    Abstract: When adopting remote sensing techniques in precision agriculture, there are two main areas to consider: data acquisition and data analysis methodologies. Imagery and remote sensor data collected using different platforms provide a variety of information volumes and formats. For example, recent research in precision agriculture has used multispectral images from different platforms, such as satellites, airborne, and, most recently, drones. These images have been used for various analyses, from the detection of pests and diseases, growth, and water status of crops to yield estimations. However, accurately detecting specific biotic or abiotic stresses requires a narrow range of spectral information to be analyzed for each application. In data analysis, the volume and complexity of data formats obtained using the latest technologies in remote sensing (e.g., a cube of data for hyperspectral imagery) demands complex data processing systems and data analysis using multiple inputs to estimate specific categorical or numerical targets. Although these secondary causes of osteoporosis are the most frequently observed causes of unexpected bone loss, they can only be diagnosed via a high degree of suspicion and clinical experience and by performing the appropriate investigations. In inflammatory disorders such as rheumatoid arthritis or chronic inflammatory bowel diseases, as well as vascular diseases, T-cell activation, and consequently pro-inflammatory cascades, trigger the increased expression of T-cell-derived RANKL. In addition, a new biomarker signature of bone-related miRNAs is promising in certain clinical features. Glucocorticoids, often used to control disease activity, decrease the number and function of osteoblasts and inhibit OPG expression. The ubiquitous occurrence of disease-related secondary changes in bone metabolism implies that numerous medical disciplines need to interact. Screening for secondary causes of osteoporosis and the search for new modes of action should present a substantial aspect of osteoporosis management.     Abstract: As Guest Editors, we are very delighted to see the reprint of our Special Issue, “Head and Neck Surgery: Clinical Updates and Perspectives", which is published in the Otolaryngology section of the Journal of Clinical Medicine. These papers are the joint effort of many excellent colleagues who have devoted their lives and careers to the exciting field of Otorhinolaryngology. As both clinicians and surgeons, we are actively advancing the diagnosis and management of the myriad of conditions affecting the head and neck region. Unprecedented discoveries in the field of genetics and molecular biology are always integrated with sound knowledge of the intricate anatomy of our discipline, while technological advancements now permit us to provide tailored and minimally invasive treatments for our patients.     Abstract: Chronic obstructive pulmonary disease (COPD) is a disorder characterized by airflow limitation and is one of the major causes of mortality and morbidity across the globe. Although several guidelines have been published over the past three decades, they were generated in response to the fact that COPD is a major cause of mortality and morbidity and remains an important social problem. In the 20th century, forced expiratory volume in one second (FEV1) was the single most important measure in patients with COPD from discriminative, evaluative, and predictive standpoints. We found that dyspnea is a better mortality predictor than FEV1 in 2002 and exercise capacity in 2003. Subsequently, it has become apparent that physical activity is more predictive of mortality. Furthermore, we have recently developed the tendency to use the reduction of the future risk of exacerbation as the endpoint of relatively large-scale clinical trials rather than the improvement of FEV1. Many researchers have continued to seek better outcome markers and have also discussed what should be used as primary or secondary endpoints in clinical trials in subjects with COPD. We have undoubtedly made great progress but still have a lot of work to do for the best interests of patients

    Abstract: Nurses represent the highest number of healthcare workers globally and have played a critical role in the COVID-19 pandemic. This reprint highlights the issues and challenges faced by nurses practicing in hospital and community settings during the COVID-19 pandemic. The manuscripts herein focus on nurses in various areas of practice, including nurse managers, highlighting the impact, experience, and reality of providing care during the crisis. Interventions and strategies to support practice environments and the mental health and wellbeing of the nursing workforce were identified. Evidence from these manuscripts shed light on how nurse leaders can provide relevant, coordinated, and consistent organizational and leadership support to better establish a safe and healthy work environment that protects and fosters the mental health and wellbeing of all nursing personnel. The pandemic exacerbated the already demanding quality of work environments for nurses, as well as their mental health and wellbeing, thereby inducing a global nursing shortage; therefore, improving these factors, which ultimately influence nurse retention, is critical. We trust that sharing the lived experiences of nurses during the COVID-19 pandemic can enable us to leverage their expertise in meeting the complex care needs of patients, including COVID-19 patients, while improving the care needed by nurses in their working environments, alongside preparing for future pandemic waves

    Abstract: The huge prevalence of type 2 diabetes is a leading human and economic concern worldwide. The diagnosis code of type 2 diabetes includes multiple clinical phenotypes showing different degrees of obesity, fat distribution patterns, types of dyslipidemia, and characteristics of insulin resistance syndrome. In addition, subjects with type 2 diabetes could develop different clinical trajectories, with age being a critical factor in progression. The chronic complications of type 2 diabetes are a major cause of mortality and disability, and are ranked in the top places in western countries’ scores. The pathophysiological heterogeneity of type 2 diabetes is not captured by the current classifications and guidelines. The potential for diabetes progression and complications developing is different depending on the type 2 diabetes cluster, which is mostly regardless of the degree of glycemic control. However, the impact on quality and life expectancy with the current therapeutic approach is not always the same for every individual. Clinical research is the key to reducing the burden of diabetes. New basic, digital, and clinical approaches are obtaining useful knowledge and designing more individualized interventions to prevent, track, and treat diabetic complications.     Abstract: The advent of optical coherence tomography (OCT) has arrived in an era in which the in vivo, cross-sectional imaging of both the retina and choroid is possible. Infra-red rays of light are used to image the chorioretinal layers with an image resolution reaching 10-15 microns. During the last three decades, OCT technology has undergone several improvements, such as spectral domain OCT (SD-OCT), swept source OCT (SS-OCT), enhanced depth imaging (EDI-OCT), wide field OCT, intraoperative OCT, OCT angiography (OCTA), and adaptive optics OCT. These enhancements help in both the qualitative and quantitative analysis of chorioretinal parameters in pathologies such as macular edema, neurosensory detachment, retinoschisis, macular holes, epiretinal membranes, and choroidal tumors. An added advantage is the role of OCT in the evaluation of optic disc disorders, including optic neuritis and glaucoma, via the quantitative analysis of the retinal nerve fiber layer. Moreover, OCT being non-invasive provides an opportunity for serial imaging during follow-up visits. Overall, OCT imaging provides tissue details, closely mimicking the histopathology, and is an invaluable asset in the armamentarium of retina physicians.     Abstract: Hepatocellular carcinoma (HCC) is the most prevalent primary liver cancer, accounting for 〉80% of primary liver cancers worldwide. The diagnosis of HCC, relying solely on noninvasive criteria, is currently under debate due to the need for molecular information that necessitates tissue biopsies, while therapy is provided in accordance with tumor stages and the anticipated advantages of major interventions, following the Barcelona Clinic Liver Cancer (BCLC) staging system. Principally, resection, transplantation and local ablation are most commonly performed in patients with early-stage HCC tumors, while TACE and systemic therapy are, respectively, the preferred treatment options for intermediate- and advanced-stage tumors. However, the HCC tumor immune microenvironment (TME) affects the response to current anti-PD-1/PD-L1 immunotherapies. Therefore, an enhanced understanding of the immunobiology of TME is essential for the development of predictive biomarkers of patient stratification and strategies of drug combinations to improve therapeutic efficacy, especially for patients with tumors that are unresponsive to anti-PD-1/PD-L1 therapy. There are several types of medical sensors that can be utilized for various applications, such as temperature probes, force sensors, pressure sensors, oximeters, electrocardiogram sensors that measure the electrical activity of the heart, heart rate sensors, electroencephalogram sensors that measure the electrical activity of the brain, electromyogram sensors that record electrical activity produced by skeletal muscles, and respiration rate sensors that count how many times the chest rises in a minute. The output of these sensors used to be interpreted by humans, which was time consuming and tedious; however, such interpretations became easy with advances in artificial intelligence (AI) techniques and the integration of the sensor outputs into computer-aided diagnostic (CAD) systems. This reprint presents some of the state-of-the-art AI approaches that are used to diagnose different diseases and disorders based on the data collected from different medical sensors. Also, novel non-invasive biomarkers for endoscopic activity and mucosal healing in ulcerative colitis, IBD, and cell-death-associated inflammatory syndromes are proposed. Moreover, a certain subset of telocytes is suggested as the pilot governor of epithelial behavior in preneoplastic gastric lesions, a newer member of IL-1 family, as it is a critical partner in lung adenocarcinoma, while distinct transcriptomic signatures and pathway profiles in human melanocytes corresponding to different Staphylococcal superantigenic enterotoxins are possible targets for therapeutic intervention in cases of toxic shocks.     Abstract: This reprint focuses on personalized medicine in ophthalmic diseases, their challenges and opportunities, which often involves methods to achieve personalized diagnosis and treatment of ophthalmic diseases. Personalized medicine is a broadly used term to encompass approaches used to tailor healthcare to the needs of individual patients. It has been early adopted in ophthalmology and is mainly achieved through disease stratification and individualization. Therefore, diagnostic techniques that can realize comprehensive individual assessment are very important. Previous studies have put forward techniques such as next-generation sequencing and translational research. Gene therapy-based treatment trials have been presented for ophthalmic diseases such as retinitis pigmentosa and age-related macular degeneration. Recently, with the rapid development of artificial intelligence and interdisciplinary collaboration, concepts such as machine learning and wearable devices have been frequently discussed in ophthalmic research.     Abstract: Conventional ophthalmic formulations lack prolonged drug release effects and mucoadhesive properties, decreasing their residence time in the precorneal area and, therefore, in drug penetration across ocular tissues, presenting a low bioavailability with a consequent reduction in the therapeutic efficacy. These limitations are related to the physiological mechanisms of the eye, for example, the diluting effect of formulations exerted by the tear fluid, constant turnover of the tear fluid, blink reflex, limited capacity of the conjunctival sac, and drainage of formulations applied by gravity.To increase the residence time of formulations on the surface of ocular tissues and increase their ability to penetrate these tissues, different strategies can be used, namely, the use of viscosifying agents, mucoadhesive polymers, stimuli-responsive polymers, microparticles, and colloidal carriers (e.g., micelles, liposomes, nanosuspensions, nanoemulsions, polymeric, and lipid nanoparticles).     Abstract: Humans have eaten nuts for thousands of years, yet their consumption was discouraged just a few decades ago due to their high-fat content. This perspective began to change in the early 1990s due to seminal publications from the Adventist Health Study, which showed that nut consumption significantly improved heart health-related outcomes. Many randomized clinical trials, epidemiological studies, and in vitro/in vivo mechanistic studies have since explored the role of eating nuts and its relation to health. Similarly, dried fruits have been scrutinized due to their concentrated sugar content despite their non-sodium micronutrient and fiber density. Due to accumulating evidence on nut and dried fruit intake and health outcomes, an international scientific meeting was organized for leading experts to examine and recapitulate in detail what is well known and established and what avenues of knowledge are still lacking in nut and dried fruit research. These findings highlight the beneficial health potentials of nuts for researchers, health professionals, other stakeholders, and the public while simultaneously pinpointing areas for further investigation to continue informing and guiding health practices and policies

    Abstract: Tryptophan is a rate-limiting essential amino acid and a unique building block of peptides and proteins. This largest amino acid serves as the precursor for the important endogenous indoleamines serotonin, N-acetylserotonin, and melatonin that act as neurotransmitters, neuromodulators, and neurohormones. Kynurenic acid is the most potent endogenous antiexitotoxic agent. Other highly relevant pathways of tryptophan are the reversible transamination to indole-3-pyruvate with formation related indolic acids that act as potent antioxidant agents. Tryptophan metabolites, such as melatonin, and structurally related agents, such as indole-3-propionic acid, act as potent catalytic antioxidants and bioenergetic agents that facilitate regeneration and protection against stress and aging. Several indole compounds act as uremic toxins since these agents can induce radical formation that is associated with enhanced oxidative stress and damage. The exploration of the effects of these protective and toxic tryptophan derived agents has revealed important molecular mechanisms and mediators of adaptation and aging. Research on tryptophan in nutrition and health can facilitate the development of new approaches to extend human health and life span. Amino acids are the building blocks of life that enable repair, as well as recycling and regeneration.     Abstract: Psychopathology, including functional psychiatric disorders, such as schizophrenia, bipolar disorder, major depressive disorder and anxiety disorders, and also dementia and challenging behaviour, is common in people with neurodevelopmental disorders (NDDs), such as intellectual and developmental disabilities (IDD), autism spectrum disorder (ASD) and attention deficit hyperactivity disorder (ADHD), and is the cause for a major public health concern. Much research has been conducted in the last three decades to understand this issue and develop effective interventions. Still, a lot remains unknown, for which high-quality translational research is urgently needed. In this Special Issue, we drew together research on recent advances addressing these issues. The areas covered included life expectancy among people with and without intellectual disabilities, the social and emotional development of people with intellectual disabilities and ASD, the influence of external environmental factors in determining aggressive behaviour in people with intellectual disabilities, parent and child training for children with autism and intellectual disabilities, how the presence of deafness influences quality of life, the views of prescribers on the deprescribing of antipsychotics among adults with intellectual disabilities, the views of people with intellectual disabilities who took part in the deprescribing, the views of direct-care workers on the causes of challenging behaviours in people with intellectual disabilities, and the validation of tools to detect cognitive decline in adults with intellectual disabilities

    Abstract: Nanomedicine is a special medical field focused on the application of nanotechnology to provide innovations for healthcare in different areas, including the treatment of a wide variety of diseases, including cancer, infections, and auto-immune disorders. The field emerged during the 1980s, aligning with the approval of the first regulatory-agency-approved nanomedical oncological drugs. Additionally, nanotechnology has played a pivotal role in the development of mRNA vaccines utilized during the COVID-19 pandemic, further establishing its enduring significance in the domains of science and biomedical innovation. The reasons for the use of nanotechnology in biomedicine can vary but are mostly the protection and/or delivery of bioactive molecules to target tissues. The general idea is to create nanoscopic platforms that can interact differently with biological systems, either through pharmacokinetic modifications or through the preferential activation of some biological pathway. The intricate biomolecular interactions that underlie the functioning of the biological system take place within the nanoscale. So, nanoparticles can be designed to be similar in size to key biological structures like large biomolecules or small organelles, enabling them to interact with biological systems in unique ways, which can improve the effects of drugs used in the treatment of certain diseases. In other words, highly distinct and improved biological effects can emerge from such interactions

    Abstract: Coronavirus disease 2019 (COVID-19) is a new respiratory disease caused by infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) that caused a global pandemic. COVID-19 patients often develop acute respiratory distress syndrome (ARDS), which involves acute and lethal respiratory failure. In a meta-analysis, the median incidence of ARDS in COVID-19 patients was found to be 35 (2-79)%, while the mortality rate in COVID-19-associated ARDS patients was found to be 39 (12.5-73.1)% (Hasan, et al. Expert Rev Repoir Med. 2020; 14: 1149). As of 12 December 2021, 270,373,764 cases had been confirmed as COVID-19 and 5,321,321 cases had resulted in death, according to the World Health Organization webpage. Various symptoms such as persistent fatigue, myalgia, and insomnia can persist after recovery from COVID-19; these symptoms are called “long COVID-19”. Patients with COVID-19 who survive but develop pulmonary fibrosis may experience persistent dyspnea upon exertion, a decrease in pulmonary function, and/or a radiological abnormal shadow. Antifibrotic drugs including nintedanib and pirfenidone are candidate drugs for the treatment of pulmonary fibrosis due to COVID-19; randomized controlled trials are ongoing. However, there are limited studies about respiratory disease caused by COVID-19. Given the clinical significance of this topic and its impact on clinical practice and public health, Medicina have published a Special Issue entitled “Respiratory Disease in the COVID-19 Era” with the aim of gathering accurate and up-to-date scientific information of this topic
