.S. drawdown in Iraq, the Palestinian-Israeli conflict, and the Iranian nuclear issue

    Abstract: Direct service workers (DSWs) provide personal care or nonmedical services to individuals who need assistance with activities of daily living. Direct service work is very physically and emotionally demanding, and pay for DSWs is too low to attract a stable and sufficiently trained pool of workers that is adequate for the needs of the vulnerable individuals who require their assistance. To help address this issue, in 2003-2004 the Centers for Medicare and Medicaid Services (CMS) awarded ten grants under the Demonstration to Improve the Direct Service Community Workforce; these grants funded initiatives to improve the recruitment and retention among DSWs. Funded initiatives included such efforts as increasing access to health care, training, mentoring, recognition, worker registries, and marketing campaigns. In 2005, CMS funded a national evaluation, by a consortium led by the RAND Corporation, to study the implementation and outcomes of the ten funded initiatives. As part of this evaluation, researchers reviewed grantees' records, interviewed project stakeholders, conducted site visits, and surveyed direct service agencies, DSWs, and consumers. In this volume, the authors present their findings on the implementation and outcomes from the ten grantees

    Abstract: Exposure to community and interpersonal violence is a public health crisis that adversely affects many children in American communities. After witnessing or experiencing trauma, many children experience symptoms of Post-Traumatic Stress Disorder and depression, behavioral problems, substance abuse, and poor school performance. The Support for Students Exposed to Trauma (SSET) program is a series of ten lessons whose structured approach aims to reduce distress resulting from exposure to trauma. Designed to be implemented by teachers or school counselors in groups of 8-10 middle school students, the program includes a wide variety of skill-building techniques geared toward changing maladaptive thoughts and promoting positive behaviors. It is also intended to increase levels of peer and parent support for affected students. Designed for SSET group leaders, the Group Leader Training Manual introduces the SSET concept and provides detailed information on selecting student participants, scheduling lessons, assuring confidentiality, coordinating with clinical backup, managing difficult situations and issues, and conducting group meetings. The Lesson Plans section supplies group leader preparation information and in-depth plans for each lesson, including agendas, example scenarios, suggestions for troubleshooting specific problems, homework assignment instructions, and cross-references to other program documentation. Take-home worksheets, letters to parents, forms, and other program materials are supplied in the section entitled Lesson Worksheets and Materials

    Abstract: Experiences from recent emergencies, such as Hurricanes Katrina and Rita, have shown that current emergency preparedness plans are inadequate to address the unique issues of special needs populations. This toolkit is meant to assist state and local public health agencies improve their emergency preparedness activities. It distills the most relevant strategies, practices, and resources from a variety of sources, including peer-reviewed research, government reports, the trade literature, and public health leaders, to identify priority populations and critical strategies. The contents include potential strategies for addressing special needs, summaries of promising practices implemented in communities across the country, information on how to select one or more practices that will work in a specific community, information on how to determine whether a practice is working, and a Web-based Geographic Information Systems (GIS) tool to identify and enumerate those with special needs in communities across the United States. Used together, this toolkit and the GIS tool are intended to provide a comprehensive resource to enable public health planners to account for special needs populations in their emergency preparedness efforts

The evaluation center is responsible for performing a longitudinal evaluation of the full scope of AHRQ's patient safety activities and for providing regular feedback to support the continuing improvement of this initiative over the four-year project period. This is the fourth and final evaluation report prepared by RAND. It presents new results for the period from October 2005 through September 2006, synthesizes the full evaluation findings over the four-year evaluation period, and discusses how AHRQ activities could be strengthened as the initiative moves forward. It also describes how AHRQ's strategy and activities developed over time, the new knowledge generated by funded projects, and the contributions of various components of the initiative to patient safety. Finally, it presents updated baseline data on selected outcome measures and discusses options for ongoing monitoring of effects on both practices and outcomes

    Abstract: Since October 2001, approximately 1.64 million U.S. troops have been deployed for Operations Enduring Freedom and Iraqi Freedom (OEF/OIF) in Afghanistan and Iraq. Early evidence suggests that the psychological toll of these deployments -- many involving prolonged exposure to combat-related stress over multiple rotations -- may be disproportionately high compared with the physical injuries of combat. Concerns have been most recently centered on two combat-related injuries in particular: post-traumatic stress disorder and traumatic brain injury. With the increasing concern about the incidence of suicide and suicide attempts among returning veterans, concern about depression is also on the rise. The study discussed in this monograph focuses on post-traumatic stress disorder, major depression, and traumatic brain injury, not only because of current high-level policy interest but also because, unlike the physical wounds of war, these conditions are often invisible to the eye, remaining invisible to other servicemembers, family members, and society in general. All three conditions affect mood, thoughts, and behavior; yet, these wounds often go unrecognized and unacknowledged. The effect of traumatic brain injury is still poorly understood, leaving a large gap in knowledge related to how extensive the problem is or how to address it. This monograph summarizes key findings and recommendations from a larger RAND document entitled Invisible Wounds of War: Psychological and Cognitive Injuries, Their Consequences, and Services to Assist Recovery (Tanielian and Jaycox [Eds.], Santa Monica, Calif.: MG-720-CCF, 2008), a comprehensive study RAND conducted of the post-deployment health-related needs associated with the three conditions among OEF/OIF veterans; the health care system in place to meet those needs; gaps in the care system; and the costs of filling those gaps and providing quality health care to all those in need. Readers desiring more details are referred to that document. Both monographs should be of interest to mental health treatment providers; health policymakers, particularly those charged with caring for our nation's veterans; and U.S. service men and women, their families, and the concerned public.     Abstract: The United States participated in several interventions and state-building efforts during the 1990s, and the rationale for U.S. engagement in such efforts received a new urgency after the 9/11 attacks. However, recent U.S. experiences in Afghanistan and in Iraq, especially, have shown that engaging in stability and reconstruction operations is a difficult and lengthy process that requires appropriate resources. Most of all, to have a chance of succeeding, such operations require a realistic understanding of the capabilities needed for them. The authors present the results of research on the U.S. civilian personnel and staffing programs for stability and reconstruction operations undertaken in other countries under U.S. leadership or with the participation of the United States. The study uses the Office of Personnel Management's Human Capital Assessment and Accountability Framework to assess the personnel requirements for such operations. The Framework advocates strategic alignment, workforce planning and development, leadership and knowledge management, results-oriented performance culture, talent management, and accountability. The authors also present recommendations that the U.S. government should consider undertaking to deal with the types of problems that the United States has encountered in post-2003 Iraq. The research draws on the rapidly growing body of literature dealing with reconstruction and stability missions, interviews with U.S. and British civilian personnel deployed to Iraq, and the authors' own experiences in Iraq as U.S. civilians involved with the Coalition Provisional Authority (CPA). The study should be of interest to policymakers dealing with stability and reconstruction operations

    Abstract: The volume of freight transported in the United States is expected to double in the next 30 years. An increased use of rail freight could allow the supply chain to accommodate these increased volumes while minimizing highway congestion and improving energy efficiency in the transportation sector. Shippers and policymakers are concerned that the existing infrastructure--much diminished after decades of track abandonment--lacks sufficient capacity to accommodate the increased demand for rail freight. This report draws from publicly available data on the U.S. railroad industry to provide observations about rail infrastructure capacity and performance in freight transportation. Railroads have improved their productivity in the past three decades, mitigating immediate concerns about capacity, but concerns about future capacity constraints appear to be justified. Insufficient data exist to determine whether rail performance is now stable, significantly declining, or improving. The railroad system is privately owned and operated, but there is a public role for easing rail capacity constraints because private decisions about transportation investment and freight shipping have public consequences for safety and the environment. A better understanding of the public and private cost trade-offs between shipping freight by truck and by rail is needed. Improvements to data quality and freight-modeling tools will improve the ability for policymakers to better target public investment in the rail freight transportation system

    Abstract: The world's two most populous countries, China and India, are undergoing dramatic demographic, societal, and economic transformations. However, the health status of residents of China and India still lags behind relative to other populations, and the health gains in each country have been uneven across subpopulations. Although they have achieved substantial advances in life expectancy and disease prevention since the middle of the 20th century, the Chinese and Indian health systems provide little protection against financial risk, and patient satisfaction is a lower priority than it should be. This paper compares the health systems of China and India to determine what approaches to improving health in these two countries do and do not work. In particular, the authors compare the health systems in China and India along three dimensions: policy levers, intermediate outcomes, and ultimate ends. The authors conclude that both countries must (1) restructure health care financing to reduce the burden of out-of-pocket medical care costs on individual patients; (2) increase access to care, especially in rural areas; (3) reduce dependence on fee-for-service contracts that promote overutilization of medical care; (4) build capacity for addressing and monitoring emerging diseases; and (5) match hospital capabilities with local needs

The studies varied widely in their definition of cost-effectiveness. Methods included cutting the workforce, trading one workforce for another, reinvesting fixed manpower in higher-valued functions, trading end-strength for experience, and making short-term technology investments to reduce manpower in the long term. The authors conclude that the method used to determine manpower requirements may not be as important as other attributes of the studies, such as the direct involvement of a senior decisionmaker; stating specific goals as part of the study charter; a holistic view of the organization being studied; publicly available and auditable results; and a clear set of measurement criteria upon which to make decisions

    Abstract: Since October 2001, approximately 1.64 million U.S. troops have been deployed for Operations Enduring Freedom and Iraqi Freedom (OEF/OIF) in Afghanistan and Iraq. Early evidence suggests that the psychological toll of these deployments -- many involving prolonged exposure to combat-related stress over multiple rotations -- may be disproportionately high compared with the physical injuries of combat. In the face of mounting public concern over post-deployment health care issues confronting OEF/OIF veterans, several task forces, independent review groups, and a Presidential Commission have been convened to examine the care of the war wounded and make recommendations. Concerns have been most recently centered on two combat-related injuries in particular: post-traumatic stress disorder and traumatic brain injury. With the increasing incidence of suicide and suicide attempts among returning veterans, concern about depression is also on the rise. The study discussed in this monograph focuses on post-traumatic stress disorder, major depression, and traumatic brain injury, not only because of current high-level policy interest but also because, unlike the physical wounds of war, these conditions are often invisible to the eye, remaining invisible to other servicemembers, family members, and society in general. All three conditions affect mood, thoughts, and behavior; yet these wounds often go unrecognized and unacknowledged. The effect of traumatic brain injury is still poorly understood, leaving a large gap in knowledge related to how extensive the problem is or how to address it. RAND conducted a comprehensive study of the post-deployment health-related needs associated with these three conditions among OEF/OIF veterans, the health care system in place to meet those needs, gaps in the care system, and the costs associated with these conditions and with providing quality health care to all those in need. This monograph presents the results of our study, which should be of interest to mental health treatment providers; health policymakers, particularly those charged with caring for our nation's veterans; and U.S. service men and women, their families, and the concerned public.     Abstract: Both high import payments for petroleum motor fuels and concerns regarding emissions of carbon dioxide (CO2) are motivating interest in possible fuel substitutes. Petroleum products derived from conventional crude oil constitute more than 50 percent of end-use energy deliveries in the United States and more than 95 percent of all energy used in the U.S. transportation sector. Almost 60 percent of liquid fuels are imported. Emissions from the consumption of petroleum account for 44 percent of the nation's CO2 emissions, with approximately 33 percent of national CO2 emissions resulting from transportation-fuel use. In this report, RAND researchers assess the potential future production levels, production costs, greenhouse gases, and other environmental implications of synthetic crude oil extracted from oil sands and fuels produced via coal liquefaction relative to conventional petroleum-based transportation fuels. The findings indicate the potential cost-competitiveness of these alternative fuels and the potential trade-offs that their deployment requires between economic and environmental considerations

The option that was selected includes internationally benchmarked curriculum standards, national testing based on those standards, independent government-funded schools, and parental choice among schools using annual school report cards. Such strategies depend on the involvement and education of adolescents, parents, law enforcement officials, merchants, and other stakeholders. This guide is designed to take communities through the process of planning, implementing, and evaluating strategies to prevent underage drinking and youth access to alcohol. The guide is structured according to the Substance Abuse and Mental Health Services Administration's (SAMHSA's) Strategic Prevention Framework, a five-step prevention approach. Within the five steps, the guide adopts the Getting To Outcomes model of empowerment evaluation, results-based accountability, and continuous quality improvement. The result is a comprehensive, step-by-step manual for developing, implementing, and evaluating a high-quality communitywide plan to prevent underage drinking and its related consequences. This study examines how 11 terrorist groups in three areas (Mindanao, the West Bank and Gaza Strip, and southwest Colombia) have attempted to exchange technologies and knowledge in an effort to reveal some of their vulnerabilities. The analysis provides the Department of Homeland Security and other national security policymakers with insight into the innovation process and suggests ways that government policies can create barriers to terrorists' adoption of new technologies

Such encroachment pressures limit the ability to conduct mission-essential testing and training. Such development can also destroy or displace native plant and animal species, the result being that military installations become islands of refuge for threatened and endangered species, which can also restrict an installation's operations. In 2003, DoD created the Readiness and Environmental Protection Initiative (REPI) to help address such encroachment pressures. The REPI program helps installations implement compatible land use partnering projects with state and local governments and non-governmental organizations to protect non-military land, which helps relieve installation encroachment pressures. In this monograph, RAND researchers assessed the effectiveness of the REPI projects. The authors also identify the main causes of encroachment; detail the benefits, both to the military and local communities, of buffering areas near installations with REPI projects; and provide recommendations for how to improve REPI's effectiveness

    Abstract: Improving the ability to respond to bioterrorism and other emergencies is an important challenge facing the U.S. public health system. Despite having a knowledgeable workforce, practice and experience, capacity, and partnerships with other responders in the community, the system₂s ability to respond may depend largely on its structure. This study examines a key question: Are state and local public health agencies related to one another in a way that facilitates emergency response? Specific objectives of this study are to explain the factors influencing the particular ways in which state and local public health systems are organized, how the various types of relationships that exist between state and local public health departments have been arrived at, and, most important, the consequences of such structures and relationships for emergency preparedness. We also examine alternative structures from several different types of service industries (public education, banking, the welfare system, and port authorities). Finally, we recommend concrete strategies to improve public health preparedness. This report will be of interest to policymakers and to public health professionals at the state and local levels who are involved in bioterrorism response and emergency preparedness, as well as to other agencies involved in emergency response

    Abstract: The one-third cut in active-duty manpower at the end of the Cold War, from 2.1 million to 1.4 million in uniform, combined with a shift in the national security environment, has today resulted in the need for longer and repeated deployments, especially for the Army and the Marine Corps, and these deployments have posed challenges for active-duty service members and for their families. The authors undertook the preparation of this monograph with the objective of offering insights into the challenges faced by active-duty service members deployed to Iraq and Afghanistan, the resiliency they and their families have shown in coping with these challenges, and the adequacy of defense manpower policy in assisting members and families. The monograph draws on the perspectives of economics, sociology, and psychology; provides a formal model of deployment and retention; reviews published work; reports on the results of focus groups conducted in each of the services; and presents findings from an analysis of survey data. The focus groups and survey data relate to the period from 2003 to early 2004. Family separation, long work hours both during and preceding deployments, and uncertainty surrounding deployments were some of the more significant stresses associated with deployments that were reported in focus groups. Furthermore, according to focus-group and survey data, even personnel who did not deploy experienced work stress as a result of long hours supporting deployments. The authors use these findings to suggest ways to improve quality of life for deployed and nondeployed personnel and their families, including improved communication home, more effectively structured deployment pays, and, when possible, greater certainty in the timing and length of the deployment cycle. The findings also underscore the importance of individual and unit preparation prior to deployment and the role of unit cohesion during deployment. The monograph may be of interest to the military services, the Office of the Secretary of Defense, individual service members and their families, members of Congress and their staff, and the media.     Abstract: The Patient Safety Improvement Corps (PSIC), part of the Agency for Healthcare Research and Quality's (AHRQ's) patient safety initiative, is a program of three one-week sessions (didactic lessons, homework, and a team project) operated collaboratively by the AHRQ and the Veterans' Affairs (VA) National Center for Patient Safety (NCPS). Its purpose is to improve patient safety in the nation by increasing the number and capacity of health care professionals with patient safety knowledge and skills, achieved through training teams from all 50 U.S. states over three years. This report presents findings from RAND's evaluation of the first two years of the PSIC. Data were collected through in-person, group interviews with trainees at the final training session in May 2004 and May 2005, and through individual telephone interviews with the first-year trainees one year later. Overall, reported experiences were positive. Participants valued the broad perspective gained, and the tools and skills they learned and continue to use. They appreciated and continued to draw upon the technical aspects, the hands-on exercises, the knowledge gained through team projects, and the reference materials. Additionally, they value the networking opportunities, and they have made efforts to spread their knowledge. Significantly, there are strong indications that the program has contributed to actions in the field to improve patient safety. Key barriers challenging trainees' program participation and ability to make changes at their home organizations included lack of resources and cultural obstacles (such as blaming individuals for system problems). A need for continued training and programs to train larger, more-diverse teams was also noted. The findings suggest that the PSIC is making important contributions toward building a national infrastructure to support implementation of effective patient safety practices

    Abstract: In November 2005, New Orleans city leaders asked RAND to estimate the repopulation of the city in the aftermath of Hurricane Katrina. The Bring New Orleans Back Commission needed estimates of the city₂s population in the immediate future (the next three to six months) and the near-term future (the next one to three years) to guide the redevelopment planning process. The study was completed in early January 2006. A conceptual framework based on the costs and benefits of migration and on the role of social networks and physical constraints guided the estimates. Housing habitability was determined to be the key driver of the future population of New Orleans. RAND developed an approach to estimating future population for four points in time based on estimates of housing habitability, which were, in turn, determined by floodwater depth and the pace of housing reconstruction, as well as an estimate of the pre-Katrina population by the condition of its housing after Katrina. An important role for policymakers in shaping the repopulation process in New Orleans will be to minimize the uncertainty faced by residents and businesses by speeding up the reconstruction process

This report is an evaluation of the asthma practice guideline demonstration. It documents the extent to which intended actions were implemented, assesses short-term effects on clinical practices, and measures the quality and limitations of available data for monitoring practice improvements and clinical outcomes. The authors found that, although the implementation scored some notable successes, resource limitations and organizational barriers curbed overall progress. They conclude that allowing for flexibility, monitoring the facilities consistently, and training providers thoroughly are the keys to implementing the practice guidelines throughout AMEDD. The also concluded that patient education was an area in need of improvement

The authors provide an overview of various approaches, methodologies, and issues related to program evaluation for health projects in developing countries, and they identify future research and actions by funding organizations that would facilitate evaluations of the impact of large-scale health interventions

Finally, it suggests ways that the job of gathering and analyzing intelligence might best be shared among federal, state, and local agencies

This book-all of whose authors were advisors to the CPA-breaks out the various elements of Iraq?s security sector, including the defense, interior, and justice sectors, and assesses the CPA?s successes and failures. Furthermore, the book identifies six problems underlying the coalition?s approach that need to be addressed if Iraq is to recover from past mistakes. Iraq needs capable security forces in the near term and sustainable security institutions for the long term. The authors emphasize that the onus must remain on the United States and its international partners to ensure that long-term institution-building remains on the Iraqi agenda

    Abstract: The Department of Homeland Security (DHS) is responsible for protecting the United States from terrorism. It achieves this goal partly through the Urban Areas Security Initiative, which allocates resources to states and urban areas. Until DHS can know the effectiveness of available risk-reduction alternatives or determine reasonable minimum standards for community preparedness, allocating homeland security resources based on risk is the next best approach; areas at higher risk are likely to have more and larger opportunities for risk reduction than areas at lower risk. This monograph offers a method for constructing an estimate of city risk shares, designed to perform well across a wide range of threat scenarios and risk types. It also proposes and demonstrates a framework for comparing the performance of alternative risk estimates given uncertainty in measuring the elements of risk. Finally, it makes five recommendations for improving the allocation of homeland security resources: DHS should consistently define terrorism risk in terms of expected annual consequences; DHS should seek robust risk estimators that account for uncertainty about terrorism risk and variance in citizen values; DHS should develop event-based models of terrorism risk; until reliable event-based models are constructed, DHS should use density-weighted population rather than population as a simple risk indicator; and DHS should fund research to bridge the gap between terrorism risk assessment and resource allocation policies that are cost-effective

    Abstract: Since the early 1980s, a prominent and consistent conclusion drawn from research on China₂s defense-industrial complex has been that China₂s defense-production capabilities are rife with weaknesses and limitations. This study argues for an alternative approach: From the vantage point of 2005, it is time to shift the focus of current research to the gradual improvements in and the future potential of China₂s defense-industrial complex. The study found that China₂s defense sectors are designing and producing a wide range of increasingly advanced weapons that, in the short term, are relevant to a possible conflict over Taiwan but also to China₂s long-term military presence in Asia. Part of a larger RAND Project AIR FORCE study on Chinese military modernization, this study examines the current and future capabilities of China₂s defense industry. The goals of this study are to 1. Assess recent trends in China₂s 25-year-long effort to reform its defense industry 2. Analyze the individual strengths and weaknesses of four specific defense-industrial sectors: missile, aircraft, shipbuilding, and information technology 3. Explain variations in performance among different defense-industry sectors, with a focus on differences in institutional arrangements, incentives, and exposure to market forces 4. Evaluate the prospects for China₂s defense industry and its ability to contribute to military modernization. The information presented in this report is based on previous RAND Project AIR FORCE work, and on primary and secondary English and Chinese sources

    Abstract: In the late 1970s and early 1980s, when crude oil prices were high, government and private-sector energy experts took a hard look at the costs and benefits of extracting oil from the vast deposits of oil shale that lie beneath the western United States (much of it under government land). Oil prices soon subsided, and interest in the topic waned. With oil prices again spiking and global demand showing no signs of abating, it makes sense to reexamine the costs and benefits of oil shale development. In this report, the authors describe the oil shale resources (estimated at more than triple the proven oil reserves of Saudi Arabia); the suitability, cost, and performance of technologies for developing these resources; and the key energy, environmental, land-use, and socioeconomic policy issues that need to be addressed by government decisionmakers. The authors conclude by outlining both the challenges and opportunities for early action with regard to those policy issues

    Abstract: In September 2002, RAND contracted with the U.S. Agency for Healthcare Research and Quality (AHRQ) to serve as the evaluation center for its national patient safety initiative. The evaluation center is responsible for performing a longitudinal evaluation of AHRQ's patient safety activities and for providing regular feedback to support the continuing improvement of this initiative over a four-year project period. This report presents findings on the history leading to the AHRQ patient safety initiative, the start-up of the initiative, and early activities through September 2003. It focuses on assessing the context and goals that were the foundation for the initiative and documents the baseline status of the activities being undertaken. The evaluation found the agency has done an impressive job in starting the patient safety initiative, despite unreasonable high expectations and insufficient funding. The evaluators identify four priorities for AHRQ that they believe will have the strongest positive impact on the future of the patient safety initiative: designing interim objectives to pull the health care system toward the long-term goal of reducing errors by 50 percent; developing a national patient safety data repository; participating in active public-private partnerships and supporting health care organizations in their implementation activities; and balancing research and adoption activities

    Abstract: This report is one of several that document a broad, two-year study by RAND Health to better understand the role and importance of Electronic Medical Record Systems (EMR-S) in improving health and reducing healthcare costs, and to help inform government actions that could maximize EMR-S benefits and increase its use. It provides the technical details and results of one component of that study that analyzes interventions in the healthcare system that use EMR-S to affect patient trajectories-i.e., the sequence of encounters a patient has with the healthcare system. The interventions are to improve patient safety, increase preventive services, expand chronic disease management, and foster healthier lifestyles. We identified four classes of trajectory-changing interventions and we selected some important interventions in each class: --Implement Computerized Physician Order Entry (CPOE) as a means to reduce adverse drug events (ADEs). --Increase the provision of the following preventive services: influenza and pneumococcal vaccinations and screening for breast, cervical, and colorectal cancer. --Enroll people with one of four chronic illnesses-asthma, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), or diabetes-in disease management programs. --Persuade people to adopt healthy lifestyles and estimate the health outcomes if everyone did so: controlled their weight, stopped smoking, ate a healthy diet, exercised, and controlled their blood pressure and cholesterol as necessary with medications. We estimated the effects of each intervention on healthcare utilization (e.g., hospital stays, office visits, prescription drug use), healthcare expenditures, and population health outcomes (workdays or schooldays missed, days spent sick in bed, mortality). These interventions generally affect trajectories by improving health and thereby reducing healthcare utilization, or by reducing a costly form of utilization (e.g., inpatient stays) and increasing a more economical form (e.g., office visits to physicians, or prescription medications). The report should be of interest to healthcare IT professionals, other healthcare executives and researchers, and officials in the government responsible for health policy

    Abstract: Innovations in information technology (IT) have improved efficiency and quality in many industries. Healthcare has not been one of them. Although some administrative IT systems, such as those for billing, scheduling, and inventory management, are already in place in the healthcare industry, little adoption of clinical IT, such as Electronic Medical Record Systems (EMR-S) and Clinical Decision Support tools, has occurred. Government intervention has been called for to speed the adoption process for Healthcare Information Technology (HIT), based on the widespread belief that its adoption, or diffusion, is too slow to be socially optimal. In this report, we estimate the current level and pattern of HIT adoption in the different types of healthcare organizations, and we evaluate factors that affect this diffusion process. First, we make an effort to derive a population-wide adoption level of administrative and clinical HIT applications according to information in the Healthcare Information and Management Systems Society (HIMSS)-Dorenfest database (formerly the Dorenfest IHDS+TM Database, Second release, 2004) and compare our estimates to alternative ones. We then attempt to summarize the current state and dynamics of HIT adoption according to these data and briefly review existing empirical studies on the HIT-adoption process. By comparing adoption rates across different types of healthcare providers and geographical areas, we help focus the policy agenda by identifying which healthcare providers lag behind and may need the most incentives to adopt HIT. Next, we employ regression analysis to separate the effects of the provider's characteristics and factors on adoption of Electronic Medical Records (EMR), Computerized Physician Order Entry (CPOE), and Picture Archiving Communications Systems (PACS), and compare the effects to findings in the literature
