    Abstract: The war in Syria has lasted for many years and continues to cast a dark shadow over its people, including healthcare workers caring for those injured and in distress. During the war, healthcare workers have been exposed to unprecedented challenges, becoming targets of bombing, killing, siege, arrest, and torture. Moreover, healthcare workers continue to experience difficult situations that require them to make decisions with no clear or easy moral choice. The present study aimed to understand the experiences of healthcare workers and the difficulties and challenges that hinder applying existing ethical frameworks and codes for disasters within Syria’s context of revolution and war. Qualitative analysis of interviews led to classification of the ethical challenges experienced by healthcare workers into four groups: the risks from providing care; stewardship of resources and work challenges; corruption and organizational pressure; and psychological, emotional, and social stress. The study also showed that both Syrian healthcare workers and the developers of ethical frameworks adopt many similar values and principles. Yet, the participants adopted several unique moral values not identified in ethical frameworks as a result of navigating their professional duties under the circumstances of the Syrian war. At last, recognizing the challenges and precarious tasks healthcare workers cope with during wars and other similar disasters could help volunteers, medical personnel, and humanitarian organizations deciding to work in Syria better understand the specific context and obstacles for ethical decision-making. Moreover, attracting attention for and support of Syrian healthcare workers locally and globally presents a long-lasting benefit

Today as a surgeon with over 25 years of clinical experience he now looks at pieces of art in surgery with professionals compassionately bringing hope to patients.But he also sees inconvenient truths in our healthcare system, with patients evolving from one predictable and preventable disease to the next and with professionals following disease trends rather than applying their knowledge to redirect disease into health. The modern disease-oriented care system becomes undesirable and unsustainable. And there are more inconvenient truths that have progressed rather than diminished in time. Healthcare is not easily accessible to the major half of the global population and for those who have access, with a growing global population, healthcare may become unaffordable in the future. Moreover, from an environmental perspective, both human and planetary health are compromised and ask for a ‘One Health’ approach. Being fascinated by the questions of how and why did this all happen, Kees van Laarhoven took up a scientific survey to comprehend and learn how things still can be changed.     Abstract: Chronic obstructive pulmonary disease (COPD) is a disorder characterized by airflow limitation and is one of the major causes of mortality and morbidity across the globe. Although several guidelines have been published over the past three decades, they were generated in response to the fact that COPD is a major cause of mortality and morbidity and remains an important social problem. In the 20th century, forced expiratory volume in one second (FEV1) was the single most important measure in patients with COPD from discriminative, evaluative, and predictive standpoints. We found that dyspnea is a better mortality predictor than FEV1 in 2002 and exercise capacity in 2003. Subsequently, it has become apparent that physical activity is more predictive of mortality. Furthermore, we have recently developed the tendency to use the reduction of the future risk of exacerbation as the endpoint of relatively large-scale clinical trials rather than the improvement of FEV1. Many researchers have continued to seek better outcome markers and have also discussed what should be used as primary or secondary endpoints in clinical trials in subjects with COPD. We have undoubtedly made great progress but still have a lot of work to do for the best interests of patients

The limited efficacy of current therapeutics and a proportion of non-responders are linked to missing aspects in their pathophysiology at the molecular and cellular levels. High-throughput single- and multi-omics approaches and their combinations are expected to pinpoint the key players underlying IMIDs’ establishment and/or progression, thus suggesting potential therapeutic targets and biomarkers. This Reprint gathered 13 papers presenting recent relevant data related to the assessment of CVD risk in RA, tailored therapeutics in RA-interstitial lung disease, IgG4-related RA, and Sjögren’s syndrome, enabling early identification of PsA and improving outcomes in patients’ with psoriasis. Also, novel non-invasive biomarkers for endoscopic activity and mucosal healing in ulcerative colitis, IBD, and cell-death-associated inflammatory syndromes are proposed. Moreover, a certain subset of telocytes is suggested as the pilot governor of epithelial behavior in preneoplastic gastric lesions, a newer member of IL-1 family, as it is a critical partner in lung adenocarcinoma, while distinct transcriptomic signatures and pathway profiles in human melanocytes corresponding to different Staphylococcal superantigenic enterotoxins are possible targets for therapeutic intervention in cases of toxic shocks. Lastly, Tregs- and dendritic cell-based IMID biomarkers are recommended to enable at more efficient, personalized, and precise treatment strategies

    Abstract: Currently, cancer is the second leading cause of death worldwide and is the most complex and challenging disease known to humankind. Due to the complex underlying mechanisms of tumorigenesis and tumor metastasis, as well as the intrinsic limitations of conventional cancer therapies, the effective treatment of cancer remains a great challenge. Novel therapeutic strategies with superior anticancer ability urgently need to be discovered. In recent years, nano-drug delivery systems have been extensively investigated and applied as alternatives to conventional cancer treatments. Cancer nanomedicines aim to achieve the targeted delivery of chemotherapeutic drugs to the tumor site, utilizing strategies such as passive targeting, active targeting and stimuli-triggered drug release, while simultaneously decreasing drug accumulation in normal tissues, which together lead to improved therapeutic efficacy and reduced side effects. Currently, several cancer nanomedicines are in regular use, and several others are in various stages of development.This Special Issue will highlight recent progress in the development of novel nanomedicines and targeted drug delivery systems for cancer therapy

    Abstract: We are in the modern era of innovations, technological developments and minimisation in the field of urology. While laparoscopy and robotic surgery have revolutionised pelvic and upper tract oncology, there is continuous evolution and increasing use of these in the field of benign conditions and paediatric urology. As lasers have changed endourology, new minimally invasive surgical techniques (MISTs) have given a new meaning to benign prostate hyperplasia (BPH) surgery. Oncological developments for prostate cancer now include treatments such as robotic prostatectomy, brachytherapy, radiotherapy, cryotherapy and HIFU. Robotic partial nephrectomy and cryotherapy have changed the face of renal cancer. While robotic cystectomy is also gaining popularity for muscle-invasive bladder cancer, new concepts of En-bloc resection of bladder cancer are emerging and being increasingly adopted. MISTs for BPH include laser (Holmium, thulium and green light), water-based treatments (Rezum, Aquablation) and other minimally invasive procedures such as prostate artery embolisation (PAE), TIND and Urolift. There is increasing emphasis on the role of suction in endourological procedures, while the quest for better intrarenal pressure and temperature monitoring continues. The use of Thulium fibre laser (TFL), Moses technology and small disposable ureteroscopes all help in pushing the boundaries of ureteroscopy (URS) and percutaneous nephrolithotomy (PCNL) procedures. These new technological developments have led to improvements in patients’ quality of life (QoL), shorter hospital stays and reductions in complication rates

    Abstract: There is growing evidence that among persons with disordered eating behavior, the identification of an “addictive-like eating” phenotype for some persons could be relevant and improve our ability to design better tailored interventions. This “addictive-like eating behavior” phenotype encompasses different terms or concepts, including “food addiction”, “eating addiction”, “compulsive eating behavior”, and “food craving”, but also applies to some persons with binge eating disorder, bulimia nervosa, anorexia nervosa, or binge eating symptoms. Although these terms may theoretically refer to different underlying causes or conceptualizations of addictive-like eating, all agree on the complex and multifaceted public health problem it represents and on the similarities it shares with other addictive disorders in terms of etiology, epidemiology, and treatment. Addictive-like eating behavior can deleteriously impact the patient’s outcome and may lead to poorer weight loss evolution, increased prevalence of co-occurring medical and/or psychiatric disorders, and/or lower quality of life. One of the key challenges for the present and future research is to better identify this subpopulation of patients exhibiting this addictive-like eating phenotype, and to identify the psychological/psychiatric factors and biological mechanisms underlying this increased vulnerability to addictive-like eating. Such preliminary knowledge is crucial for enabling the development of interventions targeting these vulnerability risk factors and, ultimately, improving patient outcomes

    Abstract: At the molecular level, the activation of growth factor receptors (GFRs) induces a mitogenic response and maintains cancer cell growth. The majority of malignant diseases are related to aberrant intra- and intercellular communication, associated with the GFR-mediated pathways. Moreover, the evasion of apoptotic signals and the requirement of angiogenesis were also found to be of fundamental importance for tumor progression and metastasis. In this context, a high expression of GFRs aids blood vessel formation, cell migration, and the inhibition of apoptosis. GFR-directed therapy that would theoretically selectively kill malignant cells and reduce the toxicity associated with nonselective conventional chemotherapy may be a promising treatment for cancer. Many intracellular proteins involved in GFR signal transduction can also function as oncogenes. Mutations affecting key proteins in the RAS/MAPK and PI3K/AKT pathways are known to be crucial in maintaining the malignancy of different types of cancers. This information has guided the development of compounds designed to target one or more of these pathways in cancer cells. Even though there have been important advances in our understanding of GFRs and their signaling, certain essential information is still lacking, and these membrane receptors are still being laboriously studied by several research groups, to find therapeutic solutions to unmet medical needs

    Abstract: Illustrations are a central component of scientific practice. They generate knowledge, shape and order it, communicate, position and discuss it at the same time. Such images, so-called epistemic images, can thus hardly be isolated from their local, social, cultural, and historical contexts. If one wants to understand the production, aesthetics, and characteristic uses of scientific images, it is necessary to take such contexts into account. The object of investigation in this thesis are the images of human embryos in the scientific work of the Göttingen anatomist Erich Blechschmidt (1904 - 1992). Initially conceived and communicated as a specialized collection, Blechschmidt's Human Embryological Documentation Collection and the images derived from it remain an anchor point for a very heterogeneous audience to this day. The work seeks explanatory approaches for the multifaceted history of the impact of Blechschmidt's images, for determining factors of their apparently enormous emotional-intuitive radiance and attraction. This is done in the sense of an iconographic-iconological analysis. The basis for this is a medical-historical investigation in the form of a reconstruction of the history of medical ideas with a focus on concepts of physiological medicine as well as a history of human embryological imaging. DNA breakdown and the overproduction of highly reactive free radicals, reactive oxygen species (ROS) and reactive nitrogen species (RNS) are considered initiators of IR-induced molecular and cellular damages, whereas NO, TGF-β and other pro-inflammatory cytokines are the primary effectors involved in radiation bystander effects (RIBE). Radiomodulators can reduce (radioprotectors and radiomitigators) or increase (radiosensitizers) IR damage. Past world events have highlighted the urgent need to develop predictive biomarkers of the IR absorbed dose and radiation countermeasures to reduce IR damage. Despite the strong economic and scientific efforts over the last decades, at present, drugs for effective protection against lethal IRs remain an unmet need. Moreover, the development of radiosensitizers that selectively increase IR damage in cancer cells but protecting, or at least do not affect, healthy tissues is also of unquestionable importance to improving patient survival and quality of life. It includes contributions that will help to understand the mechanisms involved in radiation-derived cellular responses and damage, the importance of free radical scavengers and antioxidant cellular defenses in preventing harm, and the relevance of the antiinflammatory response to improve recovery.     Abstract: Coronavirus disease 2019 (COVID-19) is a new respiratory disease caused by infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that caused a global pandemic. COVID-19 patients often develop acute respiratory distress syndrome (ARDS), which involves acute and lethal respiratory failure. In a meta-analysis, the median incidence of ARDS in COVID-19 patients was found to be 35 (2-79)%, while the mortality rate in COVID-19-associated ARDS patients was found to be 39 (12.5-73.1)% (Hasan, et al. Expert Rev Repoir Med. 2020; 14: 1149). As of 12 December 2021, 270,373,764 cases had been confirmed as COVID-19 and 5,321,321 cases had resulted in death, according to the World Health Organization webpage. Various symptoms such as persistent fatigue, myalgia, and insomnia can persist after recovery from COVID-19; these symptoms are called “long COVID-19”. Patients with COVID-19 who survive but develop pulmonary fibrosis may experience persistent dyspnea upon exertion, a decrease in pulmonary function, and/or a radiological abnormal shadow. Antifibrotic drugs including nintedanib and pirfenidone are candidate drugs for the treatment of pulmonary fibrosis due to COVID-19; randomized controlled trials are ongoing. However, there are limited studies about respiratory disease caused by COVID-19.     Abstract: Fatty acids (FAs) are considered very important biomolecules because they have been used as trophic biomarkers in the marine food chain analysis of several ecological niches. They are compounds universally present in all organisms and play very important biological roles, such as acting as building blocks in biological membranes and signaling molecules. A great number of different fatty acids (saturated, mono- and di-unsaturated, branched, halogenated, hydroxylated, and methoxylated), as well as the major polyunsaturated fatty acids (PUFAs) such as eicosapentaenoic (EPA) and docosahexaenoic (DHA) acids, occur in marine organisms. In addition, marine organisms are a rich source of metabolites with unusual structures that often exhibit interesting pharmacological properties. Moreover, it is worth emphasizing that the nutritional value of fish and marine invertebrates directly depends on their lipid composition and, in particular, their fatty acid profile, and this represents the basis of the trade of most international marine products. For this Special Issue, we invited academic and industry scientists to submit reviews and original research articles focusing on the chemistry, biological roles, and bioactivities of fatty acids from marine organisms

    Abstract: On 15 October 2020, in the middle of the COVID-19 pandemic, Dr. Souha Kanj received a Radboud honorary doctorate in recognition of her achievements in the field of infection control and prevention, efforts in antimicrobial stewardship, and research on various infectious diseases, particularly antimicrobial resistant (AMR) bacteria. This edition includes the laudatio and the speech of our honorary doctor. Kanj published the first overview of antibiotic resistance in Arab countries and discovered differences between neighboring countries in the epidemiology of antimicrobial resistance. She also developed infection prevention monitoring and education programs for healthcare workers, and private organizations in the Arab world. Thanks to her efforts, Lebanon has signed the WHO pledge for hand hygiene. Kanj is not only commended for her expertise, didactic skills, and contribution to medicine, but also for her resilience and strong belief in making a positive impact, despite the difficult journey she has been through, having studied during the Lebanese civil war and being faced with numerous challenges that hit Lebanon and the region after that

    Abstract: The importance of studying pollution via potentially toxic elements (PTEs) is becoming a key challenge considering the increasing concern about the presence of PTEs in the environment and their potential impacts on ecosystem functioning and public health due to their persistence and biotoxicity. Regional geochemical research, together with geostatistical computations which are used to identify source patterns of different pollutants related to the underlying geological features and/or anthropogenic activities, is the best approach for characterizing this pollution issue. The collection of research in this Special Issue, “Potentially Toxic Element Pollution in Urban and Suburban Environments”, represents the need of the scientific community to characterize the behavior, transport, fate, and ecotoxicological state of PTEs in environmental matrices in both urban and suburban settings. All the case studies collected are of relevant significance due to their quality, both in the methodologies and the achieved results, and interest to different environmental studies, from China to South Africa to Europe.     Abstract: Personalized medicine for Parkinson’s disease is a growing and emerging concept in light of recent recognition that Parkinson’s is a syndromic condition affecting multiple neurotransmitter systems, as well as brain and extracranial structures. The clinical expression is, thus, heterogeneous, and presentation age can range from the 30s to the 90s, with PD in older patients being associated with significant neuropathological comorbidity as well, involving not just misfolded alpha synuclein deposition but also amyloid and tau. Traditional and largely guideline-driven “one size fits all” management strategies adopted in clinical practice are, therefore, often inadequate in holistic management of a patient, particularly when aspects of motor and nonmotor symptoms are taken into consideration. In this supplement of JPM, we present a selection of papers which address several possible strands of personalized medicine in PD, ranging from genomic precision medicine to digital “checklists” to ensure delivery of holistic personalized medicine involving nonpharmacological strategies, as well.     Abstract: Sarcomas are a group of rare cancers that arise in connective tissues of the body. Surgical resection is pivotal for the management of locoregional disease. In locally advanced or metastatic disease settings, systemic therapy has an important role in the multidisciplinary management of sarcoma. Cytotoxic therapy has been the primary treatment for many years. However, recent advances in molecular pathogenesis, the investigation of the tumour microenvironment, changes in clinical trial design, and increased international collaboration have led to the development of histology-driven therapy. Furthermore, genomic profiling has highlighted that, while some sarcomas have complex karyotypes, others are driven by translocation, amplification, and mutation, representing targets for the development of novel therapies. Checkpoint inhibitors have been used as single agents or in combination in clinical sarcoma trials. This progress will move the therapeutic modality in sarcoma patients from the “one-size-fits-all” approach towards a more personalized therapeutic algorithm and better outcomes soon. In this Special Issue, we present original research and review articles highlighting novel therapeutic approaches in the treatment of sarcoma patients

    Abstract: Tryptophan is a rate-limiting essential amino acid and a unique building block of peptides and proteins. This largest amino acid serves as the precursor for the important endogenous indoleamines serotonin, N-acetylserotonin, and melatonin that act as neurotransmitters, neuromodulators, and neurohormones. Kynurenic acid is the most potent endogenous antiexitotoxic agent. Other highly relevant pathways of tryptophan are the reversible transamination to indole-3-pyruvate with formation related indolic acids that act as potent antioxidant agents. Tryptophan metabolites, such as melatonin, and structurally related agents, such as indole-3-propionic acid, act as potent catalytic antioxidants and bioenergetic agents that facilitate regeneration and protection against stress and aging. Several indole compounds act as uremic toxins since these agents can induce radical formation that is associated with enhanced oxidative stress and damage. The exploration of the effects of these protective and toxic tryptophan derived agents has revealed important molecular mechanisms and mediators of adaptation and aging. Research on tryptophan in nutrition and health can facilitate the development of new approaches to extend human health and life span. Amino acids are the building blocks of life that enable repair, as well as recycling and regeneration. Research on nutrients like amino acids, such as tryptophan and its metabolites, as well as peptides and proteins, or extracts containing this molecular metabolism modifiers can improve health. Research into the indololome is a new emerging and rapidly growing field of utmost relevance to science and society

    Abstract: Mangrove communities represent a coastal habitat located in the intertidal zone or brackish water of tropical and subtropical coastal areas, between 5°N and 5°S latitudes, spanning over 118 countries. The special ecological conditions of mangroves include high salinity, nutrient limitation, tidal gradients, high temperatures, excessively high light, and muddy anaerobic or sandy soil, which lead to various morphological and physiological adaptations of inhabiting species and act as an effective selector for the metabolic pathway via the generation of unique functional metabolites with highly unique chemical scaffolds and pharmaceutical application potential. In recent decades, numerous metabolites with uncommon structures and efficacious bioactivities have been discovered in mangrove-derived microorganisms, along with mangrove plants. For this reason, mangrove ecosystems have taken the limelight as an attractive biodiversity hotspot, attracting significant attention from organic chemists and pharmacologists. Additionally, many unique and novel chemical structures with a wide range of structural classes have suggested various biosynthetic origins containing novel functional genes and corresponding enzymes with unique catalytic functions. In this Special Issue, we welcome articles describing recent studies and pertinent reviews focusing on the latest and most important developments in bioactive product discovery from mangrove ecosystems and correlating structures with chemical synthesis, biosynthesis, genomic and metabolomics approaches, biological activities, and pharmaceutical mechanisms.     Abstract: Dual disorders, meaning the coexistence in the same individual of a substance use disorder and another mental disorder, have become an area of great interest in the fields of psychiatry and addiction and in health overall due to their high prevalence, clinical relevance and multicultural presence. This book includes a compilation of papers published between the end of 2021 and the beginning of 2023 focused on recent advances in dual disorders.
