Abstract: Energy systems have played an essential role in the history of human civilization. As our civilization evolves, energy systems are expected to adapt to the environment and desire of people for more sustainable development whilst meeting the ever-increasing energy demand of society. To address global warming and its threats to sustainable development to multiple ends, major economies around the world have announced low-carbon, carbon-neutral, or negative-carbon development targets. To meet these goals, the energy systems as we know them today need to undergo substantial structural changes in terms of the way primary energy is extracted from nature, converted to secondary energy, transmitted from conversion sites to end use, and shifted between time slots to coordinate supply and demand. The share of renewable and fossil energy in the overall energy portfolio could experience unprecedented structural change of a kind not witnessed since industrialization. To cope with this harsh transition, energy systems should be planned, designed, retrofitted, and operated in a revolutionary manner. This reprint aims to present the most recent advances in energy systems analysis towards low/zero/negative carbon emission targets via integration amongst different primary energy supplies, between multiple energy supplies and demands, across geographically separated regions, and over different time scales from seconds to seasons

Abstract: Many heterogeneous causes (e.g., metabolic, inflammatory, autoimmune, vascular, and renal diseases, and even drugs), collectively grouped as secondary causes of osteoporosis, may lead to bone loss or damage to architecture through a number of mechanisms. Although these secondary causes of osteoporosis are the most frequently observed causes of unexpected bone loss, they can only be diagnosed via a high degree of suspicion and clinical experience and by performing the appropriate investigations. In inflammatory disorders such as rheumatoid arthritis or chronic inflammatory bowel diseases, as well as vascular diseases, T-cell activation, and consequently pro-inflammatory cascades, trigger the increased expression of T-cell-derived RANKL. In addition, a new biomarker signature of bone-related miRNAs is promising in certain clinical features. Glucocorticoids, often used to control disease activity, decrease the number and function of osteoblasts and inhibit OPG expression. The ubiquitous occurrence of disease-related secondary changes in bone metabolism implies that numerous medical disciplines need to interact. Screening for secondary causes of osteoporosis and the search for new modes of action should present a substantial aspect of osteoporosis management. In the book, the current management of osteoporosis and related metabolic bone diseases is discussed

Abstract: This reprint focuses on artisanal and small-scale gold mining (ASGM) all over the world, as well as its impact on the economy, environment, and health, providing sustainable solutions to severe ASGM problems. This book presents a total of 20 published articles of regions including Southeast Asia, South America, and South Africa, as well as a general overview of ASGM issues. Studies included in this book posit that (1) ASGM activity is still widely practiced as an alternative livelihood in rural areas in many countries around the world, using toxic elements such as Hg and cyanide due to a lack of legal enforcement on the ASGM sector; (2) ASGM has a negative impact on environmental ecosystems, causes occupational health problems for miners, and causes chronic health disorders in both ASGM communities and those living far from ASGM areas. However, some studies propose that specific potential bioindicators could be used to monitor the environment and health of those living in ASGM communities, providing sustainable solutions to severe ASGM problems

Such activities are frequently time-consuming, repetitive tasks with low added value, which are physically demanding and/or dangerous. Nevertheless, robotics and intelligent systems face several scientific and technological challenges related to their integration and interoperability with other systems, safety, flexibility, re-configurability and autonomy. These challenges are especially relevant when robots operate in real unstructured environments and share a workspace with humans and other equipment. This reprint collects research achievements, ideas, and applications regarding advanced intelligent robotic systems, covering diverse technologies and application domains. Generically, the contributions cover optimal path-planning strategies and innovative designs for mobile manipulators, the integration of robotic and intelligent systems, grasping, manipulation, teleoperation, haptics, user experience approaches for collaborative robots, and multi-agent systems

Abstract: Non-destructive testing and evaluation are commonly referred to as the vast group of analysis techniques used in civil, medical, and industrial sectors to evaluate the properties of materials, tissues, components, or structures, without causing any damage. Automation offers many benefits for non-destructive testing to cope with increasing demands, including improved reliability and higher inspection speeds. Additionally, robots enable inspections in locations that are not easily accessible to human operators and allow for humans to be removed from potentially dangerous environments. However, the perceived complexity and high costs have limited the adoption of automation. As a result, the full potential that could be derived from the seamless integration of robotic platforms with sensors, actuators, and software has not been fully explored; it could still revolutionise the way that automated inspections are performed and conceived. The recent advancements in electronics, robotics, sensor technology and software pave the way for new developments in automated testing and data-driven autonomous robotic inspections in several sectors. This Special Issue aimed to attract the latest research outcomes in the field of robotic sensing. Five papers relate to inspection systems based on robotic fixed-base manipulators, three of which are associated with in-process inspection in manufacturing applications (robotic wire-arc welding and additive manufacturing). Four papers report research advancements in mobile robotic-enabled sensing

Abstract: When adopting remote sensing techniques in precision agriculture, there are two main areas to consider: data acquisition and data analysis methodologies. Imagery and remote sensor data collected using different platforms provide a variety of information volumes and formats. For example, recent research in precision agriculture has used multispectral images from different platforms, such as satellites, airborne, and, most recently, drones. These images have been used for various analyses, from the detection of pests and diseases, growth, and water status of crops to yield estimations. However, accurately detecting specific biotic or abiotic stresses requires a narrow range of spectral information to be analyzed for each application. In data analysis, the volume and complexity of data formats obtained using the latest technologies in remote sensing (e.g., a cube of data for hyperspectral imagery) demands complex data processing systems and data analysis using multiple inputs to estimate specific categorical or numerical targets. New and emerging methodologies within artificial intelligence, such as machine learning and deep learning, have enabled us to deal with these increasing data volumes and the analysis complexity

Abstract: This reprint focuses on land use/land cover (LULC), natural hazards and their interactions, changes, and impacts. Over the past few decades, the risks due to natural hazards have increased significantly, to a large extent due to changes in LULC, which are triggered mainly by anthropic pressure on landscapes, i.e., urbanization, forest management practices, agricultural practices, and the like. As a result, LULC changes contribute significantly to changes in the variability or magnitude of natural hazards, such as floods, landslides, and erosion. Therefore, this reprint provides a collection of studies focused on the interactions between LULC and different types of natural hazards, which were studied in several research areas around the world

Abstract: The Special Issue “Radioactive Pollution and Biological Effects of Radioactivity“of the Journal Life involved the participation of academic scientists, researchers, and scholars from all over the World that contributed with articles, reviews, and case reports, based on high-quality research works. The common topic of the published works concerns the radioactive pollution, which occurs when radioactive elements enter the atmosphere and reach the Earth’s surface, within solids, liquids, or gases, including the human body. This happens after natural and/or man-made activities, such as nuclear tests, industrial waste (e.g., radiodiagnostics), and excesses of naturally occurring radioactive sources. This kind of pollution entails risks of radiological contamination of the environment, with harmful effects on human health caused by the ionizing radiation. This theme has become even more current due to the increasing use of ionizing radiation for domestic, industrial, and medical purposes during the last century. Radiological monitoring is a primary objective of radiation protection in order to estimate and understand the impact of radionuclides on the environment and to assess the health risk for the population. The aim of the Special Issue has been to provide an interdisciplinary platform for researchers to exchange and share their experiences and latest achievements on all aspects of radioactive pollution. Survey data analysis, original and unpublished results of conceptual, constructive, empirical, experimental, and theoretical work were welcome, as well as other concepts related to the radiation field

Abstract: Inflammatory bowel disease (IBD) encompasses chronic idiopathic relapsing and remitting gastrointestinal autoimmune diseases characterized by chronic inflammatory disorders of complex etiology, posing clinical challenges due to their often therapy-refractory nature. The primary disorders within the IBD classification are ulcerative colitis (UC) and Crohn’s disease (CD), sharing similarities but exhibiting distinct differences, sometimes making their discrimination challenging.A prominent feature of IBD is the inflammation of the intestinal mucosa, characterized by the robust and persistent infiltration of immune cells and compromised intestinal barrier integrity, leading to a phenomenon known as “leaky gut.” Inflammation can manifest acutely or chronically, known as relapsing, and can increase in severity over time, thereby causing life-long morbidities and reduced quality of life for affected individuals, underscoring the need for a deeper comprehension of the molecular contributors to disease pathogenesis and progression

Abstract: Discoveries in biomedicine and biotechnology, especially in diagnostics, have made prevention and (self)surveillance increasingly important in the context of health practices. Frederike Offizier offers a cultural critique of the intersection between health, security and identity, and explores how the focus on risk and security changes our understanding of health and transforms our relationship to our bodies. Analyzing a wide variety of texts, from life writing to fiction, she offers a critical intervention on how this shift in the medical gaze produces new paradigms of difference and new biomedically facilitated identities: biosecurity individuals

Abstract: This volume offers a series of insights into the fascinating topic of errors and false opinions in early modern Europe. It explores the semantic richness of the category of ‘error’ in a time when such category becomes crucial to European thought and culture. During decades of increasing normativity in the social and religious sphere as well as in the epistemological status of disciplines, recognizing and correcting error becomes an imperative task whose importance can hardly be overestimated. The efforts at establishing religious, political, and scientific orthodoxy led philosophers, doctors, philologist, scientist, and theologians, to reconsider the very foundations of knowledge in the attempt to dispel errors. Spanning geographically from Italy to France, England, and Germany, the articles here gathered provide stimulating glimpses into one of the most fascinating, multifaceted, and controversial aspects of early modern culture

Abstract: Therapeutic drug monitoring (TDM) has emerged as a useful tool for optimizing biological treatment in patients with inflammatory bowel disease (IBD) and other immune-mediated inflammatory diseases (IMID). Reactive TDM has rationalized the management of primary non-response and secondary loss of response to biologics, while cumulative evidence suggests that proactive TDM is efficacious for optimizing anti-tumour necrosis factor (anti-TNF) therapy in patients with IBD. Proactive TDM can be very useful for optimizing biological therapy in specific IBD populations, such as pregnant women as well as patients with perianal fistulizing Crohn’s disease and acute severe ulcerative colitis who are characterized by high drug clearance. Preliminary data suggest that proactive TDM may also have a role in de-escalating anti-TNF therapy in patients in clinical remission including stopping the immunomodulator when combined with anti-TNF therapy. However, several knowledge gaps and limitations hinder the wide implementation of TDM in clinical practice, including the optimal drug concentration to target the lag time between sampling and results and the lack of harmonization of TDM assays. Recent advances regarding the role of TDM include the use of rapid point-of-care assays and the incorporation of model-informed precision dosing. The aim of this Special Issue is to highlight the clinical utility of TDM and discuss the barriers and recent advances of TDM of biologics in IBD and other IMID

Abstract: Physical activity and exercise testing are two key points in sports medicine. Sports medicine is a field of medicine concerned with the prevention and treatment of injuries and disorders that are related to participation in sports. Clinical assessment in sports medicine can solve many questions about diagnosis, treatment, and prognosis related to health in sports. Physical activity could be defined as any bodily movement produced by skeletal muscles that requires energy expenditure. Physical activity refers to all movement including during leisure time, for transport to get to and from places, or as part of a person’s work. Both moderate- and vigorous-intensity physical activity improve health. Regular physical activity is proven to help prevent and manage noncommunicable diseases such as heart disease, stroke, diabetes, and several cancers. It also helps prevent hypertension, maintain a healthy body weight, and can improve mental health, quality of life, and well-being. Exercise could be defined as a subset of physical activity that is planned, structured, and repetitive and has as a final or an intermediate objective, such as the improvement or maintenance of physical fitness. Gait and posture analysis, clinical assessment, biomechanical analysis, reliability and repeatability research, force platform and electromyography analysis are key in order to improve our knowledge in this area

Abstract: Lower genital tract diseases include vulvovaginal and HPV-related diseases, ranging from benign to pre-neoplastic and neoplastic lesions. Nowadays, there has been increasing interest in innovative methods to diagnose and prevent cervical cancer through HPV primary screening programs. However, other lower genital tract diseases are unrelated to HPV infection; hence, cytology, colposcopy, and sonography might play a crucial role in assessment and management. This Special Issue aims to address current and emerging issues in the field of the diagnosis, assessment, staging, and management of HPV-related and not-related pathology of the lower female anogenital tract. These may include new advancements in HPV testing and laboratory biomarkers, investigations on the actual role of cytology and implementation of diagnostic tools to implement differential diagnosis, and the current clinical management of lower genital tract diseases

Abstract: Viruses in the Parvoviridae family constitutes a most diverse and intriguing field of research. Parvoviruses have a long evolutionary history and, beneath an apparent simplicity and uniformity, can differ widely in structure, genome organization and expression, virus–cell interaction, and impact on the host. In addition to interest in the study of basic virology, the translational implications of research on parvoviruses are relevant since many viruses are important human and veterinary pathogens, while other viruses can be engineered as tools for oncolytic therapy or as sophisticated gene delivery vectors. Exploring the diversity and the inherent complexity in the biology of these apparently simple viruses is an ongoing commitment for the scientific community to address. This Special Issue entitled ‘Advances in Parvovirus Research 2022’ continued the series dedicated to research on viruses belonging to the Parvoviridae family and collected the latest contributions in the field of parvovirus research

Abstract: Sensors used to diagnose, monitor or treat diseases in the medical domain are known as medical sensors. There are several types of medical sensors that can be utilized for various applications, such as temperature probes, force sensors, pressure sensors, oximeters, electrocardiogram sensors that measure the electrical activity of the heart, heart rate sensors, electroencephalogram sensors that measure the electrical activity of the brain, electromyogram sensors that record electrical activity produced by skeletal muscles, and respiration rate sensors that count how many times the chest rises in a minute. The output of these sensors used to be interpreted by humans, which was time consuming and tedious; however, such interpretations became easy with advances in artificial intelligence (AI) techniques and the integration of the sensor outputs into computer-aided diagnostic (CAD) systems. This reprint presents some of the state-of-the-art AI approaches that are used to diagnose different diseases and disorders based on the data collected from different medical sensors. The ultimate goal is to develop comprehensive and automated computer-aided diagnosis by focusing on the different machine learning algorithms that can be used for this purpose as well as novel applications in the medical field

Abstract: With the aging of the population, the incidence rate and number of elderly nervous system diseases have increased sharply. This event has brought huge problems to society. Despite significant efforts being made to explore new treatment options and drugs, the results have been limited. The reason may be due to people's incomplete understanding of the pathogenesis of these age-related diseases. Iron is the most abundant trace element in the human body and is essential for normal life activities. Previous studies have shown that brain iron levels increase with age. The abnormal increase in brain iron levels is closely related to age-related neurological diseases. The disruption of the redox balance may be an important mechanism for the occurrence of neurological diseases caused by brain iron abnormalities. This Special Issue mainly highlights and discusses the latest research progress related to the regulation of brain iron metabolism, redox balance, and the pathogenesis of neurological diseases, such as Alzheimer’s disease (AD), Parkinson’s disease (PD), cerebral ischemia, cancer and maintenance of cellular stemness. The molecular mechanisms of iron-misregulation-induced redox imbalance in disease pathogenesis were analyzed. On this basis, further discussions were conducted on potential therapeutic targets for regulating iron metabolism to achieve effective intervention in elderly neurological diseases

Abstract: Research in ophthalmic drug delivery has developed significant advances in the few last years, and efforts have been made to develop more effective topical formulations to increase drug bioavailability, efficiency, and safety. Drug delivery to the posterior segment of the eye remains a great challenge in the pharmaceutical industry due to the complexity and particularity of the eye's anatomy and physiology. Some advances have been made with the purpose of maintaining constant drug levels in the site of action. The anatomical ocular barriers have a great impact on drug pharmacokinetics and, subsequently, on the pharmacological effect.Despite the increasing interest in efficiently reaching the posterior segment of the eye with reduced adverse effects, there is still a need to expand the knowledge of ocular pharmacokinetics that allow the development of safer and more innovative drug delivery systems. These novel approaches may greatly improve the lives of patients with ocular pathologies

Abstract: Sudden cardiac death (SCD) affects more than four million patients per year worldwide, and accounts for up to 50% of all cardiovascular deaths. Related to the improved treatment options for cardiovascular diseases during the past several decades (e.g., improved guideline adherence to pharmacotherapies, increasing supply with cardiac devices, and better revascularization strategies), the characteristics of patients presenting with SCD have significantly changed, making risk prediction for SCD even more complex. The present Special Issue reprint addresses the prognostic impact of patients’ characteristics (such as body mass index), airway and ventilation management in patients needing cardiopulmonary resuscitation, and the prognostic value of a targeted ambulance treatment quality improvement programme. Furthermore, the prognostic value of blood-derived biomarkers and cardiac pharmacotherapies for the prevention of SCD was investigated

Abstract: Thanks to the breast cancer research of recent decades, effective methods and strategies have been established, allowing the mortality rates of breast cancer patients to decrease more and more. However, we still face many challenges. The role of certain biomarkers or mutations is still not clear for treatment and surveillance. Novel drugs are often associated with a different spectrum of adverse events than that seen for conventional therapies, accordingly having an impact on patients’ compliance behaviors. In breast cancer surgery, in some cases, the question remains whether to escalate or deescalate. Additionally, with regard to diagnostics, in a digitalized world, home-based tools and therapy monitoring options are of high importance, especially under the tough conditions and supply problems seen during the COVID-19 pandemic. This Special Issue gathers original research articles and reviews demonstrating therapeutic challenges, current research strategies, and novel diagnostics in breast cancer

Abstract: This Special Issue delves deeply into the intricate distinctions between acute and chronic pain. Crucially, chronic pain is more than a mere by-product of sensory input; it entails the activation of a complex brain network intertwined with biopsychosocial factors. As such, its management emerges as a formidable clinical challenge. Within the scope of this Special Issue, focused attention is given to postoperative pain (POP) and chronic postsurgical pain (CPSP). POP, characterized by its predictability, intensity, and short duration, contrasts starkly with the potentially persistent and troublesome nature of CPSP, which can endure for over three months if not adequately addressed. The prevention of CPSP takes on paramount importance, with regional anesthesia techniques emerging as promising tools for intervention. The pivotal role of multimodal analgesia and combined therapeutic strategies is underscored. The Special Issue also delves into the intriguing realm of gender differences in pain perception, advocating for further research to illuminate these disparities. Risk factors contributing to the development of chronic pain following surgery are meticulously identified. Lastly, this Special Issue explores the potential of new technologies in the field of telemedicine and artificial intelligence to optimize care delivery, opening new avenues for enhanced patient outcomes and treatment strategies

Abstract: With increasing speed, new diagnostic and therapeutic approaches for facial nerve disorders are being published. Many of them have improved decision-making, treatment, and the rehabilitation of patients. Unfortunately, there is often a lack of high-quality studies that prove long-term effects or identify subgroups that are of particular benefit. Even a reliable characterization of patients with facial nerve disabilities is often complicated. Therefore, evidence-based management remains challenging. With this Special Issue on the topic of “Evidence-Based Diagnosis and Management of Facial Nerve Disorders”, we have brought together experts from various fields, including speech pathology, plastic surgery, otolaryngology, neurology, and physical therapy, to present the latest developments in relation to facial nerve disorders and to present a great variety of topics to our readership. We can see that the topic of facial nerve palsy offers a vast and dynamic spectrum of developments, and hope that the research presented in this Special Issue shows the importance of a multidisciplinary approach. We hope it will not only enhance our knowledge of this condition, but also ultimately improve patient outcomes and their quality of life to some extent

Abstract: Chronic immune-mediated inflammatory diseases (IMIDs), usually accompanied by cardiovascular disease and neoplasias, are now prevailed worldwide. The limited efficacy of current therapeutics and a proportion of non-responders are linked to missing aspects in their pathophysiology at the molecular and cellular levels. High-throughput single- and multi-omics approaches and their combinations are expected to pinpoint the key players underlying IMIDs’ establishment and/or progression, thus suggesting potential therapeutic targets and biomarkers. This Reprint gathered 13 papers presenting recent relevant data related to the assessment of CVD risk in RA, tailored therapeutics in RA-interstitial lung disease, IgG4-related RA, and Sjögren’s syndrome, enabling early identification of PsA and improving outcomes in patients’ with psoriasis. Also, novel non-invasive biomarkers for endoscopic activity and mucosal healing in ulcerative colitis, IBD, and cell-death-associated inflammatory syndromes are proposed. Moreover, a certain subset of telocytes is suggested as the pilot governor of epithelial behavior in preneoplastic gastric lesions, a newer member of IL-1 family, as it is a critical partner in lung adenocarcinoma, while distinct transcriptomic signatures and pathway profiles in human melanocytes corresponding to different Staphylococcal superantigenic enterotoxins are possible targets for therapeutic intervention in cases of toxic shocks. Lastly, Tregs- and dendritic cell-based IMID biomarkers are recommended to enable at more efficient, personalized, and precise treatment strategies

Abstract: In his professional journey, travelling through an upcoming field of surgery with apparently unlimited possibilities, Kees van Laarhoven witnessed major milestones and breakthroughs. Already as a medical student, he was amazed by surgical courage. During residency, he experienced hard work, diligence and perseverance of surgical professionals. Today as a surgeon with over 25 years of clinical experience he now looks at pieces of art in surgery with professionals compassionately bringing hope to patients. But he also sees inconvenient truths in our healthcare system, with patients evolving from one predictable and preventable disease to the next and with professionals following disease trends rather than applying their knowledge to redirect disease into health. The modern disease-oriented care system becomes undesirable and unsustainable. And there are more inconvenient truths that have progressed rather than diminished in time. Healthcare is not easily accessible to the major half of the global population and for those who have access, with a growing global population, healthcare may become unaffordable in the future. Moreover, from an environmental perspective, both human and planetary health are compromised and ask for a ‘One Health’ approach. Being fascinated by the questions of how and why did this all happen, Kees van Laarhoven took up a scientific survey to comprehend and learn how things still can be changed. He found that clear solutions are there! Please join him on his journey

Abstract: The advent of optical coherence tomography (OCT) has arrived in an era in which the in vivo, cross-sectional imaging of both the retina and choroid is possible. Infra-red rays of light are used to image the chorioretinal layers with an image resolution reaching 10-15 microns. During the last three decades, OCT technology has undergone several improvements, such as spectral domain OCT (SD-OCT), swept source OCT (SS-OCT), enhanced depth imaging (EDI-OCT), wide field OCT, intraoperative OCT, OCT angiography (OCTA), and adaptive optics OCT. These enhancements help in both the qualitative and quantitative analysis of chorioretinal parameters in pathologies such as macular edema, neurosensory detachment, retinoschisis, macular holes, epiretinal membranes, and choroidal tumors. An added advantage is the role of OCT in the evaluation of optic disc disorders, including optic neuritis and glaucoma, via the quantitative analysis of the retinal nerve fiber layer. Moreover, OCT being non-invasive provides an opportunity for serial imaging during follow-up visits. Overall, OCT imaging provides tissue details, closely mimicking the histopathology, and is an invaluable asset in the armamentarium of retina physicians. This special issue includes applications of OCT in various chorioretinal disorders

Abstract: Fatty acids (FAs) are considered very important biomolecules because they have been used as trophic biomarkers in the marine food chain analysis of several ecological niches. They are compounds universally present in all organisms and play very important biological roles, such as acting as building blocks in biological membranes and signaling molecules. A great number of different fatty acids (saturated, mono- and di-unsaturated, branched, halogenated, hydroxylated, and methoxylated), as well as the major polyunsaturated fatty acids (PUFAs) such as eicosapentaenoic (EPA) and docosahexaenoic (DHA) acids, occur in marine organisms. In addition, marine organisms are a rich source of metabolites with unusual structures that often exhibit interesting pharmacological properties. Moreover, it is worth emphasizing that the nutritional value of fish and marine invertebrates directly depends on their lipid composition and, in particular, their fatty acid profile, and this represents the basis of the trade of most international marine products. For this Special Issue, we invited academic and industry scientists to submit reviews and original research articles focusing on the chemistry, biological roles, and bioactivities of fatty acids from marine organisms

Abstract: Several factors have been identified as interfering with the success, rehabilitation, and fitness of athletes from childhood to adulthood, as well as in para-sport, and special populations, according to research. The performance and health of this population are affected by the relationships between stress, maturation, training load, and recovery. Environmental approaches aim to increase efficiency and physiological adaptations in this sense. In various situations and conditions, however, this stimulus–performance–adaptation relationship varies. As a result, we received contributions related (but not limited) to the following topics: training load monitoring; stress and physiological responses during exercise or sports; recovery process after exercise; changes after stress and/or training load; physiology of training in health and sports performance

Abstract: Hepatocellular carcinoma (HCC) is the most prevalent primary liver cancer, accounting for 〉80% of primary liver cancers worldwide. The diagnosis of HCC, relying solely on noninvasive criteria, is currently under debate due to the need for molecular information that necessitates tissue biopsies, while therapy is provided in accordance with tumor stages and the anticipated advantages of major interventions, following the Barcelona Clinic Liver Cancer (BCLC) staging system. Principally, resection, transplantation and local ablation are most commonly performed in patients with early-stage HCC tumors, while TACE and systemic therapy are, respectively, the preferred treatment options for intermediate- and advanced-stage tumors. However, the HCC tumor immune microenvironment (TME) affects the response to current anti-PD-1/PD-L1 immunotherapies. Therefore, an enhanced understanding of the immunobiology of TME is essential for the development of predictive biomarkers of patient stratification and strategies of drug combinations to improve therapeutic efficacy, especially for patients with tumors that are unresponsive to anti-PD-1/PD-L1 therapy. Taking into account the health burden of HCC worldwide, with this Special Issue, we aim to enhance our knowledge of innovative diagnostic and prognostic methods and to discuss experimentation with different novel treatment modalities, believed to be of the utmost priority to progress in our seemingly never-ending fight against hepatocellular cancer

Abstract: The huge prevalence of type 2 diabetes is a leading human and economic concern worldwide. The diagnosis code of type 2 diabetes includes multiple clinical phenotypes showing different degrees of obesity, fat distribution patterns, types of dyslipidemia, and characteristics of insulin resistance syndrome. In addition, subjects with type 2 diabetes could develop different clinical trajectories, with age being a critical factor in progression. The chronic complications of type 2 diabetes are a major cause of mortality and disability, and are ranked in the top places in western countries’ scores. The pathophysiological heterogeneity of type 2 diabetes is not captured by the current classifications and guidelines. The potential for diabetes progression and complications developing is different depending on the type 2 diabetes cluster, which is mostly regardless of the degree of glycemic control. However, the impact on quality and life expectancy with the current therapeutic approach is not always the same for every individual. Clinical research is the key to reducing the burden of diabetes. New basic, digital, and clinical approaches are obtaining useful knowledge and designing more individualized interventions to prevent, track, and treat diabetic complications. This Special Issue is a clear reflection of this methodological evolution

Abstract: Magnetic resonance imaging of the brain and spinal cord in patients with multiple sclerosis is crucial for diagnosis as well as monitoring and predicting disease activity. Comprehensive MR measurements including assessment of brain and spinal cord volume changes have the potential to improve patient monitoring, allow earlier detection of subclinical disease activity, and identify patients with depleted brain reserve who are at highest risk of disability progression. Cerebral atrophy measurements are now a secondary outcome of many clinical trials, although their wider use in routine clinical practice is still hampered by some technical limitations. However, evidence from clinical practice shows that assessment of brain and spinal cord volume change is feasible and has the potential to improve patient treatment

Abstract: The war in Syria has lasted for many years and continues to cast a dark shadow over its people, including healthcare workers caring for those injured and in distress. During the war, healthcare workers have been exposed to unprecedented challenges, becoming targets of bombing, killing, siege, arrest, and torture. Moreover, healthcare workers continue to experience difficult situations that require them to make decisions with no clear or easy moral choice. The present study aimed to understand the experiences of healthcare workers and the difficulties and challenges that hinder applying existing ethical frameworks and codes for disasters within Syria’s context of revolution and war. Qualitative analysis of interviews led to classification of the ethical challenges experienced by healthcare workers into four groups: the risks from providing care; stewardship of resources and work challenges; corruption and organizational pressure; and psychological, emotional, and social stress. The study also showed that both Syrian healthcare workers and the developers of ethical frameworks adopt many similar values and principles. Yet, the participants adopted several unique moral values not identified in ethical frameworks as a result of navigating their professional duties under the circumstances of the Syrian war. At last, recognizing the challenges and precarious tasks healthcare workers cope with during wars and other similar disasters could help volunteers, medical personnel, and humanitarian organizations deciding to work in Syria better understand the specific context and obstacles for ethical decision-making. Moreover, attracting attention for and support of Syrian healthcare workers locally and globally presents a long-lasting benefit

Abstract: In this Special Issue, we have compiled data and opinions from leading scientists to demonstrate the recent advances in the study of function and dysfunction of synucleins. These proteins are involved in multiple normal and pathological molecular processes and consequently have various effects on cell biology of the nervous system. Three members of the synuclein family share many structural properties and have an overlapping pattern of expression and intracellular distribution in the developing and adult nervous system, which point to functional redundancy within the family. However, each synuclein has its own functions which are not shared with the two other family members, and there are certain cellular mechanisms and pathways in which these functions could be antagonistic rather than synergistic

Abstract: Gastrointestinal (GI) malignancies represent a diverse spectrum of cancers with notable global incidence and prevalence. The comprehensive care of individuals affected by these malignancies is a multifaceted endeavor involving the collaboration of various medical disciplines. The nonspecific clinical manifestations of GI cancers often present a formidable diagnostic challenge to primary care providers, resulting in diagnostic delays. Subsequently, the treatment regimens, once initiated, are often demanding and prolonged, and profoundly impact the patients' overall quality of life. Given the intricate involvement of the digestive system, nutritional concerns are pervasive among these patients, influencing both their quality of life and survival. Despite these considerable challenges, the evolving landscape of treatment modalities has yielded improved outcomes, enabling a greater number of patients to achieve remission or extended survival. It is now imperative that we delve deeper into understanding the unique needs of this patient population and refine the care delivery pathways through enhanced diagnostic approaches, treatment strategies, and holistic support, and ultimately enhance the overall value of care provided to individuals afflicted by GI malignancies

Abstract: Childhood cancer or pediatric cancer is one of the leading causes of death among children. Treatment strategies for pediatric cancers include chemotherapy, surgery, radiation, and stem cell transplantation. These therapeutic regimens are well known to develop highly toxic and long-lasting side-effects including secondary cancers in the future lifespan of pediatric patients. Despite these intensive therapeutic regimens, cancer may still relapse as refractory and metastatic cancer. Therefore, further understanding the causes of different pediatric cancers, mechanisms of drug-resistance, and causes of relapse and metastasis is required for developing and advancing less toxic and more effective therapeutic strategies for pediatric cancers. Recent advancements in immunotherapies and targeted therapeutic approaches using pharmacological inhibitors are one step in this direction. In this Special Issue of Cancers, authors contributed original research and review articles focusing on different aspects of pediatric cancer development, causes, maintenance, and therapeutic strategies. The collected articles in this Special Issue will further enhance our knowledge and understanding of pediatric cancers and drive the development of novel therapeutic strategies

Abstract: Chronic obstructive pulmonary disease (COPD) is a disorder characterized by airflow limitation and is one of the major causes of mortality and morbidity across the globe. Although several guidelines have been published over the past three decades, they were generated in response to the fact that COPD is a major cause of mortality and morbidity and remains an important social problem. In the 20th century, forced expiratory volume in one second (FEV1) was the single most important measure in patients with COPD from discriminative, evaluative, and predictive standpoints. We found that dyspnea is a better mortality predictor than FEV1 in 2002 and exercise capacity in 2003. Subsequently, it has become apparent that physical activity is more predictive of mortality. Furthermore, we have recently developed the tendency to use the reduction of the future risk of exacerbation as the endpoint of relatively large-scale clinical trials rather than the improvement of FEV1. Many researchers have continued to seek better outcome markers and have also discussed what should be used as primary or secondary endpoints in clinical trials in subjects with COPD. We have undoubtedly made great progress but still have a lot of work to do for the best interests of patients

Abstract: Studies on global human genomic variation have shown important population-based differences in allele frequencies of common single-nucleotide polymorphisms (SNPs) that influence the expression of genes related to nutrition and, secondarily, chronic disease. Some SNP sites have known functions or associations with diseases or other phenotype characteristics, including nutritional deficiencies and metabolism dietary components. There are many components of human diets that, when combined with the impact of diverse genetics on the metabolism of certain nutrients, have the capacity to give rise to harmful diet–gene interactions. This situation has the potential capacity to modify molecular phenotypes and clinical phenotypes, including human disease. Obesity, diabetes mellitus, chronobiology, osteoporosis, cancer, and many other diseases are fields of potential investigation in this topic area. This Special Issue will include manuscripts that focus on the complex relationship between gene polymorphisms and nutrition across all physiological and chronic diseases

Abstract: Conventional ophthalmic formulations lack prolonged drug release effects and mucoadhesive properties, decreasing their residence time in the precorneal area and, therefore, in drug penetration across ocular tissues, presenting a low bioavailability with a consequent reduction in the therapeutic efficacy. These limitations are related to the physiological mechanisms of the eye, for example, the diluting effect of formulations exerted by the tear fluid, constant turnover of the tear fluid, blink reflex, limited capacity of the conjunctival sac, and drainage of formulations applied by gravity. To increase the residence time of formulations on the surface of ocular tissues and increase their ability to penetrate these tissues, different strategies can be used, namely, the use of viscosifying agents, mucoadhesive polymers, stimuli-responsive polymers, microparticles, and colloidal carriers (e.g., micelles, liposomes, nanosuspensions, nanoemulsions, polymeric, and lipid nanoparticles). This Special Issue combines the latest research and review works reporting the use of nanoparticles in ophthalmic formulations to increase their bioavailability and improve the therapeutic efficacy of ophthalmic formulations
